How Much Vitamin B12 Per Day Does a Woman Need?

The recommended daily intake of vitamin B12 for most adult women is 2.4 micrograms (mcg). That number rises slightly during pregnancy (2.6 mcg) and breastfeeding (2.8 mcg), and women over 50 often need to pay closer attention to how they get their B12 rather than how much they consume. These recommendations come from the Food and Nutrition Board at the National Institutes of Health and apply to all healthy adult women aged 19 and older.

Daily B12 by Life Stage

For women between 19 and 50, the target is straightforward: 2.4 mcg per day from food, supplements, or a combination. Most women who eat animal products regularly meet this without thinking about it, since B12 is naturally present in meat, fish, eggs, and dairy.

During pregnancy, the recommendation increases to 2.6 mcg to support fetal nervous system development. Breastfeeding pushes it slightly higher to 2.8 mcg, since B12 passes through breast milk to the infant. Prenatal vitamins typically cover this increase, but it’s worth checking the label to confirm.

Why Absorption Changes After 50

The RDA stays at 2.4 mcg for women over 50, but how your body handles B12 shifts significantly. As you age, your stomach produces less acid, and that acid is essential for separating B12 from the proteins in food. Without enough stomach acid, B12 from a steak or a glass of milk can pass right through without being absorbed. The NIH specifically recommends that adults over 50 get most of their B12 from fortified foods or supplements, because the synthetic form doesn’t require stomach acid to be absorbed.

This doesn’t mean you need a higher dose. It means the 2.4 mcg target becomes harder to hit through diet alone, even if you’re eating plenty of animal products.

Foods That Provide B12

Animal-based foods are the only reliable natural sources. A 3-ounce serving of clams provides well over 50 mcg, making it one of the richest sources available. Beef liver is similarly dense in B12. More common choices like salmon, tuna, beef, and milk each deliver meaningful amounts per serving. A single 3-ounce portion of cooked salmon provides around 4.8 mcg, which is double the daily recommendation on its own. Two large eggs provide roughly 1.1 mcg combined, and a cup of milk adds about 1.2 mcg.

Fortified breakfast cereals are one of the few plant-friendly options, with some brands providing 100% of the daily value per serving. Nutritional yeast, popular in vegan cooking, is another commonly fortified option. If you rely on fortified foods, check labels carefully because B12 content varies widely between brands.

Who Needs Supplements

Several groups of women are at higher risk of falling short on B12, even with a healthy diet.

  • Vegans and strict vegetarians: Plant foods contain no reliable natural B12. Supplementation or consistent use of fortified foods is the only way to meet the daily requirement on a fully plant-based diet.
  • Women over 50: Reduced stomach acid makes food-based B12 harder to absorb, making a supplement or fortified foods the more dependable route.
  • Women taking metformin: This common diabetes medication can reduce B12 levels, potentially affecting up to 1 in 10 people who take it. The risk increases with higher doses and longer use. If you take metformin, periodic monitoring of your B12 levels is recommended.
  • Women on acid-reducing medications: Proton pump inhibitors and similar drugs lower stomach acid, which directly impairs B12 absorption from food.
  • Women with digestive conditions: Celiac disease, Crohn’s disease, and other conditions affecting the small intestine can interfere with B12 uptake.

Supplements commonly come in doses of 500 to 1,000 mcg, which is far above the 2.4 mcg recommendation. This isn’t dangerous. Your body absorbs only a small fraction of any oral dose and excretes the rest. The Food and Nutrition Board has not established an upper limit for B12 because of its low potential for toxicity. Even at large doses, B12 is generally considered safe because the body doesn’t store excess amounts.

Cyanocobalamin vs. Methylcobalamin

Most B12 supplements contain one of two forms. Cyanocobalamin is the synthetic version found in the majority of supplements and fortified foods. Methylcobalamin is a naturally occurring form that the body uses directly without needing to convert it. Both work, and the practical differences are small. One study found the body absorbed about 49% of a 1 mcg dose of cyanocobalamin compared to 44% of the same dose of methylcobalamin. However, another study found that three times as much cyanocobalamin was excreted through urine, suggesting methylcobalamin may be retained better. Overall, research suggests the differences in bioavailability are modest and influenced by individual factors like age and genetics.

Either form will prevent and correct a deficiency. Cyanocobalamin tends to be cheaper and more widely available, which is why it remains the default in most products.

Some practitioners consider levels between 160 and 300 pg/mL a gray zone where symptoms may already be present, particularly neurological ones, even though the number isn’t flagged as critically low.
