How Long Does Seroquel (Quetiapine) Stay in Your System?

Seroquel (quetiapine) is mostly cleared from your body within about 1.5 days after your last dose, though its active byproduct can linger closer to 3 days. The exact timeline depends on whether you take the immediate-release or extended-release version, your liver function, and how long you’ve been on the medication.

Half-Life and Full Elimination

The immediate-release form of Seroquel has a half-life of roughly 6 hours, meaning half the drug is broken down and removed every 6 hours. The extended-release version (Seroquel XR) is slightly longer at about 7 hours. A drug is considered essentially eliminated after 5 to 6 half-lives, which gives you a rough clearance window of 30 to 36 hours for the immediate-release and 35 to 42 hours for the extended-release.

That’s only part of the picture, though. Your liver converts quetiapine into an active metabolite called norquetiapine, which has its own half-life of about 12 hours. Because this byproduct is still pharmacologically active, it takes around 60 to 72 hours (about 3 days) before both the original drug and its metabolite are functionally gone from your bloodstream.

How Your Body Processes Seroquel

Quetiapine is broken down almost entirely in the liver, primarily by an enzyme system called CYP3A4. This means anything that affects CYP3A4 activity can speed up or slow down how quickly you clear the drug. Certain medications, like some antifungals and HIV protease inhibitors, block CYP3A4 and can cause quetiapine to build up to higher levels and take longer to leave. On the other hand, drugs like certain anti-seizure medications and the antibiotic rifampin ramp up CYP3A4 activity and can clear quetiapine faster than expected.

Grapefruit juice is another well-known CYP3A4 inhibitor. Drinking large amounts while taking Seroquel could modestly slow elimination, though this is more relevant to drug levels during treatment than to how long it stays in your system after stopping.

Factors That Slow Clearance

Because the liver does nearly all the work of metabolizing quetiapine, liver health is the single biggest variable. People with significant liver impairment will clear the drug more slowly, and their effective elimination window can stretch well beyond the typical 1.5 to 3 day range.

Age also plays a role. Older adults generally have reduced liver enzyme activity and slower blood flow to the liver, both of which extend clearance time. Higher doses and longer durations of use can also mean the drug has accumulated more in your tissues, adding time to the tail end of elimination. Body composition matters too: quetiapine is lipophilic (it dissolves into fat tissue), so individuals with a higher percentage of body fat may retain traces somewhat longer.

Detection in Drug Tests

Standard workplace and clinical drug panels don’t test for quetiapine specifically. However, Seroquel is known to trigger false positives on immunoassay-based urine screens for two substances: methadone and tricyclic antidepressants. This happens because quetiapine’s chemical structure is similar enough to these compounds that the test’s antibodies cross-react with it. If you get a positive result and you’re taking Seroquel, a confirmatory test using more precise methods (like mass spectrometry) will distinguish quetiapine from an actual positive.

Detection Windows by Sample Type

In urine, quetiapine can be detected for roughly 2 days after a single dose using sensitive laboratory techniques. In blood, the window is similar but concentrations drop more quickly, with levels becoming very low within about 43 hours of ingestion based on forensic case data. Hair testing has a much longer window. In one documented forensic case, quetiapine was detectable in a hair segment corresponding to the time of exposure even 6 months later, at a concentration of 0.011 nanograms per milligram of hair. Hair analysis isn’t part of routine screening, but it can confirm past exposure months after the drug has left the blood and urine.

What This Means After Stopping

If you’ve been taking Seroquel at a steady dose for weeks or longer, the drug reaches a consistent level in your bloodstream (called steady state). Once you stop, the clock on those half-lives begins. Most people will have cleared the parent drug within about a day and a half, and the active metabolite within about three days. Residual effects like drowsiness or grogginess can sometimes persist a bit beyond that window, especially if you were on a higher dose, because it takes time for your brain’s receptor activity to readjust after the drug is gone.

For the extended-release formulation, expect the timeline to run slightly longer since the tablet is designed to release quetiapine more gradually. The difference is modest, adding roughly half a day to full clearance compared to the immediate-release version.
