Breast milk should only be reheated once. Once you warm it, you have a 2-hour window to use it, and any milk left after that should be thrown away. There is no official guideline supporting a second reheating cycle, and the concern is both bacterial growth and loss of the nutrients that make breast milk valuable in the first place.
Why the Limit Is Once
Every time breast milk is warmed, two things happen. First, the warm temperature creates conditions where bacteria can multiply more quickly. Second, heat breaks down the protective proteins and enzymes that give breast milk its immune-boosting properties. One warming cycle, done gently, keeps both risks low. Repeating the process compounds them.
The CDC’s guidance is straightforward: once breast milk is brought to room temperature or warmed, use it within 2 hours. If your baby started feeding and didn’t finish the bottle, that same 2-hour clock applies from the moment the feeding ended. After that, discard the leftover milk. There is no recommendation to refrigerate a warmed bottle and reheat it again later.
What Happens to the Milk When It’s Heated
Breast milk contains proteins like lactoferrin (which fights infection), along with digestive enzymes that help your baby break down fats and carbohydrates. These components are sensitive to heat. Research shows that when milk stays below about 40°C (104°F), which feels lukewarm to the touch, these proteins and enzymes remain largely intact. Once temperatures climb above 50°C (122°F), the damage increases sharply. Lactoferrin, for example, retains over 80% of its concentration below 55°C but drops to just 20% at standard pasteurization temperatures.
Digestive enzymes like lipase and amylase are stable at temperatures under 25°C but begin losing activity between 40°C and 55°C. Each reheating cycle pushes the milk through that damaging temperature range again, so even gentle warming done twice degrades more of these components than warming done once.
The Bacterial Growth Question
One small study tracked bacterial counts in breast milk that had been partially fed to an infant and then refrigerated for up to 48 hours. Surprisingly, the bacteria levels in milk the baby had drunk from were not significantly different from milk that was never exposed to the baby’s mouth, and all samples stayed within safe limits (under 100,000 colony-forming units per milliliter). At 48 hours of refrigeration, partially consumed milk actually had slightly lower average bacterial counts than the unfed milk.
This suggests that breast milk’s natural antimicrobial properties do a reasonable job of keeping bacteria in check, at least under refrigeration. But this was a small study with high variability between participants, and it doesn’t account for the added risk of warming milk a second time, which gives bacteria a fresh window of rapid growth at favorable temperatures. The conservative 2-hour rule exists precisely because conditions vary: room temperature, how long the baby fed, and individual differences in bacterial load all matter.
How to Warm Breast Milk Safely
The goal is to bring the milk to body temperature or slightly below, around 37°C (98.6°F), without overshooting. Place the sealed bottle or bag in a bowl of warm water, or hold it under warm running water for a few minutes. Swirl the container gently to distribute the heat evenly.
Never use a microwave. Microwaves heat unevenly, creating hot spots that can scald your baby’s mouth, and they destroy nutrients more aggressively than gradual warming. Heating directly on the stove carries the same risks of overheating. You also don’t need to warm breast milk at all. It’s perfectly safe to serve it cold or at room temperature if your baby will accept it, and skipping the warming step entirely avoids the nutrient loss question altogether.
Reducing Waste Without Reheating
If you’re throwing away milk regularly because your baby doesn’t finish bottles, a few adjustments help. Store milk in smaller portions, 2 to 3 ounces at a time, so you only warm what your baby is likely to eat. You can always warm a second small portion if they’re still hungry. This is far safer than warming a large bottle, refrigerating the leftovers, and trying to reheat.
For previously frozen milk that you’ve thawed in the refrigerator, the same rules apply once it’s warmed. Use it within 2 hours of warming and do not refreeze it. Thawed milk that hasn’t been warmed can stay in the refrigerator for up to 24 hours, giving you time to portion out only what you need for each feeding.
