Clonazepam is not a blood pressure medication, but it can lower blood pressure as a side effect. The drop varies widely depending on the person, but research on older adults taking benzodiazepines found their resting systolic blood pressure was about 12 mmHg lower than non-users (149 versus 161 mmHg on average). The effect is generally modest in most people and comes primarily from the drug’s calming action on the nervous system rather than any direct effect on blood vessels.
How Clonazepam Affects Blood Pressure
Clonazepam works by enhancing the activity of GABA, the brain’s primary calming chemical. GABA normally acts as a brake on the nervous system, slowing down the signals that trigger your fight-or-flight response. When clonazepam amplifies this braking effect, it reduces the nervous system’s output of stress signals, which in turn slows heart rate, relaxes blood vessel walls, and lowers blood pressure.
Animal research supports a direct connection between GABA activity and blood pressure regulation. In rats bred to have high blood pressure, researchers found reduced GABA receptor density in two key brain areas: the hypothalamus and the amygdala. Both regions help regulate cardiovascular arousal. The deficit in GABA signaling in these areas appeared to play a role in both initiating and maintaining hypertension, which helps explain why boosting GABA activity with a drug like clonazepam can bring pressure down.
That said, the blood pressure drop most people experience from clonazepam is largely secondary to anxiety relief. When you’re anxious, your body releases stress hormones that constrict blood vessels and speed up your heart. Clonazepam quiets that response. If anxiety or panic is spiking your blood pressure, the reduction can be noticeable. If your blood pressure is already normal and you’re calm, the effect is typically minimal.
What the Numbers Look Like
There is no standard “X milligrams lowers blood pressure by Y points” formula for clonazepam because it was never designed or tested as a blood pressure drug. The research that exists is limited, but a study on elderly patients with labile hypertension (blood pressure that swings unpredictably) found that clonazepam reduced systolic blood pressure fluctuations by about half and diastolic fluctuations by about 40%. This worked in 82% of the patients studied. The drug didn’t necessarily bring blood pressure to a fixed lower number; it stabilized it so it stopped spiking.
A separate study on older adults found that benzodiazepine users had baseline systolic readings roughly 12 mmHg lower than non-users. That’s a meaningful gap, roughly comparable to what a mild blood pressure medication might achieve, but it’s hard to separate the drug’s effect from the possibility that these patients were simply less anxious or had other health differences.
The Orthostatic Hypotension Risk
One blood pressure effect of clonazepam that matters practically is what happens when you stand up. Research on older adults found that benzodiazepine users experienced a significantly larger drop in systolic blood pressure within 10 seconds of standing compared to non-users, with the difference peaking at about 21 mmHg. At that point, their blood pressure was still falling while non-users’ pressure had already started recovering.
This exaggerated drop when changing positions is called orthostatic hypotension, and it’s one of the main reasons benzodiazepines increase fall risk in older adults. You might feel dizzy, lightheaded, or briefly like you might faint, especially in the first few days of taking the medication, after a dose increase, or if you stand up quickly. This effect held up even after researchers accounted for age, frailty, other medications, and existing high blood pressure.
Interactions With Blood Pressure Medications
If you already take medication for high blood pressure, adding clonazepam can produce additive effects. Combining clonazepam with a calcium channel blocker like amlodipine, for example, can cause headache, dizziness, lightheadedness, and fainting due to the combined blood pressure lowering. These symptoms are most common when first starting the combination, after a dose change, or when restarting treatment after a break.
The same principle applies to other classes of blood pressure medications, including beta-blockers and ACE inhibitors. The clonazepam doesn’t interfere with how these drugs work chemically, but the effects stack. Your blood pressure may dip lower than either medication would cause on its own, particularly when standing up or during periods of physical exertion.
What Happens When You Stop
Stopping clonazepam abruptly can cause the opposite problem. Withdrawal from benzodiazepines can trigger a rebound spike in blood pressure along with rapid heartbeat, sweating, tremor, and agitation. Because clonazepam is a longer-acting benzodiazepine, withdrawal symptoms may not appear immediately. They can begin anywhere from a few days to three weeks after the last dose.
This rebound hypertension happens because your nervous system has adapted to the calming effect of the drug. When that brake is suddenly removed, the sympathetic nervous system overcompensates, flooding the body with stress signals. The effect can be significant enough to be medically dangerous, which is why gradual tapering under supervision is standard practice rather than stopping cold.
Why It’s Not Prescribed for High Blood Pressure
Despite its ability to lower blood pressure, no clinical guidelines recommend clonazepam for treating hypertension. The FDA approves it for anxiety, panic disorder, and seizures. The blood pressure effects are inconsistent, dose-dependent in unpredictable ways, and come bundled with sedation, cognitive impairment, dependence risk, and the potential for serious withdrawal. Dedicated blood pressure medications achieve more reliable, targeted results without these tradeoffs.
Where clonazepam’s blood pressure effect does come into play clinically is in situations where anxiety is clearly driving temporary blood pressure spikes, such as white-coat hypertension or panic attacks that send readings soaring. In those cases, treating the anxiety treats the pressure spike, but the clonazepam is prescribed for the anxiety itself, not as a cardiovascular intervention.
