Whether a kidney stone in your dog can be broken up depends almost entirely on what type of stone it is. Some stones dissolve with a prescription diet over a few weeks. Others, particularly calcium oxalate stones (the most common type in dogs today), cannot be dissolved at all and require a physical procedure to remove or fragment them. Your vet will need to identify the stone type first, typically through imaging, urine analysis, or retrieving a sample for lab analysis, before choosing the right approach.
Which Stone Types Can Be Dissolved
Struvite stones are the most responsive to medical dissolution. These form in connection with urinary tract infections and can be broken down by feeding a prescription diet that’s lower in protein, magnesium, and phosphorus. The diet also acidifies the urine and includes added sodium to make your dog drink more water, which dilutes the urine. Complete dissolution typically takes one to two months, though it can take longer for larger stones. Your vet will monitor urine pH, aiming to keep it below 6.5, and will use imaging to check whether the stones are shrinking.
Urate stones can also be dissolved in many cases, as long as they aren’t caused by underlying liver disease. The approach involves feeding a low-purine diet, increasing water intake, and raising urine pH above 7. Dissolution tends to happen faster than with struvite, often within about a month.
Cystine stones respond to a low-protein, alkalinizing diet combined with a medication that helps break down the stone’s chemical bonds. Urine pH needs to reach above 7.5, and the process generally takes one to three months.
Calcium oxalate stones are the major exception. They cannot be dissolved with any diet or medication currently available. If your dog has calcium oxalate kidney stones, the only option is physical removal or fragmentation.
Shock Wave Lithotripsy
Extracorporeal shock wave lithotripsy (ESWL) is the closest thing to “breaking up” a kidney stone without opening your dog up surgically. The procedure uses focused shock waves generated outside the body to shatter stones into small fragments that can then pass naturally through the urinary tract. Your dog is placed on a specialized bed, and the shock waves travel through a water-filled cushion into the body, targeted precisely at the stone using fluoroscopy or ultrasound.
Success rates are encouraging. ESWL works in roughly 85% to 90% of dogs with calcium-containing kidney and ureteral stones, though about 30% to 50% of dogs need more than one session to fully fragment the stone. Each session involves somewhere between 1,000 and 3,500 shock waves. Dogs are placed under general anesthesia for the procedure.
ESWL works best for stones smaller than 10 millimeters. For stones between 10 and 25 mm, vets often place a ureteral stent alongside ESWL to help fragments pass. Stones larger than 25 mm may need a more invasive approach. Your dog isn’t a candidate for ESWL if there’s an uncontrolled urinary tract infection, a bleeding disorder, or an anatomical blockage downstream of the stone that would prevent fragments from passing.
The biggest limitation is availability. ESWL equipment is expensive and specialized, so it’s primarily found at veterinary teaching hospitals and large referral centers.
Laser and Surgical Options
For stones in the lower urinary tract, laser lithotripsy offers another minimally invasive route. A small scope is passed into the urinary tract, and a laser is used to break stones into fragments small enough to flush out. For kidney stones specifically, a newer percutaneous approach allows a device to be inserted through a small incision in the skin directly into the kidney. This device combines ultrasonic vibrations with a jackhammer-like ballistic force to break stones apart, and the fragments are then removed through the same access point.
Traditional surgery, called a nephrotomy, involves cutting into the kidney itself to remove stones. It’s effective but more invasive, and early studies in dogs reported a 20% to 50% reduction in kidney function afterward, though more recent research suggests the impact on function may be minimal in otherwise healthy kidneys. For bladder stones (a related but more common problem), a cystotomy is the standard procedure. Most dogs go home the same day or the next, and full recovery takes about two to three weeks.
Preventing Stones From Coming Back
Removing or dissolving stones is only half the job. Kidney stones in dogs have high recurrence rates, so long-term prevention matters just as much as the initial treatment.
For calcium oxalate stones, potassium citrate is the cornerstone of prevention. It works in three ways: it binds to calcium in the urine so there’s less available to form stones, it reduces crystal clumping, and it makes urine more alkaline, which lowers calcium excretion. Vets typically start dosing to reach a target urine pH near 6.5 and adjust from there. Your dog’s urine will need periodic testing to make sure the pH stays in the right range.
Hydration is equally important regardless of stone type. Dilute urine makes it harder for crystals to form and grow. Feeding canned or wet food instead of dry kibble is one of the simplest ways to increase your dog’s water intake. Some prescription diets also include extra sodium specifically to encourage drinking. Keeping fresh water always available and adding water to meals are low-effort strategies that make a real difference over time.
For struvite stones, preventing the urinary tract infections that cause them is the main strategy. For urate stones, a low-purine diet (which means limiting organ meats and certain proteins) reduces the raw materials that form the stones.
Signs a Stone Has Become an Emergency
Most kidney stones in dogs cause subtle or no symptoms for a long time. The situation becomes urgent when a stone moves into the ureter (the tube connecting the kidney to the bladder) and causes a blockage. If only one side is blocked and the other kidney is healthy, your dog may show little outward distress. But if both sides are blocked, or the other kidney is already compromised, signs of uremia develop quickly: vomiting, loss of appetite, lethargy, dehydration, and dropping body temperature.
The most dangerous complication is a rapid rise in blood potassium levels, which can cause the heart to slow dangerously or develop irregular rhythms. This can happen even without obvious cardiac symptoms, which is why vets check potassium levels in any suspected obstruction case. A complete urethral obstruction, where your dog is straining but unable to urinate at all, is a life-threatening emergency that needs immediate veterinary attention.
