The goal during dose titration (when your prescriber helps you find the right amount) is to land on a dose that produces a firm erection lasting long enough for intercourse but not so long that it becomes a problem. Most providers aim for an erection that resolves on its own within 60 to 90 minutes.
How Your Body Breaks It Down
Trimix contains three active compounds: alprostadil (a synthetic prostaglandin), papaverine (a smooth muscle relaxant), and phentolamine (which blocks certain receptors in blood vessel walls). Each one is metabolized differently, but all three are cleared relatively quickly.
Alprostadil is either broken down locally in the penile tissue or enters the bloodstream and is extensively metabolized on its first pass through the lungs. Systemic Effects and How Long They Last
Some men experience dizziness, lightheadedness, flushing, or a temporary drop in blood pressure after injecting Trimix. These effects reflect the small amount of medication that does enter general circulation. They typically resolve within 30 to 60 minutes as the active ingredients are metabolized. Staying hydrated, injecting while seated, and avoiding standing up too quickly can reduce the chance of feeling faint.
How Dosage Changes the Timeline
Higher doses keep the drug active in penile tissue for longer, which extends erection duration and increases the window of time the compounds are present in your system. In one study, 16 control participants experienced erections lasting beyond 4 hours when given a full 1 cc dose of Trimix, compared to only 8 who had the same response at 0.25 cc. When an Erection Lasts Too Long
An erection lasting 4 hours or more that doesn’t resolve on its own or after ejaculation is classified as priapism, and it requires emergency treatment. At the 4-hour mark, the risk of permanent tissue damage to the erectile chambers begins to climb.
