When Fetal Hiccups Start and Peak

Most people first notice the telltale rhythmic jolts sometime between weeks 21 and 24. The sensation typically becomes more noticeable and frequent as the baby grows through the second and third trimesters, with the third trimester being the period when most people are aware of hiccups on a daily basis.

A few episodes per day is well within the range of normal. A commonly cited guideline is that anything up to about four episodes a day after 32 weeks is unremarkable. Some babies hiccup more than others, just as some are more active kickers. What matters most is consistency with your baby’s own pattern rather than hitting a specific number.

What Fetal Hiccups Feel Like

Hiccups feel distinctly different from kicks or rolls. They show up as repeated, rhythmic jerky movements in one spot, almost like a tiny pulse. The spacing between each jolt is regular, unlike the unpredictable nature of kicks and stretches. Some people describe it as a gentle tapping or twitching low in the belly. In later pregnancy, you can sometimes see the rhythmic movements from the outside.

The key identifier is the repetition. A kick comes and goes. Hiccups repeat at a steady cadence, usually every few seconds, for anywhere from one to several minutes (though some bouts stretch to an hour).

Why Babies Hiccup Before Birth

Fetal hiccups appear to play a role in brain development. Research from University College London found that each time a newborn hiccups, the diaphragm contraction triggers a large wave of brain signals: two prominent brainwaves followed by a third. These signals help the brain’s sensory circuits learn to recognize and monitor the breathing muscles. Over time, this feedback loop may be how babies eventually gain voluntary control of the diaphragm, the muscle responsible for breathing.

At birth, the neural circuits that process body sensations are not fully developed. Hiccups, which send strong sensory input from the diaphragm to the developing brain, appear to help build those circuits during a critical window. This is why hiccups are so frequent during the last weeks of pregnancy and the first weeks after birth: the brain is actively wiring itself to understand what the respiratory muscles are doing. The diaphragm can encode these signals as early as ten weeks before the average time of birth.

Are Frequent Hiccups a Concern?

In nearly all cases, no. One theory that gained traction online suggested that fetal hiccups might be a response to umbilical cord compression, with the idea being that hiccupping prompts the baby to shift position and relieve pressure on the cord. However, the study behind this hypothesis involved fetal sheep, and the connection has never been scientifically studied in human fetuses. As one UT Southwestern physician noted, there is no published data or clinical experience linking fetal hiccups to poor pregnancy outcomes.

A 2017 study reinforced this. Researchers compared the experiences of 150 women who had third-trimester stillbirths with 500 women who had live births. About 80 percent of women in both groups recalled feeling fetal hiccups, with no significant difference between the two groups. Fetal hiccups were equally common in healthy pregnancies.

That said, if you notice more than four episodes of hiccups a day after 32 weeks, or if the hiccups suddenly feel much stronger than usual, it’s worth mentioning to your provider.
