The standard adult dose of Benadryl (diphenhydramine) is 25 to 50 mg every 4 to 6 hours, with a maximum of 300 mg in 24 hours. Since most Benadryl tablets contain 25 mg, that works out to a maximum of 12 tablets per day, though most people need far fewer than that.
Adult Dosage and Timing
For allergies, cold symptoms, or motion sickness, adults and adolescents can take 1 to 2 tablets (25 to 50 mg) every 4 to 6 hours as needed. The ceiling is 300 mg, or 12 standard tablets, in a 24-hour period. If you’re using Benadryl specifically for cough, the recommended limit is lower: 25 mg per dose with a maximum of 150 mg (6 tablets) per day.
In practice, most people take 1 to 2 tablets at a time and repeat only if symptoms return several hours later. Taking the maximum amount every single day isn’t intended as a routine, and you should need fewer doses on days when symptoms are mild.
Children’s Dosing Is Different
The American Academy of Pediatrics advises against giving diphenhydramine to children under 6 unless directed by a doctor. For children 6 and older, dosing is based on weight rather than age, and the interval between doses is longer: every 6 hours rather than every 4. If you’re giving Benadryl to a child, use a weight-based dosing chart (available on the AAP’s website or the product packaging) rather than estimating from the adult dose.
How Long Each Dose Lasts
Diphenhydramine reaches its peak level in your bloodstream about 3 hours after you swallow it. Its half-life averages around 8.5 hours, meaning it takes roughly that long for your body to clear half the dose. After 24 hours, only a trace amount remains. This is why you feel drowsy for hours after a single dose and why spacing doses at least 4 to 6 hours apart matters. Stacking doses too close together lets the drug accumulate faster than your body can process it.
Children tend to clear diphenhydramine faster, while older adults clear it more slowly, which means the drug lingers longer in seniors and side effects hit harder.
Who Should Avoid Benadryl Entirely
Certain conditions make diphenhydramine risky at any dose. If you have glaucoma, the drug can raise pressure inside your eyes. An enlarged prostate or existing difficulty urinating can worsen significantly because diphenhydramine tightens the bladder. People with heart disease, high blood pressure, liver disease, asthma, or stomach ulcers also face elevated risks.
Older adults are especially vulnerable. Diphenhydramine is on the Beers Criteria list of medications that geriatric patients should generally avoid. It causes more pronounced confusion, dizziness, and fall risk in people over 65, and its sedative effects are amplified because the drug clears from the body more slowly with age.
Mixing With Alcohol or Sedatives
Combining Benadryl with alcohol is a significant safety concern. Both substances depress the central nervous system, and together they can impair attention, coordination, and reaction time far more than either one alone. The same applies to prescription sedatives, sleep aids, anti-anxiety medications, and opioid pain relievers. In people who are elderly, debilitated, or have any respiratory condition, this combination can suppress breathing to dangerous levels even at otherwise normal doses.
What Overdose Looks Like
At doses above the recommended maximum, diphenhydramine toxicity produces a recognizable set of symptoms: confusion, rapid heart rate, blurry vision, dry mouth, difficulty urinating, irritability, and hallucinations. Ingesting more than 1 gram (40 standard tablets) can cause delirium, psychosis, seizures, coma, and death. The risk escalates sharply above 1.5 grams, and fatal cases have been reported at doses exceeding roughly 20 mg per kilogram of body weight.
These aren’t numbers anyone should test. Social media trends encouraging large doses of Benadryl have prompted poison control warnings, and a 2025 citizen petition has formally asked the FDA to remove oral diphenhydramine from over-the-counter cough, cold, and allergy products altogether.
Why Daily Long-Term Use Is a Problem
Benadryl is designed for short-term, as-needed use. Taking it daily over months or years raises a concern that goes beyond drowsiness. A University of Washington study that tracked nearly 3,500 adults aged 65 and older for an average of seven years found that people who used anticholinergic drugs like diphenhydramine for three years or more had a 54% higher risk of developing dementia compared to those who used the same drugs for three months or less.
That doesn’t mean a few days of Benadryl during allergy season will harm your brain. But if you find yourself reaching for it nightly as a sleep aid or daily for chronic allergies, newer antihistamines like cetirizine or loratadine are better long-term options. They don’t cross into the brain the way diphenhydramine does, so they control allergy symptoms without the sedation or the cognitive concerns.
Tolerance and Rebound Effects
Your body builds tolerance to diphenhydramine’s sedative effects relatively quickly, sometimes within a few days of regular use. This is one reason people gradually increase their dose when using it as a sleep aid, chasing the same drowsiness with more tablets. That pattern pushes you closer to the daily maximum and increases the likelihood of side effects like morning grogginess, dry mouth, constipation, and urinary retention. If you stop abruptly after using it regularly, some people experience rebound insomnia, where sleep feels worse than it did before they started taking it.
