Checking for rheumatoid arthritis (RA) involves a combination of recognizing specific symptom patterns, blood tests for antibodies and inflammation markers, and imaging to look at what’s happening inside your joints. No single test confirms RA on its own. Doctors piece together evidence from several sources to reach a diagnosis, and the process typically starts with the symptoms you describe.
Symptoms That Suggest RA
The earliest signs of RA are tenderness or pain in small joints, particularly in your fingers or toes, though larger joints like the knee or shoulder can also be involved. What sets RA apart from other joint problems is symmetry: it usually affects the same joints on both sides of your body. If your right hand’s knuckles are swollen and stiff, your left hand’s knuckles likely are too.
Morning stiffness is another hallmark, but the key detail is how long it lasts. With osteoarthritis, stiffness fades after a few minutes of movement. With RA, morning stiffness typically persists for an hour or longer before it starts to improve. Sometimes that prolonged morning stiffness is the very first symptom people notice, even before obvious swelling appears.
RA also tends to target specific joints. Its most common sites are the hands, wrists, and feet. Osteoarthritis often hits the joint closest to your fingertip, while RA usually spares that joint and instead affects the middle knuckles and the base of the fingers. If your joint pain follows that pattern, it’s a meaningful clue.
What Happens During a Physical Exam
When you see a doctor about joint pain, they’ll feel your joints for warmth, swelling, and tenderness. One common maneuver is the squeeze test: the doctor gently compresses the knuckles of your hand or the ball of your foot as a group. Pain during this squeeze suggests inflammation in those small joints and is a quick, reliable way to flag possible RA. The doctor will also check your range of motion and look for symmetrical involvement.
Guidelines from the UK’s National Institute for Health and Care Excellence recommend urgent referral to a rheumatologist if the small joints of your hands or feet are affected, if more than one joint is involved, or if three months or more have passed between your first symptoms and seeking care. That three-month window matters because early treatment significantly slows joint damage.
Blood Tests Used in Diagnosis
Two antibody tests form the backbone of RA blood work. The first is rheumatoid factor (RF), an antibody found in roughly 9 out of 10 people with RA. It’s sensitive, meaning it catches most cases, but it’s not very specific: about 1 in 4 people who test positive for RF don’t actually have RA, because RF can show up in other conditions and even in healthy older adults.
The second test looks for anti-CCP antibodies (sometimes called ACPA). This test is nearly as sensitive as RF, picking up about 88% of RA cases, but it’s much more specific at around 90%. That means a positive anti-CCP result is a strong signal pointing toward RA rather than something else. When both tests are run together, diagnostic accuracy improves further, reaching roughly 90%.
Inflammation Markers
Doctors also check two blood markers that measure general inflammation in your body: the erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP). Neither is specific to RA, but elevated levels confirm that an inflammatory process is active. In people with severely active RA, ESR values of 50 to 80 are common. These markers also help track disease activity over time once you’re diagnosed.
What If Blood Tests Are Negative?
Some people with RA test negative for both RF and anti-CCP antibodies, a situation called seronegative RA. This doesn’t rule out the disease. Doctors diagnose seronegative RA based on the overall picture: the pattern and number of joints involved, how long symptoms have lasted (especially if longer than six weeks), elevated inflammation markers, and imaging findings. The formal classification system used by rheumatologists assigns points across four categories, including joint involvement, blood antibodies, symptom duration, and inflammation levels. You can reach the diagnostic threshold even without positive antibody tests if enough other criteria are met.
How Imaging Helps
X-rays are often the first imaging test ordered, but they have real limitations in early RA. Standard X-rays can show joint erosion once it’s established, but they can’t detect the soft-tissue inflammation (synovitis) that drives the disease in its earliest stages.
Ultrasound and MRI are both more sensitive. Ultrasound can reveal inflamed joint lining as abnormal tissue inside the joint, and color Doppler mode shows increased blood flow to inflamed areas. It’s quick, widely available, and doesn’t involve radiation. However, ultrasound has trouble reaching certain joints due to probe positioning, particularly some of the smaller hand and foot joints and the wrist bones.
MRI is the most comprehensive tool. It detects synovitis, bone erosions, and bone marrow edema, a sign of inflammation inside the bone itself that often precedes visible erosion. Bone marrow edema appears as bright areas on certain MRI sequences and can predict where joint damage will develop next. Ultrasound cannot detect bone marrow edema at all, which makes MRI particularly valuable when early, aggressive disease is suspected. Both MRI and ultrasound outperform a hands-on clinical exam at finding inflammation that isn’t yet obvious from the outside.
How RA Is Distinguished From Osteoarthritis
Because joint pain is common and has many causes, part of checking for RA involves ruling out other conditions. The most frequent look-alike is osteoarthritis, and several features help separate the two. RA causes prolonged morning stiffness lasting an hour or more; osteoarthritis stiffness clears within minutes. RA affects joints symmetrically; osteoarthritis often doesn’t. RA targets the middle and base knuckles of the fingers; osteoarthritis favors the joints nearest the fingertips. And RA is driven by immune system dysfunction, so blood markers of inflammation and autoantibodies are typically present, while osteoarthritis is a wear-and-tear process that generally doesn’t produce those findings.
What the Diagnostic Timeline Looks Like
If you’re noticing joint pain and stiffness that fits the patterns described above, the typical path starts with your primary care doctor, who will do a physical exam and order blood work. If results are suggestive, or if your symptoms are strong enough on their own, you’ll be referred to a rheumatologist. The rheumatologist may order additional imaging and will apply the formal classification criteria to confirm or rule out RA.
From first appointment to diagnosis, the timeline varies, but most rheumatologists aim to confirm RA and begin treatment within weeks rather than months. The six-week symptom duration threshold is clinically significant: symptoms lasting longer than six weeks are scored higher in the diagnostic criteria and make RA more likely than a short-lived viral joint inflammation. If your symptoms have already persisted for three months or more before you seek care, that’s considered a reason for urgent referral, because joint damage in RA can begin early and is much harder to reverse than to prevent.
