The most reliable way to check your testosterone level is a simple blood draw, typically from a vein in your arm, done at a lab or clinic in the early morning before you’ve eaten. A single test isn’t enough for a diagnosis. Both the Endocrine Society and the American Urological Association require at least two blood draws on separate mornings to confirm whether testosterone is genuinely low.
What the Blood Test Measures
The standard first step is a total testosterone test, which measures all the testosterone in your blood, both the small amount floating freely and the larger portion bound to proteins. This is the most common test ordered and the one used to screen for low testosterone. A total testosterone level below 300 ng/dL is the cutoff the American Urological Association uses to support a low-T diagnosis, while the normal reference range spans roughly 193 to 824 ng/dL for adult men.
If your total testosterone comes back borderline or doesn’t seem to match your symptoms, your doctor may order additional tests. A free testosterone test measures only the unbound testosterone that your body can actively use, which represents about 2 to 3 percent of the total. A test for sex hormone-binding globulin (SHBG), the main protein that binds testosterone, helps estimate how much free testosterone you actually have. Certain conditions like obesity, liver disease, and aging can shift SHBG levels, making total testosterone alone a less reliable picture. Your doctor may also check luteinizing hormone (LH), which helps determine whether a low reading originates in the testes or in the brain’s signaling system.
How to Prepare for Accurate Results
Testosterone follows a daily rhythm, peaking in the early morning and dropping throughout the day. To get a meaningful reading, have your blood drawn before 10:00 AM, or within three hours of waking up. You should also fast beforehand, as eating can lower testosterone readings. Testing should happen when your sleep schedule has been stable for several days. If you’ve recently changed shift patterns or traveled across time zones, wait until your body has adjusted.
Certain supplements and medications can skew results. High-dose biotin (vitamin B7), commonly found in hair, skin, and nail supplements at doses of 5 to 10 mg or higher, can cause falsely elevated testosterone readings on standard lab equipment. If you’re taking a biotin supplement, stop it at least 48 to 72 hours before your blood draw. Opioid medications, corticosteroids, and some hormonal supplements can also affect testosterone levels themselves, not just the test accuracy. Let your doctor know about everything you’re taking.
Why You Need Two Tests
Testosterone levels fluctuate day to day based on sleep quality, stress, illness, and even whether you’ve exercised recently. A single low reading doesn’t necessarily mean you have a testosterone deficiency. The Endocrine Society’s clinical guidelines are explicit: clinicians should measure total testosterone on two separate mornings, both fasting, before making a diagnosis. This repeat testing catches the natural variation and prevents unnecessary treatment based on a temporary dip. If both results come back below 300 ng/dL and you have symptoms, that’s when a clinical diagnosis is made.
At-Home Test Kits
Several companies sell at-home testosterone kits that use saliva samples or a finger-prick blood spot. These are convenient but significantly less reliable than a standard lab blood draw. In one study, results from six different home testing kits varied by 26 to 41 percent in men. When compared against established clinical methods, the home kit results deviated by an average of 194 percent. Most home kits use an immunoassay method that performs especially poorly in the low testosterone range, which is precisely where accuracy matters most if you’re trying to find out whether your levels are actually low.
An at-home kit might give you a rough sense of where you stand, but no doctor will diagnose or treat low testosterone based on one. If you’re experiencing symptoms like persistent fatigue, reduced sex drive, difficulty concentrating, or loss of muscle mass, a clinical blood test is the path that leads to real answers.
What Happens After the Test
If your total testosterone is within normal range and you feel fine, there’s nothing further to do. If it’s borderline or low on the first test, your doctor will schedule a second morning blood draw, typically a week or two later. If both readings are low, the next step is usually a broader hormone panel, including free testosterone, SHBG, and LH, to figure out why levels are low and whether treatment makes sense.
Keep in mind that testosterone naturally declines with age, roughly 1 to 2 percent per year after age 30. A 55-year-old man will typically have lower levels than a 25-year-old, and that’s not automatically a medical problem. The diagnosis of testosterone deficiency depends on both the numbers and the presence of symptoms. Low numbers without symptoms, or symptoms without low numbers, don’t meet the clinical threshold on their own.
