Despite this, trazodone remains one of the most commonly prescribed medications for sleep in the United States, partly because it’s not a controlled substance and carries less concern about dependence than drugs like zolpidem or benzodiazepines.

One trade-off worth knowing about: trazodone is associated with a higher fall risk than several alternatives. In comparative data, the fall rate for trazodone users was 5.27 per 100 person-years, compared to 2.55 for extended-release zolpidem and 3.85 for benzodiazepines. This is particularly relevant for older adults or anyone who gets up to use the bathroom at night, since trazodone can cause dizziness and a drop in blood pressure when standing.

The core difference is that purpose-built sleep medications like zolpidem are designed to target sleep circuits more precisely, while trazodone’s sedation is essentially a side effect of its broader receptor activity.
