How Painful Is Knee Replacement Surgery? A Timeline

Knee replacement surgery involves significant pain, but it’s shorter-lived than most people expect. At rest, patients typically rate their pain around 4 to 5 out of 10 during the first 24 hours, dropping to about 3.5 out of 10 by the 48-hour mark. The first two weeks are the hardest stretch, and pain steadily decreases from there. By 12 weeks, most people rate their pain at 1 out of 10 or less.

That said, pain during recovery isn’t constant or uniform. It spikes during physical therapy, worsens at night, and changes character as healing progresses. Understanding the full timeline helps you prepare for what’s actually ahead.

Pain During the First 48 Hours

You won’t feel much immediately after surgery because the anesthesia and nerve blocks are still working. As those wear off, pain builds. You’ll still feel soreness after long walks or therapy sessions, and the knee may ache after sitting in one position too long, but the constant background pain of the early weeks is largely gone.

By 12 weeks, pain scores in clinical studies average under 1 out of 10. Many patients describe their knee as “stiff” or “aware” rather than painful. This is the point where most people notice that their new knee actually hurts less than the arthritic knee they replaced.

Night Pain and Sleep Disruption

  • During surgery: The surgeon injects a numbing cocktail directly into the tissues around the joint. This provides hours of targeted relief without affecting leg strength.
  • Nerve blocks: An anesthesiologist may numb specific nerves near the knee before or during surgery. The most common targets the inner thigh nerve, which controls knee pain signals without weakening your ability to walk.
  • Anti-inflammatory medications: These are a mainstay of recovery and reduce both pain and swelling. They also decrease the amount of stronger medication you need.
  • Acetaminophen: Taken on a schedule rather than as needed, it provides a baseline level of pain control.
