There’s no single answer to how long you should take a multivitamin because it depends entirely on why you’re taking one. If you’re correcting a diagnosed deficiency, you may need only a few months. If you’ve had weight-loss surgery, supplementation is lifelong. And if you’re a generally healthy adult taking one “just in case,” the evidence suggests you may not need one at all.

What the Evidence Says About Daily Use

No major U.S. health agency, professional medical organization, or government body recommends routine multivitamin use for healthy adults. The U.S. Preventive Services Task Force reviewed 78 randomized controlled trials and six cohort studies and concluded there wasn’t enough evidence to determine whether multivitamins help prevent heart disease or cancer. The American Heart Association advises against using them for cardiovascular protection, and the American Institute for Cancer Research recommends against supplements for cancer prevention, noting that whole foods provide benefits supplements can’t replicate.

That said, there is some encouraging research on cognitive health. The large COSMOS trial found that two years of daily multivitamin use improved global cognition and episodic memory in older adults, with effects equivalent to slowing cognitive aging by about two years. This is one of the few areas where long-term daily use has shown a measurable benefit in clinical trials, though it’s not yet enough to change official guidelines.

For people who don’t eat a wide variety of nutritious foods, multivitamins can help fill gaps. The NIH Office of Dietary Supplements acknowledges this role but emphasizes that supplements can’t replace the broader benefits of a balanced diet.

Correcting a Deficiency: Weeks to Months

If blood work shows you’re deficient in a specific nutrient, supplementation has a clear start and end point. Vitamin D deficiency, one of the most common, typically requires about eight weeks of high-dose supplementation to bring levels back to a healthy range. Your provider will usually recheck your levels after three months to confirm the correction worked. Once levels normalize, you’ll either stop supplementing or switch to a lower maintenance dose depending on your diet and sun exposure.

Iron deficiency follows a similar pattern. It generally takes three to six months of supplementation to rebuild your body’s iron stores, not just normalize your blood levels. B12 deficiency can resolve faster with supplementation but may require ongoing doses if you have an absorption issue or follow a strictly plant-based diet. In all these cases, the duration is driven by lab results rather than a fixed calendar.

Pregnancy: Before Conception Through Breastfeeding

Prenatal vitamins are one of the clearest cases for time-limited supplementation. The ideal window starts before you conceive, since folate is critical in the earliest weeks of pregnancy, often before you even know you’re pregnant. If you’re planning to get pregnant, starting a prenatal vitamin beforehand gives your body time to build up adequate nutrient levels.

You should continue taking a prenatal throughout pregnancy and, if you’re breastfeeding, for as long as you nurse. Your baby depends on your nutrient intake for vitamins A, D, B vitamins, DHA, and iodine during this period. If you’re not breastfeeding, the general recommendation is to continue for at least six to eight weeks postpartum while your body recovers.

After Weight-Loss Surgery: Lifelong

Bariatric surgery permanently changes how your body absorbs nutrients. Procedures that bypass or reduce portions of the stomach and small intestine make it physically impossible to extract enough vitamins and minerals from food alone. Because of this, daily vitamin and mineral supplementation after weight-loss surgery is a lifelong commitment, not a temporary recovery measure. Stopping supplements after bariatric surgery can lead to serious deficiencies in iron, calcium, B12, and fat-soluble vitamins over time, even if you feel perfectly fine.

Risks of Taking Them Too Long at High Doses

Standard multivitamins taken at the recommended dose are generally safe for extended use. The real risk comes from fat-soluble vitamins, specifically A and D, which your body stores rather than flushing out. Unlike water-soluble vitamins such as vitamin C, excess fat-soluble vitamins accumulate in your tissues over time.

Chronic vitamin A overdose can cause dry, cracking skin and lips, hair loss, bone and joint pain, blurred vision, and nausea. Long-term excess vitamin D raises calcium levels in your blood, which can affect your kidneys and bones. These problems develop gradually with repeated high doses, not from a single standard multivitamin. But if you’re stacking a multivitamin with additional individual supplements (a separate vitamin D capsule, a calcium tablet, a fish oil with added A), you can creep past safe levels without realizing it.

A good practice is to add up the total amount of each nutrient you’re getting across all your supplements and compare it to the tolerable upper intake level for that nutrient. If you’ve been taking multiple supplements daily for months or years, periodic blood work can catch accumulation before symptoms appear.

How to Decide Your Timeline

Your reason for supplementing determines how long you should continue. If you’re filling a diagnosed deficiency, plan on two to three months and then retest. If you’re pregnant or breastfeeding, continue through that entire window. If you’ve had bariatric surgery or have a chronic condition that impairs absorption, supplementation is permanent.

If none of those apply and you’re taking a multivitamin as general insurance, there’s no evidence-based reason to take one indefinitely, but there’s also little harm in a standard-dose product if your diet has consistent gaps. The most useful step is an honest look at what you eat. If you regularly consume a variety of vegetables, fruits, whole grains, and protein sources, a daily multivitamin is unlikely to add meaningful benefit. If your diet is limited by preference, budget, or medical restrictions, a basic multivitamin can serve as a reasonable safety net for as long as those limitations persist.
