Most clinical studies use between 160 mg and 480 mg of bilberry extract per day, with 160 mg being the most common single dose. The extract should be standardized to 36% anthocyanins (sometimes labeled as 25% anthocyanosides, which is the same thing measured differently). That standardization is important because it determines how much of the active compound you’re actually getting.
The Standard Dosage Range
Health Canada, which formally evaluates herbal supplements, sets the recommended range at 160 to 480 mg of standardized bilberry extract daily, with no more than 160 mg in a single dose. That means if you’re taking 480 mg per day, you’d split it into three separate doses rather than taking it all at once.
Within that range, the dose that shows up most often in research is 160 mg taken once or twice daily. A study on screen-related eye fatigue used 480 mg per day for eight weeks. Another trial testing similar outcomes used 240 mg once daily for 12 weeks. Both found measurable improvements, though the effects at 240 mg were modest. For general use, starting at 160 mg once or twice daily is a reasonable approach.
What “Standardized” Means on the Label
Bilberry’s benefits come from anthocyanins, the deep-purple pigments that act as antioxidants and support blood vessel health. A raw bilberry fruit contains only a small percentage of these compounds, so extracts concentrate them to a consistent level. The industry standard is 25% anthocyanosides, which translates to roughly 36% anthocyanins. These are two ways of measuring the same compounds, and you’ll see either number on labels depending on the brand.
If a product doesn’t specify its anthocyanin content, or lists a much lower percentage, you can’t reliably compare it to the doses used in research. A 500 mg capsule of a non-standardized bilberry powder delivers far less active compound than a 160 mg capsule of standardized extract.
Fresh Bilberries vs. Extract
One clinical trial testing bilberry’s effect on blood sugar used a 470 mg dose of standardized extract, noting that this was roughly equivalent to 50 grams of fresh bilberries (a little under a quarter cup). That gives you a sense of the concentration involved. To match even the lower end of the supplement range, 160 mg of extract, you’d need to eat fresh bilberries regularly in meaningful quantities. The extract is simply a more practical and consistent way to get a therapeutic dose.
Eye Health: What the Research Shows
Bilberry is best known as an eye supplement, though the evidence is mixed depending on what you’re hoping it will do. The popular claim that bilberry improves night vision has not held up well. A study involving U.S. Navy SEALs found no improvement in night vision or contrast sensitivity after three weeks of taking 160 mg of standardized extract daily.
Where bilberry shows more promise is in reducing eye fatigue from prolonged screen use. At 480 mg per day for eight weeks, both objective and subjective measures of eye strain improved. A lower dose of 240 mg daily over 12 weeks also showed statistically significant improvement, though the effect was small. A separate four-week trial using 200 mg of fermented bilberry extract found meaningful improvement in the eyes’ ability to shift focus, which tends to decline with age and nearsightedness.
Vascular and Blood Sugar Effects
Bilberry extract has a long history of use for circulation problems, particularly in the legs. The anthocyanins help strengthen capillary walls and reduce inflammation in blood vessels. The dosage range used for vascular support falls within the same 160 to 480 mg window, typically split into divided doses throughout the day.
There is also early evidence that bilberry may help with blood sugar management. A small trial in people with type 2 diabetes found that a single 470 mg dose of standardized extract changed how the body handled glucose after a meal. This research is preliminary and involved very few participants, so it’s not a basis for replacing any existing approach to blood sugar control. But it does suggest that bilberry’s effects extend beyond the eyes.
Safety and Interactions
Bilberry extract is generally well tolerated. Animal research examining a range of health markers found no negative effects on organ function, blood chemistry, or antioxidant status, and no drug interactions from chronic or high-dose intake have been formally documented in that setting.
The most important safety concern involves blood thinning. Bilberry can slow clotting by affecting platelet activity. At least one case of excessive bleeding has been reported in a person who had been consuming bilberry long-term and then started a prescription blood thinner. If you take warfarin, aspirin, or anti-inflammatory painkillers like ibuprofen or naproxen, bilberry may amplify their blood-thinning effects. This doesn’t mean you can’t take bilberry, but it means the combination needs to be managed carefully.
How to Take It
There is no strong evidence favoring a specific time of day for bilberry, and research trials haven’t systematically compared absorption with or without food. Since anthocyanins are water-soluble, they absorb reasonably well on their own. Taking bilberry with a meal is a common and practical approach that may reduce any mild stomach sensitivity.
If your daily dose exceeds 160 mg, splitting it into two or three doses spread throughout the day aligns with how clinical trials have structured their dosing and stays within the recommended single-dose ceiling. Most studies lasted between 4 and 12 weeks, so giving bilberry at least a month before evaluating whether it’s working for you is reasonable.
