Strattera (atomoxetine) stays in your system for roughly 1 to 2 days for most people, but it can linger for up to 4.5 days if your body processes it more slowly. The difference comes down to your genetics and liver function, which determine how quickly you break the drug down.
Half-Life: The Key Number
The half-life of a drug is how long it takes your body to eliminate half of it. For Strattera, that number varies dramatically between two groups of people. About 90% of the population are “extensive metabolizers,” meaning their liver enzymes break down atomoxetine efficiently. In this group, the half-life is about 5.2 hours. The remaining roughly 7% to 10% are “poor metabolizers” with a half-life of 21.6 hours, more than four times longer.
The enzyme responsible is called CYP2D6. If you have two working copies of the gene for this enzyme, you clear the drug quickly. If both copies are reduced or nonfunctional, the drug hangs around much longer. Most people don’t know which group they fall into unless they’ve had pharmacogenomic testing.
Total Clearance Time
A drug is generally considered cleared from your system after about five half-lives, when less than 3% of the original dose remains. For extensive metabolizers, that math works out to roughly 26 hours, just over a day. For poor metabolizers, full clearance takes closer to 108 hours, or about 4.5 days.
Keep in mind these are estimates for the active parent drug. Your body converts atomoxetine into inactive byproducts (primarily a compound that gets tagged for removal by the kidneys). Those metabolites may be detectable in urine for somewhat longer, even though they aren’t producing any therapeutic effect. Over 80% of a dose leaves through the urine, with a small fraction (2% to 17%, depending on your metabolizer status) exiting through feces. Less than 3% of the drug comes out unchanged; almost all of it is broken down first.
Liver Function Makes a Big Difference
Because the liver does nearly all the work of processing Strattera, any impairment in liver function slows clearance significantly. In people with moderate liver problems, the body’s ability to clear atomoxetine drops to about half the normal rate, effectively doubling how long the drug stays in your system. In severe liver impairment, clearance falls to one quarter of normal, meaning the drug can persist roughly four times longer than expected. If you have a liver condition, your prescriber will typically adjust your dose to account for this slower processing.
Strattera and Drug Testing
Strattera is not a stimulant and is not a controlled substance. Standard workplace and sports drug panels screen for amphetamines, opioids, cannabinoids, and similar substances. Atomoxetine does not belong to any of those categories and will not trigger a positive result on a standard urine drug screen. Specialized laboratory tests could detect it if someone were specifically looking for it, but this is not part of routine testing.
There have been rare reports of atomoxetine causing a false positive for amphetamines on certain immunoassay screens, though confirmatory testing (which labs run automatically when a positive pops up) will rule it out. If you’re concerned, having your prescription documented is sufficient.
What This Means If You Miss a Dose or Stop
Because Strattera’s therapeutic benefit builds over weeks of consistent use (unlike stimulant ADHD medications, which work dose by dose), a single missed dose won’t cause a dramatic drop in effectiveness. The drug itself leaves your bloodstream within a day or two, but its effects on brain chemistry take longer to fully wear off. Most people notice symptoms gradually returning over several days to a week after stopping.
If you’re discontinuing Strattera, the active drug will be out of your body within a day for most people, or about five days if you’re a slow metabolizer. There is no formal tapering requirement listed in the prescribing information, but some people experience a return of ADHD symptoms or mild discomfort when stopping abruptly, so a gradual reduction over a week or two is a common approach.
Factors That Can Slow Elimination
- CYP2D6 poor metabolizer status: Extends the half-life from about 5 hours to nearly 22 hours.
- Liver disease: Moderate impairment roughly doubles clearance time; severe impairment can quadruple it.
- Medications that inhibit CYP2D6: Certain antidepressants (like fluoxetine and paroxetine) block the same enzyme that breaks down Strattera, effectively turning an extensive metabolizer into a poor one.
Age and kidney function play smaller roles. Because the kidneys excrete mostly inactive metabolites rather than the active drug itself, kidney impairment has less impact on how long the active medication stays in your system.
