How Many Units of Botox Are in a Syringe?

There is no fixed number of Botox units in a syringe. Botox doesn’t come pre-loaded in syringes at all. It arrives at your provider’s office as a freeze-dried powder in a vial, and the practitioner mixes it with saline, then draws the exact number of units needed into a syringe for your treatment. The amount in any given syringe depends entirely on how the provider diluted the powder and how many units your treatment requires.

How Botox Gets From Vial to Syringe

Botox Cosmetic is sold in single-use vials containing either 50 or 100 units of dried powder. Before it can be injected, the provider adds a specific amount of sterile saline to the vial to turn the powder into a liquid. This step is called reconstitution, and the amount of saline used determines how concentrated the final solution is.

The standard dilution recommended by the manufacturer calls for 2.5 mL of saline added to a 100-unit vial (or 1.25 mL for a 50-unit vial). This produces a concentration of 4 units per 0.1 mL of liquid. From there, the practitioner uses a small syringe, typically a 1 mL insulin-style syringe, to draw up exactly the volume needed for your dose. A full 1 mL syringe at this standard dilution holds about 40 units of Botox.

Why Syringe Markings Don’t Show Units

This is where things get confusing. The markings on the syringe measure volume in milliliters, not Botox units. Your provider has to do the math based on their chosen dilution. At the standard 2.5 mL dilution of a 100-unit vial, the conversions work out to roughly:

  • 0.1 mL = about 4 units
  • 0.25 mL = about 10 units
  • 0.5 mL = about 20 units
  • 1.0 mL = about 40 units

If a provider uses more saline, the solution becomes more dilute, meaning fewer units per milliliter. If they use less saline, each milliliter contains more units. The total number of units doesn’t change, just how much liquid carries them. This is why asking “how many units are in a syringe” doesn’t have a single answer. Two syringes filled to the same line could contain very different doses depending on how the Botox was mixed.

How Many Units a Typical Treatment Uses

For cosmetic treatments, the total number of units per session varies by area. Clinical studies that led to FDA approval used these doses for the upper face: 20 units for the frown lines between the eyebrows (glabellar lines), 20 units for the horizontal forehead lines, and 24 units for crow’s feet (12 per side). Treating all three areas together totals about 64 units.

In practice, doses can range a bit. Canada, for instance, approved crow’s feet doses from 12 to 36 units and forehead doses from 8 to 24 units. Your provider adjusts the dose based on your muscle strength, the look you want, and how you’ve responded to previous treatments. A person with strong forehead muscles may need more units than someone with finer muscle activity.

At the standard dilution, a full 100-unit vial reconstituted with 2.5 mL of saline could treat the entire upper face for one patient and still have some left over, or it might be split across multiple syringes for different injection sites during the same appointment.

Why Botox Is Priced by the Unit

Because syringe volume is meaningless without knowing the dilution, Botox is priced per unit rather than per syringe. The average cost falls between $10 and $30 per unit depending on where you live and who’s injecting. Urban practices tend to charge $15 to $25 per unit, while rural areas sometimes price closer to $10 to $15. So a 20-unit frown line treatment might run $200 to $500, and a full upper-face session of 64 units could cost $640 to $1,600 or more.

If a provider quotes you a price “per syringe” rather than per unit, ask how many units that syringe contains and what dilution they use. This is the only way to compare pricing accurately between offices. A cheaper syringe with fewer units isn’t a better deal.

What This Means at Your Appointment

When you sit down for Botox, your provider has already reconstituted the vial and calculated how much liquid to draw for your specific dose. You’ll likely see a small syringe that looks partially filled. That’s normal. Most cosmetic treatments require well under 1 mL of solution total, spread across multiple small injections. The provider may use one syringe for several injection points or prepare separate syringes for different areas of the face.

Some product is inevitably lost during the drawing process. A study in the Journal of Clinical and Aesthetic Dermatology noted that waste is unavoidable when pulling liquid through a vial stopper, which is one reason vials are sized to provide slightly more than the labeled amount. Unlike dermal fillers, which come pre-loaded and ready to inject, all FDA-approved botulinum toxin products currently require this manual reconstitution step.
