Caffeine typically raises blood pressure by about 5 to 10 mmHg systolic (the top number) and 3 to 5 mmHg diastolic (the bottom number) within an hour of consumption. That spike is temporary in most people, but the size of the increase depends on how often you drink caffeine, whether you already have high blood pressure, and even your genetics.

The Typical Increase After a Cup of Coffee

When researchers measure blood pressure before and after caffeine intake, the diastolic reading (bottom number) consistently rises by about 4 mmHg within 60 to 90 minutes. Central blood pressure, which measures pressure closer to the heart, tends to jump a bit more: roughly 5 mmHg systolic and 4 mmHg diastolic in studies using a standard cup of regular coffee. These numbers come from healthy adults with normal blood pressure at baseline, so they represent the mild end of the range.

If you already have high blood pressure, the spike is larger. In a study that gave participants about 250 mg of caffeine (roughly one strong cup of coffee), men with diagnosed hypertension experienced a blood pressure increase more than 1.5 times greater than people with optimal readings. After that single dose, 89% of the diagnosed hypertensive group crossed into the hypertensive range (140/90 or above). By comparison, everyone in the normal and optimal groups stayed within safe limits.

Why Caffeine Raises Blood Pressure

Your body naturally produces a molecule called adenosine that relaxes blood vessel walls and helps keep blood pressure in check. Caffeine blocks the receptors that adenosine binds to, effectively canceling out that relaxation signal. With adenosine sidelined, blood vessels constrict slightly, and your heart pumps against more resistance. The result is a measurable bump in pressure that shows up on a blood pressure cuff within about 30 minutes and peaks around 45 to 60 minutes after you drink your coffee.

How Long the Spike Lasts

For most people, the blood pressure increase is brief. It peaks within the first hour and generally fades over the next few hours as your body metabolizes the caffeine. The effect is most pronounced in people who don’t drink caffeine regularly. If you’re an occasional coffee drinker who has a cup before a doctor’s appointment, your reading may look higher than usual. If you drink coffee every morning, your body has likely adapted.

Tolerance Makes a Big Difference

Regular caffeine drinkers develop a tolerance that blunts the blood pressure response significantly. People who consume caffeine daily don’t experience the same short-term spike as occasional users, and habitual consumption is not linked to a higher long-term risk of developing hypertension. This is one of the clearest findings in the research: caffeine’s effect on blood pressure is primarily an acute phenomenon in people whose bodies aren’t used to it.

That said, tolerance has limits. If you normally drink one cup a day and suddenly have three or four, you can still see a meaningful increase because you’ve exceeded the dose your body has adapted to.

Genetics Play a Role

Your liver processes caffeine using a specific enzyme, and the gene that controls this enzyme comes in two common variants. People with the “fast metabolizer” version break down caffeine quickly and tend to see smaller blood pressure effects. People with the “slow metabolizer” version keep caffeine circulating in their blood longer, and studies show they experience a larger systolic increase after the same dose. In one study, only the slow metabolizers had a significant rise in systolic blood pressure after caffeine, while fast metabolizers showed no meaningful change.

You can’t easily tell which version you carry without genetic testing, but if you’ve always noticed that coffee makes you jittery or keeps you up at night, you may be a slow metabolizer, and you’re likely experiencing a bigger blood pressure spike too.

Coffee vs. Energy Drinks

The caffeine in energy drinks raises blood pressure through the same mechanism as coffee, but energy drinks often come with additional ingredients like taurine, guarana, and high sugar content that can amplify cardiovascular effects. Nutritional scientists at the University of Colorado have noted that energy drinks can raise blood pressure and heart rate, trigger irregular heart rhythms, and that people with high blood pressure, heart disease, or diabetes should avoid them entirely. If you’re choosing between the two, plain coffee is the safer option.

How Much Caffeine Is Considered Safe

The FDA considers up to 400 milligrams of caffeine per day safe for most healthy adults. That’s roughly two to three 12-ounce cups of brewed coffee. This threshold is based on general health effects, not blood pressure specifically, so if you have hypertension or are sensitive to caffeine, your practical limit may be lower.

A useful strategy if you’re monitoring your blood pressure: avoid caffeine for at least 30 minutes before taking a reading at home. This gives you a more accurate baseline and prevents caffeine from inflating your numbers. If your blood pressure runs borderline high, paying attention to timing and dose matters more than cutting caffeine out completely.
