How to Become a Radiology Nurse: Steps and Salary

Becoming a radiology nurse requires an RN license and hands-on clinical experience, with most nurses entering the specialty after working in another area of nursing first. The full path from starting nursing school to earning a radiology-specific certification typically takes four to seven years, depending on whether you pursue an associate or bachelor’s degree and how quickly you accumulate the required clinical hours.

Earn a Nursing Degree

You have two entry points: an associate degree in nursing (ADN), which takes about two years, or a bachelor of science in nursing (BSN), which takes four. Both qualify you to sit for the licensing exam, and both can lead to radiology nursing. The ADN is the fastest route to start working, but many hospitals prefer or require a BSN for specialty positions, and you’ll find more advancement opportunities with a four-year degree. Build the Right Clinical Foundation

Most radiology nursing positions are not entry-level. Hiring managers look for nurses who already have strong skills in IV access, patient assessment, sedation monitoring, and emergency response. The departments that build these skills fastest are the emergency department, intensive care unit, and post-anesthesia care unit. Even one to two years in a fast-paced acute care setting will give you a significant advantage when applying to radiology teams.

The reason is practical: radiology nurses work with less direct physician oversight than nurses on a hospital floor. You need to be comfortable placing IV lines, recognizing when a patient is deteriorating, and responding quickly when something goes wrong. Those instincts develop through high-volume clinical experience.

What Radiology Nurses Actually Do

The day-to-day work centers on preparing patients for imaging and procedures, administering contrast media or sedation, and monitoring patients throughout. Before any contrast injection, you screen patients for risk factors like kidney problems or prior allergic reactions, verify the correct patient and dose, and check that the IV line is functioning properly by flushing it with saline. During the scan or procedure, you watch for signs of an adverse reaction, which can range from mild hives to anaphylactic shock.

The variety is one of the specialty’s biggest draws. In a single shift, you might sedate a patient for a CT-guided biopsy, manage an allergic reaction to contrast dye, and assist during a complex interventional radiology procedure that involves hemodynamic monitoring similar to what ICU nurses do. Image-guided procedures like biopsies and radiofrequency ablations have increasingly replaced open surgeries, which means radiology nurses are involved in more and more of these minimally invasive treatments.

Diagnostic Imaging vs. Interventional Radiology

Within the specialty, the two broadest tracks are diagnostic imaging and interventional radiology (IR). Diagnostic imaging nursing covers CT, MRI, ultrasound, and fluoroscopy suites. The pace varies, but much of the work involves patient preparation, contrast administration, and post-procedure observation.

Interventional radiology is the higher-acuity side. IR nurses assist with catheter-based procedures, tumor ablations, and emergency interventions for patients with active bleeding. The skill set overlaps heavily with critical care nursing: you’ll titrate medications through IV drips, monitor blood pressure and heart rhythm in real time, and manage sedation for procedures that can last hours. If you thrive in high-stakes, fast-moving environments, IR tends to be the more compelling fit. The CRN certification exam reflects this emphasis, with interventional radiology accounting for 35% of the test content.

Learn Radiation Safety

Working around imaging equipment means regular exposure to low levels of radiation, so radiology nurses follow a principle called ALARA: as low as reasonably achievable. The CDC breaks this into three protective strategies: time, distance, and shielding. You minimize the time you spend near a radiation source, maximize your distance from it whenever possible, and use physical barriers like lead aprons, thyroid shields, and lead-lined glasses when you can’t step away.

You’ll also wear a dosimeter, a small device that tracks your cumulative radiation exposure over time. Your facility’s radiation safety officer reviews these readings regularly to make sure no one is exceeding safe limits. Most hospitals train new radiology nurses on these protocols during orientation, but understanding the basics before you apply signals that you’re serious about the specialty.

Earn the CRN Certification

The Certified Radiology Nurse (CRN) credential, issued by the Radiologic Nursing Certification Board, is the recognized specialty certification. It isn’t required to work in radiology, but it strengthens your resume, may increase your pay, and demonstrates mastery of the field.

To sit for the exam, you need a current RN license and at least 1,500 hours of radiology nursing practice within the past three years. That works out to roughly 18 to 24 months of full-time work in a radiology department. The exam itself is a three-hour, computer-based test with up to 150 multiple-choice questions. It covers five main areas in roughly equal weight: patient assessment and care planning, administering and monitoring treatments, patient and family education, safety and emergency management, and quality improvement. Responding to Contrast Reactions

One of the most critical skills in radiology nursing is managing adverse reactions to contrast media. Most reactions are mild: nausea, a warm flushing sensation, or a few hives. But severe reactions, though rare, can be life-threatening. These include airway swelling that causes stridor (a high-pitched breathing sound), bronchospasm with dropping oxygen levels, seizures, dangerous spikes in blood pressure, and full anaphylactic shock with plummeting blood pressure and racing heart rate.

Radiology nurses are often the first to recognize these reactions and initiate the response. That means securing airway and IV access, starting oxygen, positioning the patient with legs elevated if blood pressure drops, and alerting the responding physician. You need to know the emergency protocols cold, because these situations unfold in minutes. Hospitals keep crash carts and specific medications stocked in imaging suites for exactly this reason. This is also why prior experience in emergency or critical care nursing is so valued: you’ve already practiced responding to rapidly deteriorating patients.

Salary and Career Outlook

Radiology nursing pays well relative to general nursing. Median salaries vary by region, but figures in the mid-$90,000 range are typical, with higher-cost areas and experienced nurses pushing well above $100,000. Interventional radiology nurses, given the higher acuity of their work, often land at the upper end of the pay range. Earning your CRN certification and gaining IR experience are the two most reliable ways to increase your earning potential within the specialty.

Demand is steady and growing. As imaging technology advances and more procedures shift from open surgery to image-guided alternatives, hospitals need nurses who can work in these environments. Radiology departments that once relied primarily on technologists are increasingly staffing dedicated nursing teams, particularly in interventional suites where patients require sedation and complex monitoring.
