How Painful Is an Achilles Tear: What to Expect

An Achilles tendon tear is widely described as extremely painful, though the initial sensation often surprises people. Rather than a gradual buildup, most people feel a sudden, sharp pain in the back of the ankle accompanied by the sensation of being kicked or struck in the calf. Many hear an audible pop or snap at the moment it happens. The intensity is immediate and severe, but the nature of the pain changes significantly as you move from injury through treatment and recovery.

What the Moment of Injury Feels Like

The most consistent description from people who rupture their Achilles tendon is that it feels like someone kicked them hard in the back of the leg. Many instinctively turn around to see who hit them, only to realize no one did. That impact-like sensation is followed immediately by sharp pain concentrated in the back of the heel and lower calf.

A popping or snapping sound often accompanies the tear, which can be loud enough for bystanders to hear. This sound comes from the tendon fibers separating. After the initial shock, some people describe the pain settling into a deep, throbbing ache, while others report a surprising drop in sharp pain within minutes as the torn ends of the tendon stop pulling against each other. That temporary reduction can be misleading, making some people think the injury is less serious than it is.

Partial Tears vs. Full Ruptures

Not all Achilles injuries hurt the same way. Achilles tendon injuries are graded on a scale of 1 to 3, where grades 1 and 2 are partial tears and grade 3 is a complete rupture. The word “rupture” typically refers only to that full, grade 3 tear where the tendon separates entirely.

With a partial tear, you may still feel that kicked-in-the-calf sensation, but you can often still walk, though with noticeable pain. Bruising and swelling develop around the back of the ankle, and the pain gets worse during activities that load the tendon: walking, running, jumping, or pushing off to climb stairs. Daily movement is painful but possible.

A complete rupture is a different experience. Walking becomes extremely difficult, especially on inclines or uneven ground. The tendon swells and thickens visibly, and the area around the back of the heel becomes tender to the touch. Putting weight through the foot feels unstable because the tendon can no longer transmit force from the calf muscle to the foot. Most people cannot walk normally and need immediate help getting around.

Pain Levels After Surgery

If you have surgical repair, the post-operative pain follows a fairly predictable arc. The first three weeks tend to be the most uncomfortable. During this phase, clinicians expect pain levels to stay below a 5 out of 10 before you progress to the next stage of recovery. You’ll likely be prescribed pain medication for this period, along with instructions to ice the area and keep your leg elevated above heart level for the first three days to control swelling.

Between weeks 4 and 6, you begin partial weight-bearing in a protective boot with heel wedges. The goal during this phase is to gradually increase how much body weight you put through the leg, typically adding about 25% of your body weight per week until you can stand fully on the surgical side without pain. By this stage, pain should be dropping below a 3 out of 10. Some people find the transition from crutches to walking in the boot surprisingly uncomfortable, as the tendon begins bearing load again for the first time.

From weeks 7 through 12, the benchmark shifts. Instead of tolerating a certain pain level, the goal becomes zero pain and zero swelling after exercise and rehab sessions. This standard stays in place for months. Any return of pain or swelling during this period is a signal to back off and let the tendon recover before progressing.

What Hurts During Rehabilitation

Recovery from an Achilles repair typically spans 3 to 6 months before you return to activities like running, and longer before high-intensity sports. Certain milestones along the way tend to be more uncomfortable than others.

The shift to weight-bearing around week 4 is the first notable pain point. Your calf muscles have weakened rapidly during immobilization, and the repaired tendon is still fragile. Loading it again produces soreness and stiffness that many people describe as a deep ache rather than sharp pain.

Later in rehab, when you begin strengthening exercises that involve lowering your heel below a step (eccentric loading), the tendon is placed under controlled stress for the first time. This phase is essential for building tendon strength, but it commonly causes temporary soreness. The same applies when you progress to a return-to-running program or plyometric drills like hopping and jumping. Each of these stages should be pain-free during and after the activity. If they aren’t, it’s a sign you’ve progressed too quickly.

Nerve-Related Pain

A small nerve runs along the outer edge of the Achilles tendon and provides sensation to the outside of the foot and ankle. In some cases, the force of the rupture itself or swelling around the injury site can stretch or compress this nerve. When that happens, you may feel numbness, tingling, or a burning sensation along the outer foot in addition to the typical tendon pain.

This nerve involvement is relatively uncommon, but it can also occur as a complication of surgery. If numbness or tingling develops after a rupture or after surgical repair and doesn’t resolve within a few days, it may indicate nerve irritation that needs separate evaluation. The sensation is distinct from the deep, aching pain of the tendon injury itself, feeling more like pins and needles or a patch of skin that’s fallen asleep.

How Long the Pain Lasts Overall

The sharp, acute pain from the initial tear typically lasts a few days to a week before transitioning into a more manageable ache, especially once the leg is immobilized. Surgical pain adds another layer, with the most intense discomfort concentrated in the first two to three weeks post-operation. Most people stop needing prescription pain relief within that window.

The longer tail of recovery involves low-grade soreness and stiffness rather than acute pain. This can persist for several months as you rebuild strength and flexibility. Many people report that by three months post-surgery, day-to-day pain is minimal, but the tendon still feels stiff or tight during physical activity. Full return to sport, with no pain during explosive movements, generally takes six months or longer. The overall experience is front-loaded: the injury and early recovery are intensely painful, but each week tends to feel meaningfully better than the last.
