A 37.5 mg dose of tramadol is on the low end of the opioid spectrum, roughly equivalent to 7.5 mg of morphine taken by mouth. That puts it in the same ballpark as 30 mg of codeine, making it one of the milder prescription opioid doses available. It’s not a standalone dose you’d typically see prescribed on its own. Instead, 37.5 mg is almost always paired with 325 mg of acetaminophen in a combination tablet designed for short-term acute pain.
How It Compares to Other Pain Medications
The standard way to compare opioid strength is by converting any dose to its morphine equivalent. Tramadol has a conversion factor of 0.2, meaning each milligram of tramadol equals about one-fifth of a milligram of morphine. A 37.5 mg tramadol dose works out to 7.5 morphine milligram equivalents (MME). For context, a typical starting dose of immediate-release morphine is 15 mg, so 37.5 mg of tramadol delivers about half the opioid punch of that baseline morphine dose.
A head-to-head clinical trial compared tramadol/acetaminophen (37.5 mg/325 mg) tablets directly against codeine/acetaminophen (30 mg/300 mg) capsules in adults with chronic low back pain and osteoarthritis. Pain relief scores were virtually identical between the two groups, with relief kicking in within 30 minutes and lasting at least six hours. Patients in both groups used an average of 3.5 tablets per day. The tramadol combination caused less drowsiness (17% vs. 24%) and less constipation (11% vs. 21%) than the codeine combination, though headaches were slightly more common with tramadol.
Compared to over-the-counter options like ibuprofen or acetaminophen alone, the tramadol combination provides stronger relief for moderate pain. But the gap isn’t enormous. The combination tablet was specifically designed so the two ingredients work together: the tramadol kicks in faster than acetaminophen alone, and the acetaminophen extends the duration of relief beyond what tramadol alone provides.
Why 37.5 mg Exists as a Dose
You won’t find tramadol prescribed as a standalone 37.5 mg tablet. That dose exists specifically because of the combination product. Standard tramadol-only tablets come in 50 mg and 100 mg strengths. The 37.5 mg amount was calibrated to pair with 325 mg of acetaminophen in a single pill, creating a combination intended for short-term use of five days or less. The typical direction is two tablets every four to six hours, up to eight tablets per day. Two tablets deliver 75 mg of tramadol and 650 mg of acetaminophen per dose.
At one tablet (37.5 mg tramadol), the opioid component is quite mild. Most of the clinical studies that demonstrated the combination’s effectiveness used two-tablet doses, which is why single-tablet dosing may feel underwhelming for anything beyond mild pain.
How It Works in Your Body
Tramadol is unusual among opioids because it works through two different mechanisms. Part of its pain relief comes from activating opioid receptors in the brain, the same targets morphine hits. But tramadol also blocks the reabsorption of two brain chemicals involved in mood and pain signaling (norepinephrine and serotonin), which adds a secondary layer of pain relief that pure opioids don’t provide.
Here’s where it gets interesting: tramadol itself is only a weak opioid. Your liver converts it into a more potent active form that does the heavy lifting on opioid receptors. How effectively your body makes that conversion depends on a specific liver enzyme that varies significantly from person to person based on genetics. Roughly 5 to 10 percent of people of European descent are “poor metabolizers” who produce very little of the active form, meaning tramadol may barely work for them. On the other end, “ultra-rapid metabolizers” convert tramadol much more efficiently, experiencing stronger effects and a higher risk of side effects from the same dose. Research in cancer patients confirmed that people with reduced enzyme activity had significantly lower blood levels of the active pain-relieving form, shifting the drug’s metabolism down a different, less effective pathway.
This genetic variability means two people can take the same 37.5 mg tablet and have genuinely different experiences. If tramadol has never seemed to work well for you, or if it hits you harder than expected, your metabolizer status is likely the reason.
Where It Falls on the Risk Spectrum
At 7.5 MME per tablet, a single 37.5 mg dose sits well below the thresholds that raise clinical concern. CDC guidelines note that opioid risks increase continuously with dosage, with additional precautions recommended when a patient’s total daily opioid intake from all sources reaches 50 MME. Even at the maximum labeled dose of eight combination tablets per day (300 mg tramadol, or 60 MME), you’d be right around that threshold.
Tramadol is a Schedule IV controlled substance, reflecting its lower abuse potential compared to Schedule II opioids like oxycodone or morphine. That said, “lower” doesn’t mean zero. Physical dependence can develop with regular use, and withdrawal symptoms can occur if the drug is stopped abruptly after even a few weeks. The dual mechanism that makes tramadol unique also introduces a risk that pure opioids don’t carry: serotonin syndrome, a potentially dangerous reaction that can occur if tramadol is combined with antidepressants or other medications that raise serotonin levels.
The 37.5 mg dose, especially as a single tablet, carries modest risk for most adults. The combination product’s five-day limit exists partly because of the acetaminophen component (which can stress the liver with prolonged use) and partly to minimize the chance of opioid dependence developing from what’s meant to be a short-course medication.
