Feraheme (ferumoxytol) starts raising hemoglobin levels within days, with a meaningful increase typically seen around 16 days after infusion. Most people notice symptom improvements like reduced fatigue within a few days to one week. Full correction of iron deficiency anemia, however, takes closer to two months.
How Feraheme Delivers Iron
Feraheme is an IV iron product made of iron oxide particles coated in a carbohydrate shell. That shell keeps the iron inactive while it circulates in your blood. Once the particles are taken up by specialized immune cells in your liver, spleen, and bone marrow, the shell breaks down and the iron is released inside those cells. From there, the iron either goes into your body’s storage reserves or gets picked up by a transport protein in the blood and shuttled to the bone marrow, where it’s built into new red blood cells.
This process means the iron doesn’t flood your bloodstream all at once. It’s gradually released and put to work, which is why lab values and symptoms improve over days and weeks rather than hours.
When Hemoglobin Levels Start Rising
In a Phase III clinical trial comparing Feraheme to another IV iron product, the median time to a hemoglobin increase of 2 g/dL (or to reach a hemoglobin of 12 g/dL) was 16 days. That was significantly faster than the comparison group, which took 22 days to hit the same benchmark. The hemoglobin increase tends to begin within the first week but starts to plateau somewhere between 5 and 14 days after infusion.
Ferritin, which reflects your body’s iron stores rather than active red blood cell production, responds a bit differently. Ferritin levels typically show an effect within 7 to 9 days after infusion. Because ferritin can spike temporarily right after IV iron, most providers wait several weeks before ordering follow-up bloodwork to get an accurate picture of your iron stores.
When You’ll Feel the Difference
If you’re getting Feraheme for iron deficiency anemia, the symptoms you’re most eager to resolve (fatigue, brain fog, shortness of breath, feeling cold) generally start improving within a few days to a week after infusion. Some people feel noticeably better within 48 hours, while others take the full week.
That early improvement doesn’t mean your iron levels are fully corrected. Completely rebuilding your iron stores and resolving anemia can take about two months. So while you may feel significantly better in the short term, the full biochemical recovery is a longer process.
Dosing Schedule
Feraheme is given as two separate IV infusions. Each infusion runs over at least 15 minutes, and you’ll be monitored for at least 30 minutes afterward. The two doses are spaced apart to allow your body to process the first round of iron before adding more. For most people, the total time commitment at the infusion center is under an hour per visit.
What to Expect During the Infusion
Feraheme carries an FDA boxed warning for the risk of serious allergic reactions, including anaphylaxis. In clinical trials, serious hypersensitivity reactions occurred in about 0.2% to 0.4% of patients. Milder reactions like itching, rash, hives, or wheezing were more common, affecting roughly 3.5% of patients in one large study. These reactions can happen even if you tolerated a previous dose without any problems, which is why monitoring after each infusion is required.
More routine side effects can include headache, nausea, dizziness, or mild discomfort at the injection site. These typically resolve within a day or two.
How Feraheme Compares to Oral Iron
Feraheme is specifically designed for people who haven’t responded well to oral iron supplements or can’t tolerate them. Oral iron can take three to six months to fully correct anemia, and many people experience stomach pain, constipation, or nausea that makes it hard to stick with. Feraheme delivers a large dose of iron directly into your bloodstream in just two visits, bypassing the digestive system entirely. The tradeoff is the need for supervised infusions and a small risk of allergic reaction, but the speed advantage is substantial: measurable hemoglobin improvement in about two weeks versus months with pills.
