Valium (diazepam) can be detected in urine for anywhere from a few days to six weeks, depending on how often and how long you’ve been taking it. After a single dose, most people will test positive for roughly 10 to 14 days. Chronic or heavy use extends that window significantly, sometimes to four to six weeks or longer.
The reason for such a wide range comes down to Valium’s unusually slow metabolism. It produces active byproducts that linger in your body far longer than the drug itself, and those byproducts are exactly what urine tests look for.
Why Valium Stays Detectable So Long
Valium is one of the longest-lasting benzodiazepines on the market. The drug itself has an elimination half-life of about 120 hours, meaning it takes roughly five days for your body to clear just half of a single dose. But Valium doesn’t simply disappear. Your liver converts it into several active metabolites, and one of those, nordiazepam, has a half-life that averages around 310 hours (about 13 days). Another breakdown product, oxazepam, is the metabolite that persists longest in urine.
In a controlled study of 11 healthy volunteers who each took a single 10 mg dose of Valium, researchers tracked urine concentrations for two weeks. The median time oxazepam remained detectable was 252 hours, or about 10.5 days. The range across participants stretched from roughly 8.5 to 13.5 days. Diazepam itself was detectable in urine for about 11 days. That’s from one pill in a healthy person, which illustrates just how persistent this drug is compared to shorter-acting benzodiazepines.
Detection Windows by Usage Pattern
Your detection window depends heavily on whether you took Valium once or have been using it regularly. Here’s what to expect:
- Single use: 1 to 2 weeks
- Occasional use: 1 to 2 weeks
- Regular use: 2 to 4 weeks
- Chronic or heavy use: 4 to 6 weeks, sometimes longer
With repeated dosing, Valium and its metabolites accumulate in your body’s fat tissue. Each new dose adds to the reservoir before the previous one has fully cleared. Over weeks or months of regular use, this buildup means your body has a much larger total amount to process once you stop, and clearance can take well over a month.
Factors That Speed Up or Slow Down Clearance
Not everyone metabolizes Valium at the same rate. Several biological factors shift the timeline in meaningful ways.
Age is one of the strongest predictors. Research examining diazepam pharmacokinetics found that age and liver disease were equally important in determining how long the drug stays active. Older adults process Valium more slowly, partly because liver enzyme activity declines with age and partly because body composition changes. A 70-year-old may take significantly longer to clear the same dose than a 25-year-old.
Body fat percentage matters because diazepam is highly fat-soluble. It gets absorbed into fatty tissue and then slowly released back into the bloodstream over time. People with higher body fat tend to store more of the drug and release it more gradually, extending the detection window.
Liver function is the single most important factor in how quickly your body actually breaks down and eliminates diazepam. People with liver disease, particularly cirrhosis, show significantly reduced clearance rates. The drug’s volume of distribution (how widely it spreads through the body) also increases with liver impairment, compounding the delay. Sex also plays a role in how the drug distributes through the body, though it’s a smaller factor than liver health or age.
Hydration, kidney function, and overall metabolic rate round out the picture, but they have a more modest effect compared to the factors above.
How Urine Tests Detect Valium
Standard drug tests don’t look for diazepam directly. Instead, they screen for a class of compounds: benzodiazepine metabolites. The initial screening test is an immunoassay, which flags a sample as positive if benzodiazepine metabolites exceed 100 nanograms per milliliter. This is the cutoff recommended by the scientific community for benzodiazepines, though it’s worth noting that federal workplace guidelines (HHS) don’t set a mandatory threshold for benzodiazepines because they can be legally prescribed.
Immunoassay screens are fast and inexpensive, but they aren’t perfect. Their sensitivity for detecting benzodiazepines ranges from about 36% to 75%, depending on the specific testing method used. That means some true positives can be missed, and some results need a closer look. When a screening test comes back positive, labs typically run a confirmatory test using gas chromatography-mass spectrometry (GC-MS), which identifies the exact drug and its metabolites at much lower concentrations. GC-MS is particularly better at picking up low-level benzodiazepine concentrations that immunoassays might miss.
False Positives on Benzodiazepine Screens
If you haven’t taken Valium or any benzodiazepine and still test positive, a false positive is possible but uncommon. The anti-inflammatory drug oxaprozin is the most well-documented cause. The antidepressant sertraline (Zoloft) has also been flagged as a potential culprit, though the concentrations needed to trigger a false positive are at least 200 times higher than what you’d see with a normal therapeutic dose. In practice, this makes a sertraline-related false positive extremely unlikely for someone taking the medication as prescribed.
If you receive a positive result you believe is incorrect, a confirmatory GC-MS test can distinguish between actual benzodiazepine metabolites and cross-reacting compounds. This second test is highly specific and resolves nearly all false positive cases.
Valium Compared to Other Benzodiazepines
Valium has one of the longest detection windows of any benzodiazepine. Shorter-acting drugs like alprazolam (Xanax) or lorazepam (Ativan) typically clear from urine within a few days of a single dose. Valium’s extended timeline is a direct consequence of its long half-life and the fact that it produces active metabolites with even longer half-lives.
