The most common side effects are drowsiness and dry mouth, which affect a substantial number of people who take it. The Rebound Problem
The most serious concern with clonidine is what happens if you stop taking it suddenly. Because the drug works by calming the sympathetic nervous system (your body’s “fight or flight” wiring), abruptly removing it can cause a surge of stress hormones. This rebound effect can produce a dangerous spike in blood pressure, sometimes within 24 hours of the last dose.
In one documented case, an 86-year-old woman developed severe hypertension and a heart attack 36 hours after her clonidine was stopped. Cardiac testing showed no blockage in her arteries. The heart attack was caused purely by the excessive demand placed on her heart from the blood pressure spike. This is why every clinical guideline emphasizes that clonidine must be tapered gradually, never stopped cold.
The rebound risk also matters in practical, everyday situations. If you miss doses because of a pharmacy delay, travel, or simply forgetting, you’re at risk for a withdrawal spike that could push your blood pressure higher than it was before you started the medication. This is one of the key reasons prescribers prefer other drug classes that don’t carry this risk.
