A properly fitting breast pump flange allows only your nipple to be pulled into the tunnel, with the sides of your nipple lightly touching the tunnel walls and gliding gently back and forth during suction. If you’re experiencing pain, redness, or poor milk output, the flange size is likely off. Getting this right makes the difference between pumping sessions that work and ones that leave you sore and frustrated.
What a Good Fit Looks Like
When a flange fits correctly, three things happen simultaneously. First, only your nipple enters the tunnel, not large amounts of surrounding areola tissue. Second, the sides of your nipple touch the walls of the tunnel without being compressed or squeezed. Third, your nipple moves a little bit back and forth inside the tunnel with each suction cycle, gliding rather than sticking or dragging.
You should be able to see a small amount of space around the nipple as it moves, but not so much that the areola gets pulled deep into the tunnel. Think of it like a door in a door frame: the door should fit snugly but still swing freely. If there’s no movement at all, the flange is too tight. If there’s a wide gap and your areola is being sucked in alongside your nipple, it’s too loose.
Pumping with a correct fit should feel like a firm tugging sensation, not sharp or pinching. Some mild discomfort in the first few seconds is normal, especially when you’re new to pumping, but it shouldn’t persist through the session.
Signs the Flange Is Too Small
When a flange is too small, your nipple rubs against the sides of the tunnel with each suction cycle. This friction causes pain during and after pumping, and over time it can lead to cracked skin, blanching (where your nipple turns white from restricted blood flow), and visible redness or irritation at the base of your nipple. You might also notice creasing or a line across your nipple after removing the flange, similar to a mark left by a tight ring on your finger.
A too-small flange also restricts milk flow. Because the nipple can’t move freely and the surrounding tissue is being compressed, the pump can’t create effective suction. If you’re pumping for a full session and getting less milk than expected, flange size is one of the first things to check.
Signs the Flange Is Too Large
An oversized flange pulls too much areola tissue into the tunnel alongside your nipple. This creates a different kind of discomfort: swelling and tenderness around the areola rather than at the nipple tip. You may see a large ring of areola tissue getting sucked in and out with each pump cycle, which stretches the tissue unnecessarily.
Like a too-small flange, a too-large one also reduces pumping efficiency. The suction spreads across a wider area instead of focusing on the nipple, so the pump struggles to draw milk effectively. Some people assume bigger is more comfortable, but an oversized flange can cause just as many problems as one that’s too tight. It can also contribute to clogged ducts because certain areas of the breast aren’t being drained properly.
How to Measure Your Nipple
Flange size is based on your nipple diameter, not your areola or breast size. To measure, use a ruler or printable sizing tool and measure straight across the base of your nipple (where it meets the areola) in millimeters. Do this after pumping or nursing, when your nipple is its fullest size, since nipples swell slightly with stimulation.
Once you have your nipple diameter, add roughly 2 to 4 millimeters to find your starting flange size. Flanges are available from 13mm all the way up to 40mm, and the sizing is universal across brands. A 20mm flange works the same whether it’s made by Spectra, Medela, or any other manufacturer. Here’s a general reference:
- 13 to 15mm nipple diameter: 15 to 17mm flange (extra small)
- 16 to 17mm nipple diameter: 18 to 20mm flange (small)
- 18 to 21mm nipple diameter: 21 to 24mm flange (medium, the standard size shipped with most pumps)
- 22 to 25mm nipple diameter: 25 to 28mm flange (large)
- 26mm+ nipple diameter: 30mm+ flange (extra large)
Most pumps come with a 24mm or 28mm flange in the box. Many people need something smaller, so don’t assume the included size is your size.
Your Size Can Change Over Time
Your nipple size isn’t fixed during breastfeeding. Nipples often swell in the early postpartum weeks as your milk supply regulates, then gradually change as your body adjusts. Many people find they need a different flange size at six weeks postpartum than they did in the first week. Some notice changes when their supply shifts, during hormonal fluctuations, or even between left and right sides (it’s common to need different sizes for each breast).
If pumping suddenly becomes uncomfortable after weeks of working fine, remeasure. Your nipples may have changed enough to need a different size. It can also help to measure at different times of day, since nipple size can fluctuate slightly depending on whether you’ve recently nursed or pumped.
Silicone vs. Hard Plastic Flanges
Standard flanges are rigid plastic, which holds its shape under suction and delivers consistent, direct pressure. These work well for most people but can feel unforgiving if you’re between sizes or have sensitive skin. Silicone flanges and silicone inserts are softer and more flexible, which some people find gentler and more comfortable, especially during longer sessions.
The trade-off is that silicone’s flexibility can slightly reduce suction efficiency for some people, meaning sessions take a little longer or output dips. Others find the improved comfort actually helps with letdown and they pump more. If rigid plastic flanges feel fine, there’s no need to switch. But if you’re experiencing friction or discomfort even with the right size, a silicone option is worth trying. Silicone inserts that fit inside your existing hard plastic flanges are an inexpensive way to test this without buying a whole new set.
Getting It Right Takes Trial and Error
Flange fitting isn’t always a one-and-done measurement. The sizing chart gives you a starting point, but the real test is what you see and feel during pumping. Watch your nipple in the tunnel during a session. Check that it moves freely without rubbing, that minimal areola tissue enters the tunnel, and that you’re not in pain after the first minute or so. Adjusting by even one size (a few millimeters) can make a noticeable difference in both comfort and output.
If you’ve tried multiple sizes and still can’t find a comfortable fit, a lactation consultant can observe a pumping session and identify the issue. Some people have elastic nipple tissue that stretches more than average during suction, which makes standard sizing guidelines less reliable. In those cases, a professional fitting helps pinpoint whether you need a smaller flange, a different material, or an adjustment to your pump’s suction settings.
