A healthy adult heart beats 60 to 100 times per minute at rest, but it can safely dip well below that range depending on your fitness level, sleep state, and individual physiology. The medical term for a slow heart rate is bradycardia, generally defined as anything below 50 beats per minute. Whether a low number is normal or dangerous depends almost entirely on whether it causes symptoms.
What Counts as Too Low
For most adults, a resting heart rate between 60 and 100 bpm is considered standard. Drop below 50 bpm and you’re technically in bradycardia territory. But “technically” is doing a lot of work in that sentence, because millions of people walk around with heart rates in the 40s and 50s with zero problems. Athletes, people on certain heart medications, and naturally fit individuals often sit comfortably in that range.
The threshold where doctors get genuinely concerned is around 35 to 40 bpm, especially if you’re also experiencing dizziness, fainting, unusual fatigue, shortness of breath, or quick exhaustion during exercise. Those symptoms mean your heart isn’t circulating enough blood and oxygen to keep your body running properly. A heart rate in the low 40s with no symptoms is a very different situation from a heart rate in the low 40s with lightheadedness and fatigue.
How Low Athletes Can Go
Endurance training reshapes the heart. Years of sustained aerobic exercise make the heart larger and more efficient, pumping more blood per beat. That means it doesn’t need to beat as often to deliver the same amount of oxygen. Elite athletes commonly have resting heart rates of 40 to 50 bpm, and some go significantly lower.
Five-time Tour de France winner Miguel Indurain had a resting heart rate of 28 bpm. Michael Phelps, the most decorated Olympian in history, ran below 40 bpm at rest. These are extreme examples, but they illustrate that the heart can function perfectly well at rates that would alarm most people seeing the number on a fitness tracker for the first time. The key distinction is that these low rates reflect a powerful, efficient heart, not a malfunctioning one.
Your Heart Rate Drops During Sleep
Even if you’re not an athlete, your heart rate falls substantially while you sleep. During the night, your sleeping heart rate typically runs 20% to 30% lower than your daytime resting rate. For a healthy adult, that puts the normal sleeping range at roughly 50 to 75 bpm, though dipping into the 40s during deep sleep is not unusual.
If you wear a smartwatch or fitness tracker overnight and notice a reading in the low 40s, that’s not automatically a red flag. The Cleveland Clinic considers any sleeping heart rate between 40 and 100 bpm to fall within the normal window. Numbers below 40 during sleep are worth paying attention to, particularly if you’re waking up feeling unrested, dizzy, or short of breath.
Normal Ranges Change With Age
What counts as “low” depends heavily on how old you are. Babies and young children have naturally faster hearts. A newborn’s resting rate ranges from 100 to 205 bpm, while toddlers sit between 98 and 140 bpm. By school age (5 to 12), the range narrows to 75 to 118 bpm. Adolescents and adults settle into the familiar 60 to 100 bpm range.
This means a heart rate of 70 bpm that would be perfectly normal in an adult could signal a problem in an infant. The younger the child, the more concerning a relatively “normal-sounding” adult heart rate becomes.
Medical Causes of a Slow Heart Rate
When a low heart rate isn’t explained by fitness or sleep, several medical conditions can be responsible. The heart’s electrical system runs on a precise sequence of signals. If those signals slow down or get blocked, the heart rate drops.
- Sinus node problems: The sinus node is your heart’s natural pacemaker, sitting at the top of the heart. When it malfunctions, it can produce signals too slowly or alternate between abnormally slow and fast rhythms.
- Heart block: Electrical signals that normally travel from the upper chambers to the lower chambers get delayed or blocked entirely. Depending on the severity, this can cause mild slowing or dangerously low rates.
- Electrolyte imbalances: Abnormal levels of potassium or calcium in the blood interfere with the electrical signals that keep your heart beating at a steady pace.
- Sleep apnea: Obstructive sleep apnea repeatedly interrupts breathing during sleep, which can trigger episodes of significant heart rate slowing overnight.
- Inflammatory conditions: Diseases like rheumatic fever or lupus can damage heart tissue and disrupt its electrical pathways.
Medications That Lower Heart Rate
Several common medications are specifically designed to slow the heart. Beta-blockers, one of the most widely prescribed drug classes in the world, work by blocking the stress hormones that tell your heart to speed up. They relax the heart and blood vessels, bringing down both heart rate and blood pressure. Calcium channel blockers produce a similar slowing effect through a different mechanism.
Sedatives and opioids can also lower heart rate as a side effect. If you’re on any of these medications and your resting heart rate sits in the 40s or 50s, that may be an expected result of the drug doing its job rather than a sign of trouble. The concern arises when the rate drops low enough to cause symptoms like dizziness or fatigue, which could mean the dose needs adjusting.
Cold Temperatures Push It Lower
Hypothermia slows the heart dramatically. As body temperature falls, each heartbeat takes longer to complete its full contraction and relaxation cycle. The heart naturally compensates by beating less frequently. Research published in the American Journal of Physiology found that this slowing is actually protective: during hypothermia, beating faster would leave too little time for the heart to fill with blood between beats, worsening function rather than improving it.
In severe hypothermia cases, heart rates can fall into the 20s or lower before the heart eventually stops. This is why cold-water drowning victims are sometimes revived even after prolonged periods without a pulse. The cold slows metabolism enough that the brain and organs can survive longer without oxygen.
Symptoms That Matter More Than the Number
The single most important thing to understand about a low heart rate is that the number alone doesn’t tell you much. A rate of 45 bpm in a fit 30-year-old who feels great is completely different from a rate of 45 bpm in a 70-year-old who keeps getting dizzy.
The symptoms that signal a problem are consistent: dizziness or lightheadedness, fainting or near-fainting episodes, persistent fatigue, shortness of breath, and becoming exhausted unusually quickly during physical activity. These happen because your heart can’t push enough oxygenated blood to your brain, muscles, and organs at that pace. If your heart rate runs low but you feel fine, have good energy, and can exercise without unusual fatigue, your heart is almost certainly doing its job efficiently.
