How Many Hours Between Ibuprofen and Excedrin: Safe Timing

You should wait at least 8 hours between taking ibuprofen and Excedrin. This timing matters because Excedrin Extra Strength contains aspirin, and ibuprofen interferes with how aspirin works if the two are taken too close together. Beyond that interaction, combining these drugs also doubles up on a class of pain relievers called NSAIDs, which raises the risk of stomach bleeding and kidney problems.

Why These Two Drugs Conflict

Excedrin Extra Strength isn’t just one ingredient. Each caplet contains 250 mg of acetaminophen, 250 mg of aspirin, and 65 mg of caffeine. The aspirin is the problem. Both aspirin and ibuprofen are NSAIDs, and they compete for the same spot on the enzyme that controls pain and inflammation. When ibuprofen gets there first, it physically blocks aspirin from doing its job. The FDA specifically warns that ibuprofen can weaken aspirin’s effects through this competitive binding.

This matters most for people who take low-dose aspirin for heart protection, but it also means the aspirin in your Excedrin may not work as intended if ibuprofen is still active in your system.

The 8-Hour Rule

The FDA recommends taking ibuprofen at least 8 hours before aspirin to avoid significant interference. If you flip the order and take Excedrin first, you need to wait at least 30 minutes before taking ibuprofen. That asymmetry exists because aspirin locks onto its target permanently, while ibuprofen’s hold is temporary. Once aspirin has had 30 minutes to bind, ibuprofen can no longer displace it. But if ibuprofen is already present, it blocks aspirin from binding at all, and you need to wait for the ibuprofen to clear.

In practical terms: if you took ibuprofen in the morning, wait at least 8 hours before reaching for Excedrin. If you took Excedrin first, wait at least 30 minutes before taking ibuprofen, though spacing them further apart is still the safer approach.

Doubling Up on NSAIDs Raises Real Risks

Even with proper timing, taking both ibuprofen and Excedrin in the same day means you’re using two different NSAIDs. Research from the French national pharmacovigilance database found that using two or more NSAIDs simultaneously raised the risk of gastrointestinal bleeding roughly 11-fold compared to not using any NSAIDs. A single NSAID raised that risk about 7-fold. The risk of acute kidney failure also climbed, from about 3 times higher with one NSAID to nearly 5 times higher with two.

The FDA’s drug facts label for ibuprofen explicitly warns against combining it with other NSAID-containing products, including aspirin. The chance of stomach bleeding goes up further if you’re over 60, have a history of ulcers, drink three or more alcoholic drinks a day, or take blood thinners or steroids.

What You Can Safely Alternate Instead

If one pain reliever isn’t cutting it, the safest combination is ibuprofen and plain acetaminophen (Tylenol). These work through completely different mechanisms and don’t compete with each other. You can even take them at the same time for short-term relief of severe pain, though alternating them on a schedule works well for ongoing symptoms.

A common alternating schedule looks like this:

  • 8:00 AM: Ibuprofen (200 to 400 mg)
  • 12:00 PM: Acetaminophen (500 to 1,000 mg)
  • 4:00 PM: Ibuprofen
  • 8:00 PM: Acetaminophen

This keeps pain relief steady throughout the day without exceeding the daily limits for either drug. Over-the-counter ibuprofen tops out at 1,200 mg per day, and acetaminophen should stay at or below 3,000 mg per day for most adults.

The Caffeine Factor

Each Excedrin caplet contains 65 mg of caffeine, roughly the amount in a cup of coffee. A standard two-caplet dose delivers 130 mg. If you’re sensitive to caffeine or already drinking coffee throughout the day, this can add up and cause jitteriness, trouble sleeping, or a faster heart rate. It won’t interact with ibuprofen, but it’s worth factoring in if you’re taking Excedrin later in the day.
