The human back contains roughly 40 muscles, though the exact count depends on whether you count each individual segment of a muscle group separately or treat it as one unit. Anatomists organize these muscles into three layers: superficial, intermediate, and deep. The superficial muscles are the ones you can see and feel. The deep muscles are small, stacked along your spine, and mostly work without you ever thinking about them.
Why the Count Varies
You’ll see numbers ranging from about 20 to over 40 depending on the source, and neither is wrong. The issue is how you count. The erector spinae, for example, is often described as one muscle group, but it’s actually three columns (iliocostalis, longissimus, and spinalis), each of which has subdivisions running along different parts of the spine. Count the erector spinae as one muscle and you get a lower total. Count each subdivision and the number climbs. The same applies to several other deep muscle groups that repeat at nearly every vertebral level.
Superficial Layer: The Big Movers
The superficial back muscles are the largest and most familiar. They don’t actually move the spine itself. Instead, they connect your spine to your shoulder blades and upper arms, controlling movements like pulling, lifting, and shrugging. There are five main muscles in this layer:
- Trapezius: the large diamond-shaped muscle stretching from the base of your skull down to the middle of your back and out to each shoulder. It shrugs your shoulders, tilts your head, and pulls your shoulder blades together.
- Latissimus dorsi: the widest muscle in your body, spanning from your lower back up to your upper arm. It powers pulling motions like rowing or doing a pull-up.
- Rhomboid major and rhomboid minor: two muscles between your spine and shoulder blade that squeeze your shoulder blades together.
- Levator scapulae: a narrow muscle running from your upper neck to the top of your shoulder blade, responsible for lifting the shoulder blade upward.
These muscles are “extrinsic,” meaning they technically belong to the upper limb but happen to live on your back. They’re controlled by nerves that also serve the arms and shoulders rather than the spinal nerves that supply the deeper layers.
Intermediate Layer: Breathing Support
Tucked beneath the superficial muscles are two thin muscles that assist with breathing by moving your ribs:
- Serratus posterior superior: sits between your shoulder blades and helps lift the upper ribs when you inhale.
- Serratus posterior inferior: sits at the bottom of your ribcage and pulls the lower ribs downward during exhalation.
These are also classified as extrinsic muscles. They’re supplied by intercostal nerves, the same nerves that run between your ribs. Most people never feel these muscles working, and their contribution to breathing is modest compared to the diaphragm.
Deep Layer: The Spine’s Support System
The deep, or intrinsic, back muscles are the true back muscles. They run along the spine from the pelvis to the skull and are responsible for extending, rotating, and stabilizing your vertebral column. This is where the muscle count gets complicated, because many of these muscles exist as chains of small segments repeating at each vertebral level. They fall into several groups.
Splenius Muscles
The splenius capitis and splenius cervicis sit in the upper back and neck. They extend and rotate your head and neck, like when you look over your shoulder while driving.
Erector Spinae
This is the largest deep muscle group, forming three parallel columns that run nearly the full length of the spine. The outermost column is the iliocostalis, the middle is the longissimus, and the innermost is the spinalis. Each column has cervical, thoracic, and lumbar subdivisions. Together they keep you upright and let you bend backward and to the side. When people talk about strengthening their “lower back,” they’re usually referring to the erector spinae.
Transversospinalis Group
Beneath the erector spinae sits a group of muscles that run diagonally, connecting the side of one vertebra to the back of a vertebra above it. They’re stacked in three layers of their own:
- Semispinalis: the most superficial of the three, spanning four to six vertebrae at a time. It has capitis, cervicis, and thoracis portions and is a powerful extender of the upper back and neck.
- Multifidus: shorter fibers spanning two to five vertebrae. The multifidus is one of the most important spinal stabilizers, and weakness in these muscles is closely linked to chronic lower back pain.
- Rotatores: the shortest and deepest, connecting one vertebra to the next (rotatores breves) or skipping one level (rotatores longi). They’re most developed in the thoracic spine and contribute to rotation.
Smallest Deep Muscles
At the very deepest level, small muscles bridge adjacent vertebrae. The interspinales connect the bony bumps (spinous processes) of neighboring vertebrae, and the intertransversarii connect the side projections (transverse processes). These muscles are so short and sit so close to the spine’s axis of rotation that they generate very little force. Their real job appears to be sensory: research has found they contain up to six times more stretch receptors than other deep back muscles. They essentially act as motion sensors, feeding your nervous system constant information about spinal position so it can fine-tune the activity of larger muscles around them.
Four small suboccipital muscles also sit at the very top of the spine, just beneath the base of the skull. They fine-tune head movements, particularly the small nodding and rotating adjustments your head makes constantly throughout the day.
How These Layers Work Together
Your back muscles don’t work in isolation. The deep layers fire first to stabilize individual vertebrae, then the erector spinae extends or bends the spine as a whole, and the superficial muscles transfer force between the spine and the arms. Something as simple as picking up a bag of groceries involves all three layers simultaneously. The tiny rotatores and multifidus lock down the vertebrae, the erector spinae extends your trunk, and the latissimus dorsi helps your arm pull the load upward.
This layered system also explains why back pain can be so hard to pin down. A strain in a deep stabilizer muscle like the multifidus feels different from a pulled trapezius, yet both register as “back pain.” The deep muscles are harder to isolate with stretching or strengthening, which is part of why targeted exercises focusing on slow, controlled spinal movements tend to be more effective for chronic back pain than general lifting.
