That number surprises many people who expect their temperature to return to a perfect 98.6°F after taking it. Tylenol takes the edge off a fever rather than eliminating it entirely, and understanding the realistic range helps you set the right expectations.

How Many Degrees It Actually Lowers

In studies of febrile children, acetaminophen lowered body temperature by an average of 0.78°C (about 1.4°F) per dose, based on analysis of nearly 5,000 doses. In critically ill adults, the average reduction was 0.86°C (roughly 1.5°F), compared to 0.56°C in patients whose fevers went untreated. So Tylenol does work, but the net advantage over letting a fever resolve on its own is modest, around half a degree Fahrenheit in some cases.

The difference was small but consistent, and it grew slightly larger as time went on.

Ibuprofen also tends to last a bit longer, with effects stretching closer to the six-hour end of the window, while Tylenol’s relief often fades closer to four hours. For people who can take either medication safely, ibuprofen has a slight edge for fever specifically. But Tylenol remains the better option for anyone who needs to avoid anti-inflammatory drugs, including people with certain stomach or kidney conditions and very young infants.

Dosing for Adults

The standard adult dose for fever is 500 to 1,000 mg every four to six hours. The FDA sets the maximum at 4,000 mg per day across all acetaminophen-containing products, and that “all products” part is critical. Acetaminophen hides in hundreds of combination medications, including cold and flu remedies, sleep aids, and prescription painkillers. It’s easy to double up without realizing it.

Many doctors recommend staying closer to 3,000 mg per day as a practical ceiling, especially if you’re taking it for more than a few days or if you drink alcohol regularly. Liver toxicity from acetaminophen typically requires much higher doses (above 10 to 15 grams in a single episode), but chronic overuse at lower levels can still cause damage over time.

Children’s doses are based on weight, not age. The standard is 10 to 15 mg per kilogram of body weight per dose, given every four to six hours, with no more than five doses in 24 hours.
