To calculate your waist-to-height ratio, divide your waist circumference by your height. Use the same unit for both measurements (inches or centimeters). If your waist is 32 inches and you’re 66 inches tall, your ratio is 32 ÷ 66 = 0.48. The key number to remember: try to keep your waist measurement below half your height, which means a ratio under 0.5.
The Formula
The math is straightforward:

It doesn’t matter whether you use inches or centimeters, as long as both numbers are in the same unit. A person who is 170 cm tall with a 78 cm waist has a ratio of 0.46. That same person measured in inches (67 inches tall, 30.7 inches around the waist) gets the same result.
How to Measure Your Waist Correctly
Your waist measurement is the part most people get wrong, and it matters more than you might think. Breathing alone can shift your waist circumference by about 1 to 2 centimeters. Research using 3D body scanning found that waist girth is largest during normal resting breath and smallest after a full exhale, with differences of roughly 1 cm in women and 2 cm in men.
For the most consistent reading:
- Stand up straight with your feet together and your arms relaxed at your sides.
- Find the right spot. Wrap a tape measure around your midsection at the midpoint between the bottom of your ribs and the top of your hip bones. This is usually right around your navel, or slightly above it.

What Your Result Means
The UK’s National Institute for Health and Care Excellence (NICE) classifies adult waist-to-height ratios into three tiers:
- 0.4 to 0.49: Healthy central adiposity. No increased health risks from belly fat.
- 0.5 to 0.59: Increased central adiposity. Your risk for type 2 diabetes, high blood pressure, and cardiovascular disease starts to rise.
- 0.6 or above: High central adiposity. Further increased health risks.
The simplest way to remember this: your waist should measure less than half your height. A ratio below 0.5 is the goal.
Why This Ratio Matters More Than BMI
BMI tells you whether your overall weight is proportionate to your height, but it can’t distinguish between muscle and fat, and it says nothing about where your fat is stored. Two people with identical BMIs can have very different health risks depending on how much fat sits around their organs.
Waist-to-height ratio directly reflects central obesity, the fat packed around your midsection and internal organs. This visceral fat is metabolically active and drives inflammation, insulin resistance, and arterial damage in ways that fat stored on your hips or thighs does not. A study tracking over 2,700 adults with initially healthy hearts found that waist-to-height ratio was the only body measurement that independently predicted the development of early-stage artery calcification over five years. BMI did not.
The relationship goes beyond heart disease. Research on older adults found that a high waist-to-height ratio is associated with increased mortality from cardiovascular disease, cancer, respiratory disease, and even Alzheimer’s disease. In a Chinese cohort of older adults, increases in waist-to-height ratio over time predicted a 35% higher risk of developing multiple chronic conditions simultaneously. BMI-based screening missed that risk entirely.
About 63% of the damage linked to a high ratio works through familiar pathways like blood sugar, cholesterol, and blood pressure problems. But a substantial portion operates through other mechanisms, meaning that belly fat contributes to disease even in people whose standard blood work looks normal.
Ethnicity and the 0.5 Cutoff
The 0.5 threshold is a useful general guideline, but it may not be precise enough for everyone. A large analysis of U.S. adults from the National Health and Nutrition Examination Survey found that over 90% of both men and women across nearly all ethnic groups already exceeded the 0.5 cutoff, which suggests it may be too low to meaningfully distinguish risk levels in some populations.
Researchers identified more specific cutoffs for central obesity that varied by sex and ethnicity. For White adults, the thresholds were 0.57 for men and 0.58 for women. For Black adults, 0.55 for men and 0.57 for women. For Asian adults, 0.56 for men and 0.59 for women. For Hispanic adults, 0.57 for men and 0.59 for women. A study of over 29,000 adults in Colombia, most over 60, similarly found that a cutoff of 0.6 was more appropriate for older populations than the standard 0.5.
These numbers don’t change how you calculate the ratio. They simply mean that where the “danger zone” begins can vary depending on your background and age. The NICE thresholds (under 0.5 healthy, 0.5 to 0.59 increased risk, 0.6 and above high risk) remain a solid framework for most people.
Using This Ratio for Children
One advantage of waist-to-height ratio is that it works for kids too, without needing age-specific growth charts. NICE recommends measuring it in children aged 5 and older, using the same three-tier system as adults: under 0.5 is healthy, 0.5 to 0.59 signals increased risk, and 0.6 or above indicates high risk.
Research on Chinese children and adolescents found the ratio to be highly accurate at identifying both general obesity and metabolic syndrome. The optimal cutoffs in that population were slightly below the adult threshold: 0.47 for boys and 0.45 for girls. In practical terms, a child’s waist should measure comfortably less than half their height. The diagnostic accuracy was strong, correctly identifying over 85% of children with metabolic syndrome while also correctly clearing more than 80% of those without it.
Because children grow constantly, BMI percentiles require recalculation at every visit using age and sex charts. Waist-to-height ratio stays simple: measure, divide, compare to 0.5. That makes it especially practical for routine screening.
How Often to Check
NICE guidelines recommend that healthcare providers measure waist circumference and calculate waist-to-height ratio during routine health checks, GP registration, and consultations for long-term conditions, at least for adults with a BMI under 35. Above that BMI, waist-to-height ratio adds less useful information since central obesity is already very likely present.
You can also track it at home. All you need is a tape measure. Checking every few months gives you a simple way to monitor whether lifestyle changes are actually reducing the fat that matters most. Unlike weight on a scale, which fluctuates with water, muscle, and meals, waist-to-height ratio stays relatively stable day to day and tracks the specific type of fat most strongly linked to chronic disease.
