Xyzal (levocetirizine) typically causes drowsiness that peaks within the first 3 to 6 hours after you take it, then gradually fades as the drug is cleared from your body. The half-life is about 8 hours in adults, meaning the sleepy feeling usually wears off well before the 24-hour allergy protection ends. This is exactly why the manufacturer recommends taking it at night: you sleep through the drowsiest window and wake up with the antihistamine still working but the sedation largely gone.
When Drowsiness Starts and Peaks
Xyzal is absorbed fast. Blood levels peak roughly 45 to 60 minutes after swallowing a tablet on an empty stomach. If you take it with a high-fat meal, that peak shifts to about 2 hours and the concentration is lower, which can slightly delay and blunt any drowsiness.
The sedating effect doesn’t track perfectly with blood levels, though. While drug concentrations peak early, the strongest effects on alertness tend to hit between 3 and 6 hours after the dose. After that window, sedation tapers off even though the antihistamine effect continues for a full 24 hours. For most people who take their dose at bedtime, the heaviest drowsiness overlaps with normal sleep, and by morning the feeling has passed.
How Many People Actually Get Drowsy
Drowsiness is the most commonly reported side effect, but it still affects a minority of users. In clinical trials of adults and adolescents 12 and older, 6% of people taking the standard 5 mg dose reported drowsiness, compared to 2% on a placebo. At the lower 2.5 mg dose, the rate was 5%. So roughly 1 in 17 people notice it at the standard dose, and some of that drowsiness would have happened anyway (as the placebo rate shows).
Children ages 6 to 12 reported drowsiness at an even lower rate of about 3%, versus less than 1% on placebo. Younger children metabolize the drug a bit differently, but the overall pattern is similar: drowsiness is possible but not the norm.
Why Taking It at Night Solves the Problem
Xyzal is specifically designed to be taken once daily in the evening. The official recommendation is a 5 mg tablet at night for adults and children 12 and older (or a half tablet for milder symptoms). This isn’t just marketing. Because the sedation window falls in those first several hours, an evening dose lets you ride out any drowsiness while you’re already in bed. The allergy-blocking effect peaks later and holds steady through the next day.
If you’ve been taking Xyzal in the morning and feeling sluggish, switching to a bedtime dose is the simplest fix. Many people who do this report no noticeable daytime drowsiness at all.
Factors That Can Make Drowsiness Last Longer
A few things can stretch out or intensify the sedating effect:
- Alcohol. The FDA label specifically warns against combining Xyzal with alcohol because the two together cause greater reductions in alertness than either one alone. If you drink in the evening and take your dose around the same time, expect the grogginess to be stronger and potentially linger into the next morning.
- Other sedating medications. Sleep aids, anti-anxiety drugs, muscle relaxants, and opioid pain medications can all compound the drowsiness. The same applies to older antihistamines like diphenhydramine (Benadryl).
- Kidney function. Levocetirizine is cleared primarily through the kidneys. If your kidney function is reduced, the drug stays in your system longer, which can extend both the therapeutic effect and the drowsiness. People with significant kidney impairment are typically prescribed a lower dose for this reason.
- Individual sensitivity. Some people are simply more sensitive to antihistamines. If you’ve felt drowsy on cetirizine (Zyrtec), you’re likely to notice a similar effect with Xyzal. A controlled trial found that the sedation profiles of the two drugs are essentially the same, so switching between them for drowsiness reasons alone is unlikely to help.
How Xyzal Compares to Other Antihistamines
Xyzal is classified as a second-generation antihistamine, the same category as cetirizine (Zyrtec), loratadine (Claritin), and fexofenadine (Allegra). These newer antihistamines cross into the brain far less than older ones like diphenhydramine, which is why they cause less sedation. But “less” isn’t “none,” and cetirizine and levocetirizine sit at the more sedating end of the second-generation spectrum.
If Xyzal’s drowsiness bothers you even with nighttime dosing, fexofenadine and loratadine are generally considered less sedating alternatives. They work through the same mechanism of blocking histamine but have even less tendency to cross into the brain. The tradeoff is that some people find them slightly less effective for severe allergy symptoms.
What to Expect in the First Few Days
Drowsiness from Xyzal often improves after the first few days of regular use. Your body adjusts to the drug, and the sedation tends to diminish while the allergy relief remains consistent. If you’re starting Xyzal for the first time, it’s worth giving it three to five days before judging whether the drowsiness is a lasting problem for you. Avoid driving or operating heavy machinery during this initial period, especially if you’re taking your dose during the day while you figure out your best timing.
