How to Care for Burn Blisters and Prevent Infection

Burn blisters form when the top layer of skin separates from the layer beneath it, and the space fills with clear fluid that cushions and protects the raw tissue underneath. The single most important thing you can do is leave that blister intact and keep it clean. Beyond that, proper cooling, the right dressing, and watching for infection will determine how well your burn heals.

Cool the Burn Immediately

Run cool (not cold) water over the burn for about 10 minutes. This limits tissue damage, reduces swelling, and eases pain. Use a gentle stream from a faucet or pour water from a container directly over the area.

Cold water and ice can actually make the injury worse. Extreme cold restricts blood flow to damaged tissue right when your body needs circulation the most to begin repairs. For the same reason, skip the frozen peas or ice packs. Butter, toothpaste, and other home remedies trap heat against the skin and introduce bacteria. Stick with plain, cool running water.

Why You Should Leave the Blister Intact

That bubble of fluid is not just swelling. It’s a biological dressing. The intact blister roof keeps bacteria out and moisture in, creating the ideal environment for new skin to form underneath. Popping or peeling it at home exposes raw tissue to infection and dries out the wound bed, which slows healing.

Clinical guidelines from burn care specialists recommend leaving blisters that are 5 mm (about the size of a pencil eraser) or smaller completely alone. Blisters on the palms of your hands or soles of your feet also benefit from being left intact because the skin in those areas is thicker and provides better natural protection. Even larger blisters are sometimes left untouched when infection risk from opening them outweighs the benefit.

Larger, tense blisters that restrict joint movement or cause significant pain are sometimes drained and “de-roofed” by a healthcare provider using sterile technique. This is not something to attempt yourself. The risk of introducing bacteria with a sewing needle or kitchen scissors is real, and the consequences of a burn wound infection can be serious.

How to Dress and Protect the Burn

Keep the burn covered for the first several days to protect it from friction, dirt, and bacteria. The goal is a dressing that stays moist without sticking to the wound. A non-adherent (non-stick) gauze pad works well. You can find these at any pharmacy, often labeled as “non-stick pads” or impregnated with a thin layer of petroleum. Secure the pad loosely with medical tape or a rolled gauze bandage.

Before applying the dressing, you can spread a thin layer of petroleum jelly (like Vaseline) or an antibiotic ointment over and around the blister. This keeps the area moist, which supports faster skin regeneration and reduces the chance of the bandage pulling on healing tissue when you change it. Medical-grade honey dressings are another option. Research shows they heal superficial burns as well as, or better than, several conventional treatments by maintaining a moist environment while also fighting bacteria.

Change the dressing once a day or whenever it gets wet or dirty. Each time, gently wash the area with mild soap and clean water, pat it dry, reapply ointment, and cover with a fresh non-stick pad.

Managing Pain and Swelling

Over-the-counter pain relievers are usually enough for a minor burn blister. Ibuprofen (Advil, Motrin) and naproxen (Aleve) are good choices because they reduce both pain and inflammation. Acetaminophen (Tylenol) helps with pain but won’t address swelling. Follow the dosing instructions on the package. Do not give aspirin to anyone under 19 years old.

Elevating the burned area above heart level when you’re resting also helps reduce swelling, especially in the first 48 hours. If the blister is on your hand or forearm, propping your arm on a pillow while sitting or sleeping makes a noticeable difference in how much the area throbs.

What Healing Looks Like

In the first day or two, expect the area around the blister to be red, swollen, and tender. This is your immune system responding normally. Over the next one to two weeks, the fluid inside the blister gradually reabsorbs. The blister roof may dry out and peel away on its own, revealing pink or reddish new skin underneath. This new skin is fragile and sensitive to sunlight, so keep it covered or apply sunscreen for several months to reduce the risk of permanent discoloration.

Some skin discoloration, peeling, and flaking at the burn site is completely normal during healing. Dead skin cells shed as your body replaces them with new tissue. Resist the urge to pick at peeling skin, as pulling it away before it’s ready can damage the healing layer beneath.

Signs of Infection to Watch For

Most small burn blisters heal without complications, but infection is the main risk. Check the burn each time you change the dressing and look for these warning signs:

  • Increasing redness that spreads outward from the burn rather than shrinking over time
  • Worsening pain after the first two days, rather than gradual improvement
  • Cloudy or greenish fluid inside the blister or leaking from the wound
  • A foul smell coming from the wound when you remove the dressing
  • Fever or chills that develop after the burn
  • Red streaks extending away from the wound toward your body, which can signal the infection is spreading

Any of these signs means the burn needs professional evaluation promptly.

Burns That Need Medical Attention

Not every burn blister can be managed at home. Seek professional care for burns that involve the face, hands, feet, genitalia, or any major joint (elbow, knee, shoulder, ankle). These locations carry higher risks of scarring, restricted movement, or complications.

Burns in children under 10 and adults over 50 warrant a lower threshold for getting medical help, because healing is slower and complications are more common in these age groups. The same applies if you have diabetes, a weakened immune system, or any condition that impairs wound healing.

Chemical burns and electrical burns, including lightning injury, always require professional evaluation regardless of how they look on the surface. Electrical burns in particular can cause deep tissue damage that isn’t visible. Any burn that wraps around an arm, leg, or finger also needs medical attention because swelling can cut off circulation.

If your burn blister is larger than about 3 inches across, or if you have multiple blisters from the same injury, it’s worth having a provider assess the depth and extent of the damage rather than guessing at home.
