Zoloft (sertraline) takes roughly 5 to 6 days to clear from your body after your last dose. That timeline is based on the drug itself, but sertraline produces an active metabolite that lingers considerably longer, potentially keeping traces in your system for several weeks. How quickly you personally clear it depends on your age, liver function, and genetics.
How the 5-to-6-Day Timeline Works
Sertraline has an average elimination half-life of about 26 hours, meaning that every 26 hours, the concentration in your blood drops by half. Pharmacologists use a “five and a half half-lives” rule to estimate when a drug is essentially gone (about 97% eliminated). For sertraline, that math works out to roughly 143 hours, or just under 6 days.
But the parent drug is only part of the story. Your liver breaks sertraline down into a metabolite called desmethylsertraline, which is still pharmacologically active. This metabolite has a half-life of 56 to 120 hours, two to nearly five times longer than sertraline itself. Using the same elimination rule, desmethylsertraline could persist for 13 to 27 days after your final dose. So while the sertraline molecule itself is gone within a week, its byproducts can remain detectable for much longer.
Factors That Speed Up or Slow Down Clearance
Age
Older adults clear sertraline significantly more slowly. In studies comparing patients 65 and older to adults in their late twenties and early thirties, plasma clearance was approximately 40% lower in the older group. A separate study found the elimination rate in elderly individuals was 16 to 63% slower than in younger adults. This means that for someone over 65, the drug could stay in the system meaningfully longer than the average 6-day estimate.
Genetics
Sertraline is processed primarily by a liver enzyme called CYP2C19, and people carry different genetic versions of this enzyme. In a study of 1,200 patients, those with reduced-function versions of the gene (called “poor metabolizers”) had sertraline blood levels 2.68 times higher than people with normal enzyme activity. They were also nearly 9 times more likely to have drug concentrations above the expected therapeutic range. If you’re a poor metabolizer, sertraline sits in your system substantially longer. Roughly 2 to 3% of people of European descent fall into this category, though the percentage varies by ethnic background.
Liver Function
Since the liver does the heavy lifting in breaking down sertraline, any impairment slows the process. In patients with liver cirrhosis, peak drug concentrations were 1.7 times higher and the elimination half-life was significantly prolonged compared to people with healthy liver function. Even moderate liver disease can extend clearance times noticeably.
Dose and Duration of Use
When you take sertraline daily, it builds up in your body and reaches a stable “steady state” concentration after about 7 days of consistent dosing. Higher doses and longer treatment durations mean more drug has accumulated in your tissues by the time you stop. Someone who has been on 200 mg daily for years will take longer to fully clear the drug than someone who took 50 mg for a few weeks.
Zoloft and Drug Testing
Standard workplace urine drug screens test for common drugs of abuse like amphetamines, opioids, benzodiazepines, cannabis, and cocaine. Sertraline is not one of the target substances. However, sertraline has been reported in some cases to trigger false-positive results for benzodiazepines or amphetamines on immunoassay-based screening tests. These are initial screening tests, not confirmatory ones. If a false positive occurs, a follow-up confirmatory test (which identifies the exact chemical structure) will distinguish sertraline from actual drugs of abuse and clear the result.
Because of the long-lasting metabolite, traces of sertraline or its breakdown products could theoretically show up in sensitive laboratory assays for several weeks after your last dose, even after you’ve stopped feeling any effects from the medication.
What Happens After You Stop
Even though the drug clears your bloodstream within about a week, your brain needs time to readjust. Sertraline works by increasing serotonin availability, and your nervous system adapts to that higher level over the course of treatment. When you stop, the sudden drop can cause what’s known as discontinuation syndrome.
Symptoms typically appear within 2 to 4 days of your last dose and commonly include dizziness, nausea, irritability, “brain zaps” (brief electrical shock sensations), insomnia, and flu-like feelings. For most people, these symptoms last 1 to 2 weeks, though in some cases they can persist longer. Sertraline has a shorter half-life than some other antidepressants in its class, which makes it somewhat more likely to cause noticeable discontinuation effects compared to longer-acting options. Gradual dose reduction over weeks, rather than stopping abruptly, significantly reduces the risk and severity of these symptoms.
The 5-to-6-day clearance window for the parent drug and the 2-to-4-week window for the active metabolite are averages.
