You can’t guarantee you’ll prevent diastasis recti, but you can significantly reduce the stress placed on your abdominal midline through how you move, breathe, and exercise throughout pregnancy. About half of women have a measurable separation at six weeks postpartum, and roughly 30% still have it a year after delivery. The separation happens when hormonal changes loosen the connective tissue running down the center of your abdomen while your growing uterus pushes outward against it. Some risk factors are outside your control, but daily habits make a real difference in how much strain that midline tissue absorbs.
What Actually Causes the Separation
Your two rectus abdominis muscles (the “six-pack” muscles) are connected by a band of connective tissue called the linea alba. During pregnancy, hormones increase the laxity of this tissue while the expanding uterus stretches it from behind. The combination of loosened connective tissue and sustained mechanical pressure causes the two muscle bands to drift apart. A gap of 2 centimeters or more at the midline is generally considered clinically significant.
The position of your baby matters too. An anteriorly positioned fetus concentrates tensile forces on the linea alba rather than distributing them evenly across the abdominal wall, which accelerates the separation. This is one reason why the condition can vary so much between pregnancies for the same person.
Risk Factors You Can and Can’t Control
Carrying twins places substantially more pressure on the rectus muscles than a singleton pregnancy, and each additional pregnancy increases your risk further. A higher BMI is a consistent risk factor because extra adipose tissue within the abdominal cavity adds outward pressure on the abdominal wall even beyond what the uterus creates. Diabetes also appears to weaken the rectus muscles over time through changes in muscle fiber composition, making the tissue more vulnerable to separation.
Maternal age, interestingly, has not been found to be a significant independent risk factor during the pregnancy itself. However, collagen in the linea alba naturally decreases as you age, which can make recovery slower in later years. The takeaway: if you have one or more of these risk factors, the movement and exercise strategies below become even more important.
Strengthen the Deep Core, Not the Surface
The most protective exercises during pregnancy target the deepest layer of your abdominal muscles, not the outer “six-pack” layer. Traditional sit-ups and crunches work the surface muscles and create a bulging, forward force on the linea alba. After 12 weeks of pregnancy, these movements should be off the table entirely. The same goes for any exercise that causes your belly to visibly dome or cone outward along the midline.
Deep core exercises, like modified planks held with proper form, have been shown to be more effective than traditional ab work at engaging the muscles that act like a corset around your midsection. The key is slow, controlled movement paired with intentional breathing. A simple starting point: lie on your back (in early pregnancy) or sit upright, place your hands on your lower belly, and exhale slowly while drawing your navel gently toward your spine. Hold for a few seconds, then release. This trains the deep abdominal layer to activate and support the midline without generating the outward pressure that worsens separation.
A pelvic floor physical therapist can assess your specific situation and give you a progression of exercises tailored to each trimester. This is especially worthwhile if you’re carrying multiples or have had diastasis recti in a previous pregnancy.
How You Breathe Matters More Than You Think
Most people breathe by pushing their belly outward on each inhale. During pregnancy, this pattern repeatedly loads the linea alba with outward pressure dozens of times per minute. A better approach is to breathe laterally, expanding your ribcage to the sides and back rather than letting your belly balloon forward. Place your hands on your lower ribs and practice feeling them widen on each inhale. This distributes the pressure of breathing across a wider area and keeps your deep core engaged.
Equally important: avoid bearing down, sometimes called the Valsalva maneuver. This is the breath-holding, pushing-down effort you might instinctively use when lifting something heavy, straining on the toilet, or getting up from a low position. It spikes internal abdominal pressure and drives force directly into the linea alba. Exhale through the effort instead.
Safe Lifting During Pregnancy
Clinical guidelines recommend a maximum of 51 pounds for an ideal lift during pregnancy, meaning infrequent, two-handed lifting of compact objects held close to your body without twisting, stooping, or reaching. In practice, most daily lifting should stay well below that number, and the technique matters as much as the weight.
Three rules to follow:
- Don’t lift from the floor. Bending forward deeply with your hands below mid-shin puts significant strain on both your back and your abdominal wall. If something is on the ground, squat down by bending your knees rather than hinging at the waist, or ask for help.
- Don’t lift overhead. As pregnancy shifts your center of mass forward, overhead lifting increases postural instability and forces your core to compensate in ways that load the midline.
- Exhale as you lift. Breathe out steadily through the exertion phase. Never hold your breath and push.
The Log Roll: Protecting Your Midline Every Day
Getting in and out of bed is one of the most common ways pregnant women unknowingly strain their abdominal midline. The typical sit-up motion to get out of bed creates exactly the kind of forward, crunching pressure you want to avoid. The log roll method eliminates this.
To lie down: sit on the edge of the bed, keep your trunk completely straight (imagine it as a plank of wood that can’t bend or twist), and use your arms to lower your upper body to the side while your legs rise onto the bed in a single, coordinated motion. To get up, reverse it: roll onto your side facing the edge, lower your legs off the bed, and use your arms to push your upper body upright. Your trunk stays straight throughout, and your abs never have to do the work of a sit-up.
This same principle applies any time you’re getting up from a reclined position, whether it’s a couch, a bathtub, or the floor. Roll to your side first, then push up with your arms.
Posture and Alignment Throughout the Day
As your belly grows, your lower back tends to curve inward more (an exaggerated anterior pelvic tilt) to counterbalance the weight in front. This posture pushes the abdominal contents forward and increases the resting tension on the linea alba all day long. Standing with your shoulders stacked over your hips, your ribs over your pelvis, and a gentle engagement of your deep core reduces this baseline load.
A few practical adjustments help: when standing for long periods, place one foot on a low stool to reduce the pelvic tilt. When sitting, use a small pillow or rolled towel behind your lower back to maintain a neutral spine. Avoid slouching, which compresses the abdomen, and avoid overarching, which stretches it. The goal is a neutral position where your deep core can do its job without the linea alba absorbing extra force.
What About Belly Bands and Support Belts
Maternity support belts can provide temporary relief by compressing and supporting the abdominal muscles, particularly in the third trimester when the load on the midline is highest. They can reduce discomfort during activity and may take some strain off the linea alba during tasks like walking or light exercise.
They’re a supplement, not a solution. Wearing a support belt for too long can actually weaken your abdominal muscles by doing their job for them, which is counterproductive. Use one during specific activities when you need the support, then take it off so your muscles stay active. A belt will not prevent diastasis recti on its own, and it’s no substitute for the deep core work and movement habits described above.
Putting It All Together
Prevention isn’t one big intervention. It’s a collection of small, daily choices: breathing into your ribs instead of your belly, rolling to your side before getting out of bed, skipping crunches in favor of deep core activation, exhaling through effort instead of holding your breath, and maintaining posture that keeps your trunk stacked and neutral. None of these habits are difficult individually. The challenge is making them automatic before your third trimester, when the mechanical load on your abdominal wall peaks. Starting these practices in the first trimester gives you months to build the muscle memory and core strength that will matter most when your belly is at its largest.
