Xylazine, a veterinary sedative increasingly found in the illicit drug supply, is metabolized rapidly in the body. The parent drug clears from blood and urine within just a few hours, but its breakdown products can linger longer, and specialized hair testing can detect exposure for up to three months. The exact window depends on which type of test is used and what specimen is collected.

How Quickly Your Body Breaks Down Xylazine

Xylazine is processed fast. In pharmacological studies, the drug itself drops to very low concentrations within one to three hours after use. The body breaks it down primarily by splitting open its chemical ring structure, producing a metabolite called 2,6-xylidine (also known as 2,6-dimethylaniline). A second key breakdown product, 4-hydroxy-xylazine, is considered the longer-lasting marker and has been traced in urine for up to 25 hours.

This rapid metabolism is one reason xylazine can be tricky to detect. If testing focuses only on the parent drug, the window to catch it is extremely narrow. Labs that screen for the metabolites have a much better chance of confirming exposure.

Detection Windows by Test Type

Blood and Urine

In blood, xylazine itself is detectable for roughly one to three hours after use. Urine offers a slightly longer window because the kidneys continue filtering out metabolites after the drug has left the bloodstream. The primary metabolite, 4-hydroxy-xylazine, can be found in urine for approximately 25 hours. Beyond that point, concentrations typically fall below what most lab instruments can pick up.

These numbers come from controlled veterinary research, and human metabolism of xylazine is still not fully characterized. Individual factors like liver function, body composition, hydration, and whether the drug was injected, swallowed, or absorbed under the tongue all influence how quickly it clears. Injection bypasses the liver’s first pass at breaking the drug down, which creates higher initial concentrations and may slightly extend the detectable window.

Hair Testing

Hair follicle testing provides a much longer detection window. At least one major U.S. drug testing laboratory offers hair analysis specifically for xylazine and hydroxy-xylazine. Hair collected close to the scalp can reveal drug exposure over approximately three months, the same general window used for other substances in hair panels. This makes hair the only specimen type capable of detecting xylazine use weeks or months after the fact.

Will Xylazine Show Up on a Standard Drug Test?

No. Standard workplace drug panels (5-panel, 10-panel, or 12-panel tests) do not screen for xylazine. These panels are designed to detect common drugs of abuse like opioids, amphetamines, cannabis, cocaine, and benzodiazepines. Xylazine is a veterinary tranquilizer with a completely different chemical structure, so it won’t trigger a positive result on any of those categories.

Detecting xylazine requires specialized forensic toxicology methods, typically liquid chromatography paired with mass spectrometry. Some medical examiner offices and forensic labs have added xylazine to their panels. Connecticut’s Office of the Chief Medical Examiner, for example, has included it in routine postmortem toxicology screening since 2013. But in a clinical setting or a standard employment drug screen, xylazine will not be tested for unless it is specifically requested.

How Long the Effects Last

The sedative effects of xylazine typically last between three and five hours. Onset depends on how the drug enters the body: injection produces the fastest effects, followed by rectal use, then sublingual or oral routes. People who use xylazine often describe the experience as heavily sedating and unpleasant rather than euphoric, which is part of why it is rarely sought out on its own. It most commonly appears mixed into fentanyl or heroin, sometimes without the user’s knowledge.

This creates a dangerous mismatch during overdose situations. Because xylazine is so frequently combined with fentanyl, naloxone (Narcan) should still be given if someone is experiencing respiratory depression. Naloxone will reverse the opioid component and can restore normal breathing. But it does nothing to reverse xylazine’s effects. A person may start breathing again after naloxone but remain deeply sedated for hours. This does not mean the naloxone failed or that the fentanyl is “naloxone-resistant.” It means the xylazine sedation is running its own course and requires time and supportive care to wear off. There is currently no reversal agent for xylazine that is safe for use in humans.

Why Xylazine Detection Matters

Xylazine’s presence in the drug supply has grown dramatically. CDC data from 20 states and Washington, D.C. showed that the percentage of fentanyl-related overdose deaths where xylazine was also detected jumped 276% between January 2019 and June 2022, rising from 2.9% to 10.9%. Despite this surge, xylazine is not a federally controlled substance under the Controlled Substances Act, though some states have placed it on their own controlled substances lists. The FDA took steps in early 2023 to restrict its unlawful importation.

The combination of rapid metabolism, absence from standard drug panels, and growing prevalence means xylazine exposure often goes undetected. If you or a healthcare provider suspects xylazine exposure, the most reliable approach is to request specific forensic toxicology testing within the first 24 hours, targeting metabolites rather than the parent drug. For longer-term confirmation, hair testing remains the only practical option.
