Immediate-release melatonin typically wears off in about 3 to 5 hours. The supplement has a short half-life of roughly 45 minutes, meaning your body eliminates half of it in that time. After four rounds of that elimination cycle, levels in your blood drop low enough that the sleep-promoting effect fades. Extended-release formulations last longer, closer to 6 to 7 hours.
Immediate-Release vs. Extended-Release Timelines
The type of melatonin you took is the single biggest factor in how long it sticks around. Immediate-release tablets dissolve quickly, flooding your system with melatonin within about 30 minutes. Blood levels spike, then drop off fast. In a pharmacokinetic study comparing the two formulations, immediate-release melatonin stayed above a useful sleep-maintaining threshold for a median of 3.7 hours, while extended-release melatonin maintained that level for 6.7 hours.
This distinction matters if you’re waking up in the middle of the night. Immediate-release melatonin is designed to help you fall asleep but may not keep you asleep through a full eight-hour night. Extended-release versions meter out the dose gradually, mimicking the way your brain naturally produces melatonin across the night. If you took an extended-release tablet, expect the effects to linger closer to 7 hours rather than 4.
Why It Might Last Longer for Some People
Your liver does the heavy lifting when it comes to clearing melatonin from your body. A specific liver enzyme breaks melatonin down into a byproduct that gets flushed out through urine. Anything that slows this enzyme down will keep melatonin circulating longer than the typical 3 to 5 hours.
Age is one major variable. Older adults process melatonin more slowly, and peak blood concentrations after the same dose can be dramatically higher, up to 240% greater in people over 45 compared to younger adults. That means a 5 mg tablet might hit an older person two to three times harder than a younger one, and take noticeably longer to clear. If you’re over 50 and feeling groggy the next morning, this is likely why. Starting with a lower dose (0.5 to 1 mg) can help.
Liver health also plays a role. Since the liver is responsible for breaking melatonin down, any condition that compromises liver function can slow clearance and extend the duration of effects.
Caffeine and Other Substances That Slow Clearance
Caffeine competes for the same liver enzyme that processes melatonin. In a study of 12 healthy adults, taking caffeine alongside melatonin increased melatonin blood levels by an average of 120 to 142%. That’s not a small bump. It means melatonin lingers substantially longer and hits harder when caffeine is still in your system.
This interaction cuts both ways depending on your timing. Caffeine itself has a half-life of about 5 hours, so an afternoon coffee could still be occupying that enzyme when you take melatonin at bedtime. The result: your body clears the melatonin more slowly, potentially pushing its effects well past the usual 4-hour window. Nonsmokers and people with certain genetic variations in that liver enzyme are especially susceptible to this effect. Smokers, on the other hand, tend to have a more active version of the enzyme and may clear melatonin faster than average.
Other substances that use the same metabolic pathway, including certain medications like some antidepressants and heart medications, can similarly extend how long melatonin stays active. If you take prescription drugs and find melatonin affects you longer than expected, this enzyme competition is a likely explanation.
How Long Grogginess Can Last
Even after melatonin’s direct sleep-promoting effect fades, residual drowsiness can hang on. The Mayo Clinic advises avoiding driving or operating machinery for five hours after taking melatonin. For most people taking an immediate-release dose, any morning grogginess clears within an hour or two of waking. But if you took a high dose (5 mg or more), used an extended-release formula, or have any of the slower-metabolism factors above, that foggy feeling can stretch further into the morning.
The dose you take matters more than most people realize. Your brain’s natural melatonin production peaks at levels equivalent to roughly 0.3 mg of supplemental melatonin. Most store-bought tablets contain 3 to 10 mg, which is 10 to 30 times that amount. Higher doses don’t necessarily help you fall asleep faster, but they do take longer to clear and increase the chance you’ll feel sluggish the next day. If you’re consistently waking up groggy, cutting your dose in half is a reasonable first step.
Timing It Right
Knowing melatonin’s timeline helps you pick the right moment to take it. For immediate-release tablets, taking them 30 to 60 minutes before you want to fall asleep gives the supplement time to reach peak levels right as you’re lying down. Since the effect lasts about 4 hours, this works well if your main problem is falling asleep rather than staying asleep.
If you need to be sharp at a specific time the next morning, count backward. An immediate-release dose taken at 10 p.m. will mostly clear by 1 or 2 a.m., well before a 6 a.m. alarm. An extended-release dose taken at the same time may still be active at 4 or 5 a.m. For a very early morning, stick with immediate-release and a lower dose to minimize any overlap with your wake-up time.
