Kidney stone surgery typically takes between one and four hours, depending on the type of procedure. Smaller stones treated with shock waves can be done in about an hour, while more invasive surgery for large stones may take up to four hours. The total time you spend at the hospital will be longer than the procedure itself once you factor in preparation, anesthesia, and recovery.
Three Main Procedures and Their Timeframes
Not all kidney stone surgeries are the same. The procedure your surgeon recommends depends on the size, location, and composition of the stone. Each one has a different expected duration.
Shock wave lithotripsy (SWL) is the least invasive option. It uses focused sound waves from outside the body to break stones into smaller pieces you can pass naturally. The procedure usually takes about one hour, though it can run shorter or longer depending on how many stones you have and their size. It’s typically used for stones smaller than about 2 centimeters that are still in the kidney or upper ureter.
Ureteroscopy involves passing a thin, flexible scope through the bladder and up into the ureter or kidney to reach the stone directly. The surgeon then uses a laser to break the stone into fragments. According to Johns Hopkins Medicine, ureteroscopy usually lasts one to three hours. The wide range reflects the variability in stone size and location. A small stone sitting low in the ureter can be reached and fragmented quickly, while a larger stone lodged deep in the kidney takes considerably more time.
Percutaneous nephrolithotomy (PCNL) is reserved for larger or more complex stones. The surgeon makes a small incision in your back and passes instruments directly into the kidney to remove the stone. PCNL usually takes two to four hours to complete. Because it’s the most involved procedure, it also requires a hospital stay afterward, unlike the other two options which are often done on an outpatient basis.
What Adds Time Beyond the Procedure
The surgical time your doctor quotes usually refers to the procedure itself, not your total time at the facility. Plan for several additional hours on either side. Before surgery, you’ll go through check-in, change into a gown, have an IV placed, and meet with the anesthesia team. Anesthesia induction and positioning typically add 20 to 40 minutes before the surgeon begins. After the procedure, you’ll spend time in a recovery area while the anesthesia wears off, which can take another one to two hours.
For an outpatient ureteroscopy, you might arrive at 6 a.m. and leave by early afternoon. For PCNL, expect to stay at least one night in the hospital.
Factors That Change How Long It Takes
Several variables push a procedure toward the shorter or longer end of its range. Stone size is the most obvious: a 5-millimeter stone fragments much faster than a 15-millimeter one. The number of stones matters too. Treating multiple stones in a single session naturally extends operating time.
Stone hardness also plays a role. Harder stones, which show up as brighter on a CT scan, resist laser fragmentation and take longer to break apart. However, the type of laser your surgeon uses doesn’t make a significant difference. A clinical trial comparing the two main laser technologies found that the active treatment time averaged about 20 minutes with either system, with no meaningful difference based on stone size or location.
If you have stones in both kidneys, your surgeon may treat both sides in a single session rather than scheduling two separate surgeries. Treating both sides at once takes longer than treating one side alone, but the total operating time is still shorter than the combined time of two staged procedures. One study found single-session bilateral ureteroscopy averaged 86 minutes, compared to 139 minutes total when the same work was split across two separate surgeries.
Anatomical factors can also extend the procedure. A narrow ureter, unusual kidney anatomy, or prior scar tissue from previous surgeries can make it harder for the surgeon to reach the stone, adding time.
Recovery Time After Each Procedure
Most people care about surgical duration partly because they want to plan the rest of their week. Here’s what to expect.
After shock wave lithotripsy, you can usually go home within a couple of hours and return to normal activities within a day or two. You’ll still need to pass the stone fragments over the following days to weeks, which can cause some discomfort.
After ureteroscopy, expect to go home the same day. A temporary stent is often placed in the ureter to keep it open while swelling subsides. The stent stays in for about a week and can cause bladder pressure and urgency. Most people return to work within two to three days, though physical activity may be limited for a week.
PCNL has the longest recovery. You’ll typically stay in the hospital for one to two nights. A drainage tube may be left in your back temporarily. Full recovery usually takes one to two weeks, and heavy lifting is restricted for several weeks after that.
