How Often Do Cats Get Rabies? Risk, Signs & Prevention

Rabies in cats is uncommon in the United States, but cats are consistently the most frequently reported rabid domestic animal in the country. Among pets, cats test positive for rabies more often than dogs, largely because vaccination rates in cats tend to be lower and because cats are more likely to roam outdoors and encounter infected wildlife. The CDC estimates that exposure to rabid or potentially rabid cats costs roughly $33 million per year in post-exposure treatment for people.

Why Cats Top the List Among Pets

Cats are natural hunters, and that instinct puts them in direct contact with the wildlife species that carry rabies: raccoons, skunks, bats, and foxes. A cat that spots a bat fluttering near the ground or corners a raccoon in a yard is far more likely to engage than a dog on a leash. Feral and stray cats face even higher risk because they lack vaccination and live in environments where wildlife encounters are routine.

Vaccination campaigns over the past several decades have dramatically reduced rabies in dogs, but cats have not benefited equally. Many cat owners skip rabies boosters, and feral cat colonies often go entirely unvaccinated. In an August 2024 outbreak in Maryland, a single rabid feral cat in an unmanaged colony led to three people needing post-exposure treatment. That kind of event illustrates how one unvaccinated cat in a group can create a public health emergency quickly.

The Pattern Outside the U.S.

In countries where dog rabies has been controlled through mass vaccination, cat rabies is emerging as a growing concern. Brazil offers a clear example. Between 2003 and 2013, surveillance in the state of São Paulo found that cats tested positive for rabies at ten times the rate of dogs. The primary driver was spillover from bats: half of confirmed cat rabies cases in Brazil between 2011 and 2022 were caused by a virus variant commonly found in bats. As cities expand into bat habitats and feral cat populations grow, the overlap between the two species increases.

How Cats Contract Rabies

A cat gets rabies through a bite from an infected animal. Saliva carrying the virus enters the wound, and the virus travels along nerves toward the brain. The incubation period in cats averages about two months but can range from two weeks to several months, sometimes even longer, depending on where the bite occurred and how much virus was transmitted. A bite on the face or neck, closer to the brain, tends to produce a shorter incubation period than a bite on a hind leg.

What Rabies Looks Like in a Cat

Rabies in cats tends to follow a “furious” pattern more consistently than in dogs. Early signs include behavior changes: a friendly cat may become aggressive, or a normally independent cat may become unusually clingy. Cats in the furious phase may attack without provocation, vocalize excessively, or seem disoriented.

The paralytic phase typically begins about five days after the first symptoms appear. The cat loses coordination, develops weakness in the hind legs, and eventually becomes paralyzed. Coma and death follow. Most cats die within three to four days of showing symptoms, and the entire clinical course from first signs to death usually lasts one to ten days. Once symptoms appear, rabies is virtually always fatal. There is no treatment.

How Vaccination Protects Your Cat

Rabies vaccines for cats are highly effective. After an initial shot at 12 weeks of age followed by a booster one year later, modern vaccines provide protection for at least three years. Some vaccines are labeled for one-year duration, others for three years, but studies have confirmed that a complete primary vaccination series followed by a one-year booster generates a strong, sustained immune response that holds up against rabies challenge three years later.

Your local laws determine whether your cat needs annual or triennial boosters. Most U.S. states require rabies vaccination for cats, though enforcement varies. Even if your cat lives entirely indoors, a bat can enter a home through a chimney, attic, or open window, making vaccination worthwhile regardless of lifestyle.

What Happens After a Potential Exposure

If your cat has been potentially exposed to a rabid animal, what happens next depends entirely on vaccination status. A cat that is current on its rabies vaccine receives wound care, a booster shot, and then goes home under your supervision for a 45-day observation period. A cat that is overdue for a booster but has documented proof of at least one prior vaccination gets the same treatment: wound care, booster, and 45 days of observation.

The situation is far more serious for cats with no vaccination history. The recommendation from veterinary authorities is euthanasia. If you decline, the alternative is a minimum four-month quarantine, during which the cat must remain isolated and show no symptoms. That quarantine is costly, stressful for the animal, and only permitted with approval from local rabies control authorities. The difference between a 45-day watch at home and a four-month quarantine (or euthanasia) comes down to whether the cat was vaccinated.

Indoor Cats and Actual Risk

An indoor cat that never encounters wildlife faces extremely low risk. But “indoor” is not always airtight. Cats escape through doors, screens tear, and bats occasionally find their way inside. Bat bites can be so small that owners never notice them. If you wake up to find a bat in a room with your cat, that counts as a potential exposure even if you see no visible bite wound.

Outdoor and indoor-outdoor cats face meaningfully higher risk, especially at dawn and dusk when wildlife is active. Cats in rural areas or near wooded land encounter raccoons, skunks, and foxes more frequently. Feral cat colonies near urban green spaces are particularly vulnerable because they combine lack of vaccination with consistent wildlife contact.
