Xanax (alprazolam) is fully eliminated from your body within about two to six days after a single dose, depending on your individual metabolism. However, drug tests can pick up its breakdown products for varying lengths of time, and several personal factors can push that timeline significantly longer.
How Your Body Processes Xanax
After you swallow a standard Xanax tablet, it reaches peak levels in your blood within one to two hours. The extended-release version (Xanax XR) takes longer, peaking around 10 hours after a morning dose. From that point, your liver begins breaking the drug down into smaller compounds, the most important being a metabolite called alpha-hydroxyalprazolam. This metabolite actually shows up in higher concentrations in urine than the original drug itself, which is why labs often screen for it instead of Xanax directly.
The average elimination half-life of alprazolam in healthy adults is about 11.2 hours, with a range of 6.3 to 26.9 hours according to FDA labeling data. That means every 11 hours or so, your body clears roughly half of what’s left. A drug is considered effectively eliminated after four to five half-lives, when 94% to 97% of it is gone. For a healthy adult with an average half-life, that works out to roughly 46 to 56 hours, or about two to two and a half days.
Detection Windows by Test Type
Just because the drug is functionally cleared from your system doesn’t mean a test won’t find it. Different testing methods look at different biological samples, and each has its own detection window.
- Urine: After a single therapeutic dose, the median detection time is about 36 hours, with a range of 26 to 61 hours. Urine tests typically screen for that primary metabolite rather than alprazolam itself, which is why the window extends a bit beyond the drug’s own half-life.
- Oral fluid (saliva): After a single dose, alprazolam is detectable for a median of about 26 hours (range 4 to 37 hours). In people who have been taking high or repeated doses, detection in saliva can stretch to about 2.5 days.
- Blood: Blood tests generally reflect the drug’s active concentration, so detection closely tracks the elimination timeline of two to six days depending on individual factors.
- Hair: Hair follicle tests can catch many drugs for up to 90 days, but alprazolam is an exception. Research using sensitive lab methods found that alprazolam was not reliably detected in hair samples, even when the drug was known to have been taken. This makes hair testing an unreliable method for confirming Xanax use specifically.
These windows assume a single dose. Regular or heavy use builds up drug levels in your body, extending every one of these timelines considerably.
Why It Stays Longer in Some People
The range from person to person is wide, and the FDA’s own data spells out exactly why. Three factors stand out: age, body weight, and liver health.
Age. Older adults clear alprazolam more slowly. In healthy elderly subjects, the average half-life rises to 16.3 hours (range 9.0 to 26.9 hours) compared to 11.0 hours in younger adults. That alone can add an extra day or more to full elimination. Elderly individuals also tend to have higher concentrations of the drug in their blood at the same dose, meaning effects linger longer too.
Obesity. In people with higher body mass, the average half-life nearly doubles to 21.8 hours (range 9.9 to 40.4 hours). Alprazolam dissolves into fatty tissue, creating a reservoir that releases the drug back into the bloodstream slowly. At the upper end of that range, full clearance could take over a week.
Liver disease. Since your liver does the heavy lifting in metabolizing alprazolam, impaired liver function dramatically slows the process. In patients with alcoholic liver disease, the half-life averaged 19.7 hours but ranged from 5.8 to a striking 65.3 hours. At that upper extreme, the drug could remain in your system for well over two weeks.
Other factors that can slow clearance include taking medications that compete for the same liver enzymes, being of Asian descent (some studies suggest slightly slower metabolism), and being dehydrated or having reduced kidney function.
Single Dose vs. Regular Use
Everything above primarily applies to a single dose. If you take Xanax daily, the drug accumulates in your tissues before your body can fully clear each previous dose. This means stopping after weeks or months of use results in a longer detection window because there’s simply more drug stored in your body that needs to be processed out.
Repeated high-dose use also extends saliva detection. Research on patients entering detoxification after heavy benzodiazepine use found detection times well beyond what single-dose studies predict, precisely because of this accumulation effect. The more frequently you’ve taken Xanax and the higher the doses, the longer every type of test will be able to find it.
Typical Clearance Estimates at a Glance
For a healthy adult of average weight taking a single dose, expect the drug to be functionally gone in two to three days and undetectable on a standard urine screen within about two days. For older adults, those with higher body mass, or anyone with liver problems, a more conservative estimate is four to seven days for full clearance after a single dose. Chronic users should assume significantly longer windows, potentially stretching into weeks for complete elimination.
Keep in mind that “no longer detectable on a drug test” and “fully cleared from your body” aren’t always the same thing. Trace amounts below a test’s cutoff threshold can linger even after a screen comes back negative. The detection windows listed above reflect the point at which standard lab methods can no longer confirm use, not necessarily the point at which every last molecule is gone.
