Regular-strength acetaminophen tablets are typically 325 mg or 500 mg each. You can take a dose every four to six hours as needed, but the total across 24 hours should never exceed 4,000 mg. For extra-strength products (500 mg per tablet), the recommended ceiling is lower at 3,000 mg per day, with doses spaced at least six hours apart.

Many pharmacists and physicians recommend staying closer to 3,000 mg daily even when using regular-strength products, especially if you’re taking it for more than a few days in a row. Children’s doses are based on weight, not age. If you don’t know your child’s weight, age can serve as a rough guide, but weighing them gives a more accurate dose. For children under 12, acetaminophen can be given every four hours as needed, with a maximum of five doses in 24 hours. Children over 12 can take extra-strength tablets every six hours, up to six tablets per day.

A few hard rules: children under 2 should not receive acetaminophen without a doctor’s guidance. Extra-strength 500 mg products are not for children under 12. Extended-release 650 mg products are not for anyone under 18.

Hidden Acetaminophen in Other Medications

The most common way people accidentally exceed the daily limit is by taking multiple products that all contain acetaminophen without realizing it. Acetaminophen is an ingredient in dozens of over-the-counter and prescription drugs, often listed under the abbreviation “APAP” on prescription labels.

Common OTC products that contain it include NyQuil, Theraflu, Sudafed, Alka-Seltzer Plus Liquid Gels, and Actifed. On the prescription side, painkillers like Vicodin, Percocet, Lortab, Endocet, and Fioricet all include acetaminophen alongside their primary ingredient. If you’re taking any of these and also popping Tylenol for a headache, you could easily blow past 4,000 mg without knowing it.

Before taking any new medication, check the active ingredients on the label. If acetaminophen or APAP appears, add that amount to your daily running total.

How Acetaminophen Damages the Liver

At normal doses, your liver processes acetaminophen efficiently. Most of the drug gets broken down through standard detox pathways and leaves your body in urine. A small fraction, however, gets converted into a toxic byproduct. Your liver neutralizes this byproduct using a natural antioxidant called glutathione, and at recommended doses, the system handles it without trouble.

When you take too much, the normal pathways get overwhelmed. The liver produces more of the toxic byproduct than glutathione can neutralize.
