A single drop of Upneeq typically lifts the eyelid for about 6 to 8 hours. The effect peaks around 2 hours after you put the drop in and gradually tapers from there. It’s a once-daily eye drop, so you get one window of improvement per dose.
How Quickly It Starts Working
Upneeq starts lifting the eyelid within about 5 minutes of instillation. In phase 3 clinical trials, researchers measured significant eyelid elevation beginning at that 5-minute mark, with the maximum lift occurring roughly 2 hours after the dose. That rapid onset is one of the main selling points: you can put in a drop and see a visible difference before you leave the house.
The 6-to-8-Hour Window
According to the FDA-approved prescribing information, the eyelid lift continues through at least 8 hours after a dose. Clinical trials confirmed that both the physical lift and the improvement in upper visual field were still statistically significant at the 6-hour mark. Beyond 6 hours, the effect starts to wind down, though some people report noticing a difference slightly longer.
The practical takeaway: if you have something important in the evening, you’ll get more mileage from applying Upneeq in the afternoon rather than first thing in the morning. Most people time their drop around whatever part of the day matters most to them.
How It Works
Upneeq contains a low concentration of oxymetazoline, the same active ingredient found in certain nasal decongestant sprays, but formulated specifically for the eye at a much lower strength (0.1%). When applied as an eye drop, it stimulates a small smooth muscle in the upper eyelid called Müller’s muscle. This muscle contracts, pulling the eyelid up by a few millimeters. Once the drug wears off, the muscle relaxes and the lid returns to its resting position. That’s why the effect is temporary and needs to be repeated daily.
Dosing: Once a Day, No More
Upneeq is approved for one drop per affected eye, once daily. Each dose comes in a single-use vial that should be thrown away immediately after use, even if liquid remains. You cannot save the leftover solution for later. Unopened vials should be stored at room temperature and kept away from excessive heat.
Using it more than once a day isn’t recommended. Oxymetazoline can cause rebound effects when overused (a well-known issue with nasal sprays containing the same ingredient), and applying extra doses could reduce effectiveness over time or increase the risk of side effects like eye redness and dryness.
Who It’s Designed For
Upneeq is FDA-approved specifically for acquired ptosis, the age-related drooping of one or both upper eyelids that develops over time. It was not studied in people with congenital ptosis (present from birth) or ptosis caused by nerve damage or muscle disease. If your drooping is caused by something other than normal aging or wear on the eyelid muscle, the drop may not produce the same results.
Precautions Worth Knowing
Upneeq has no absolute contraindications listed on its label, but it does carry several caution flags. People with unstable cardiovascular disease, uncontrolled high or low blood pressure, or narrow-angle glaucoma should discuss risks with their prescriber before using it. The same goes for anyone taking beta-blockers, blood pressure medications, cardiac glycosides, or MAO inhibitors, all of which can interact with oxymetazoline.
The drop should also be kept out of reach of children. Accidental oral ingestion of oxymetazoline-containing solutions has caused serious reactions in kids, including dangerously slowed breathing, changes in heart rate, and sedation severe enough to require hospitalization.
What to Realistically Expect
Upneeq offers a modest but noticeable lift. For many people, a couple of millimeters is the difference between looking alert and looking tired, or between a partially blocked upper visual field and a clear one. But it’s not a substitute for surgical ptosis repair if the drooping is severe. Think of it as a daily maintenance tool: quick, noninvasive, and temporary. The effect resets every 24 hours, and you’ll need to keep using it each day to maintain the lift.
