Valtrex (valacyclovir) is mostly cleared from your body within about 15 to 20 hours of your last dose, assuming healthy kidney function. The drug itself disappears from your bloodstream within roughly 3 hours, but it converts into an active form called acyclovir, which has a half-life of 2.5 to 3.3 hours. That means acyclovir levels drop by half every 2.5 to 3.3 hours until the drug is essentially gone.
How Valtrex Is Processed in Your Body
When you swallow a Valtrex tablet, your intestines and liver rapidly convert it into acyclovir, the compound that actually fights the virus. This conversion happens so quickly that the original valacyclovir form becomes undetectable in your blood within about 3 hours. Acyclovir then circulates through your bloodstream, doing its antiviral work, before your kidneys filter it out.
Oral valacyclovir delivers about 54% of its acyclovir into your bloodstream. That’s three to five times more than taking acyclovir directly as a pill, which is the whole reason valacyclovir was developed. It’s a more efficient delivery system for the same active drug. Acyclovir levels in your blood peak about 2 hours after you take a dose, then steadily decline as your kidneys clear it.
The Math Behind Total Clearance
A drug is generally considered cleared after about 5 to 6 half-lives. With acyclovir’s half-life averaging 2.5 to 3.3 hours, that works out to roughly 13 to 20 hours for over 97% of the drug to leave your system. After a single dose, most people will have negligible amounts remaining within a day.
If you’ve been taking Valtrex on a regular schedule for suppressive therapy, the drug doesn’t build up significantly in your body. Because the half-life is short, each dose is largely cleared before or shortly after the next one arrives. Once you stop taking it, the same 15 to 20 hour window applies from your final dose.
Kidney Function Changes Everything
Your kidneys do the heavy lifting when it comes to removing acyclovir. If your kidneys aren’t working well, the drug stays in your system much longer. In people with end-stage renal disease, the half-life of acyclovir jumps to approximately 14 hours. That means full clearance could take 3 to 4 days instead of less than one. During hemodialysis, the half-life drops back down to about 4 hours because the dialysis machine helps filter the drug out.
This is why dosing adjustments are standard for people with kidney problems. The relationship is straightforward: the less efficiently your kidneys filter, the longer acyclovir lingers in your blood at higher concentrations.
Age Affects How Quickly It Clears
Children and older adults process Valtrex differently than young, healthy adults. Kids generally clear acyclovir faster because their kidneys work more efficiently relative to their body size, particularly between ages 3 months and 6 years. As children get older, their clearance rate gradually slows and approaches adult levels by around age 12.
Older adults experience the opposite effect. Acyclovir concentrations tend to run higher in geriatric patients because kidney function naturally declines with age. Even in older adults with no diagnosed kidney disease, the drug may take somewhat longer to fully clear compared to a 30-year-old with the same dose.
Why the Dosing Schedule Matters
Because Valtrex clears relatively quickly, it needs to be taken at regular intervals to maintain effective levels in your body. The dosing depends on what’s being treated. For a first outbreak of genital herpes, the standard regimen is 1 gram twice a day for 7 to 10 days. For recurrent episodes, shorter courses are used, such as 500 mg twice daily for 3 days. Suppressive therapy to prevent outbreaks uses 500 mg to 1 gram once daily on an ongoing basis.
For shingles, doses are taken three times daily to keep acyclovir levels consistently high enough to fight the virus. The frequent dosing isn’t because the drug accumulates or is hard to clear. It’s the opposite: because it leaves your system quickly, you need to keep replenishing it.
Valtrex and Drug Testing
Valacyclovir is an antiviral medication, not a controlled substance. It does not appear on standard drug test panels, whether 5-panel, 10-panel, or 12-panel screens. These tests look for classes of drugs like opioids, amphetamines, benzodiazepines, and cannabis. Antiviral medications are simply not part of what’s being screened. There is no reason Valtrex would cause a positive result on a workplace or pre-employment drug test.
