A stuffy nose almost always feels worse at night, and there’s a straightforward reason: lying down removes gravity from the equation. During the day, mucus naturally drains down the back of your throat without you noticing. When you’re flat on your back, that drainage stalls, mucus pools in your sinuses, and blood flow increases to the nasal tissues, causing them to swell. The good news is that several simple adjustments can make a real difference before you resort to medication.
Why Congestion Gets Worse at Night
Your body handles mucus differently depending on your position. Upright, gravity pulls drainage downward and keeps your nasal passages relatively clear. Lying down eliminates that assist, so mucus accumulates and inflamed sinus tissue swells further. If you have any degree of sinus inflammation, whether from a cold, allergies, or chronic sinusitis, the effect is amplified.
Bedroom allergens play a role too. Dust mites live in mattresses, pillows, and bedding, and their waste particles are a top trigger for nasal congestion. You spend hours with your face inches from these surfaces, breathing them in continuously. Pet dander, mold, and pollen tracked indoors all concentrate in the bedroom and compound the problem overnight.
Elevate Your Head
The single most effective free fix is propping your head and upper body up while you sleep. This restores some of the gravitational drainage you lose when lying flat, reducing the amount of mucus that builds up in your sinuses. You don’t need a dramatic incline. An extra pillow or two works for most people, or you can place a foam wedge under your existing pillow for a more gradual slope. Some people slide blocks or risers under the head of the bed frame so the entire mattress tilts slightly, which is easier on your neck than stacking pillows.
Nasal Strips and Internal Dilators
Adhesive nasal strips (the kind you stick across the bridge of your nose) physically pull open the narrowest part of the nasal airway. Studies on external strips show roughly a 21% improvement in airflow through the nasal valve and a 27% reduction in nasal resistance. That’s enough for many people to notice a meaningful difference, especially if your congestion is partly structural rather than purely from swelling.
Internal nasal dilators, small silicone or plastic inserts placed just inside each nostril, go further. In one study of 30 people who reported nasal obstruction during sleep, internal dilators increased inspiratory airflow by 110% compared to no treatment. Research on both types of dilators has shown improvements in sleep quality, less dry mouth in the morning, reduced snoring, and fewer nighttime awakenings. They’re drug-free, reusable, and worth trying before reaching for a spray.
Saline Rinse Before Bed
Flushing your nasal passages with saline right before bed clears out mucus, allergens, and irritants that have accumulated during the day. You can use a squeeze bottle, neti pot, or premixed saline packets. The key is using distilled, sterile, or previously boiled water, never straight tap water. A rinse won’t reduce tissue swelling on its own, but it removes the material that triggers swelling and gives your other remedies a cleaner starting point. Many people find that combining a saline rinse with head elevation is enough to sleep comfortably through a mild cold.
Reduce Allergens in Your Bedroom
If your nose clogs up most nights rather than only when you’re sick, allergens are a likely culprit. A few targeted changes can cut your exposure dramatically:
- Encase your mattress and pillows in dust-mite-proof covers. These zip around the entire surface and create a barrier between you and the mites living inside.
- Wash bedding weekly in water heated to at least 130°F (54°C). Lower temperatures won’t kill dust mites.
- Keep pets out of the bedroom, or at minimum off the bed. Dander settles into fabric and persists for weeks even after the animal leaves the room.
- Run a HEPA air purifier in the bedroom with the door closed. This filters airborne particles continuously while you sleep.
These measures take a week or two to show full effect because existing allergen levels in fabric and carpet need time to drop. Stick with the routine before deciding it isn’t helping.
Humidity and Steam
Dry air thickens mucus and irritates already-swollen nasal tissue. A cool-mist humidifier in the bedroom keeps moisture levels in a comfortable range (roughly 40 to 50% relative humidity). Going higher than that encourages mold and dust mite growth, so a simple hygrometer is worth the few dollars to monitor the level.
A hot shower right before bed serves double duty. The steam loosens thick mucus and temporarily shrinks swollen tissue, giving you a window of easier breathing as you fall asleep. If a shower isn’t practical, leaning over a bowl of hot water with a towel draped over your head provides a similar effect in a few minutes.
What Works (and Doesn’t) at the Pharmacy
Decongestant nasal sprays containing oxymetazoline or phenylephrine work fast, typically within minutes. They constrict blood vessels in the nasal lining, which reduces swelling almost immediately. The critical limitation is time: using these sprays for more than three consecutive days can cause rebound congestion, a condition called rhinitis medicamentosa, where your nose becomes more blocked than it was before you started the spray. Reserve them for your worst nights, not as a nightly habit.
For oral decongestants, check the active ingredient. The FDA announced in 2024 that oral phenylephrine, the decongestant found in many “PE” branded cold products, is not effective as a nasal decongestant at recommended doses. An advisory committee unanimously agreed that the data don’t support its use. Pseudoephedrine, which is kept behind the pharmacy counter (you’ll need to show ID), remains effective. If you’ve been taking a shelf product labeled “PE” and wondering why it doesn’t help, that’s the reason.
Steroid nasal sprays like fluticasone are a better option for ongoing congestion from allergies. They reduce inflammation rather than just constricting blood vessels, so there’s no rebound risk with daily use. They take a few days of consistent use to reach full effect, so starting one the night you get stuffed up won’t provide instant relief.
Antihistamines help when allergies drive your congestion. Older types like diphenhydramine cause drowsiness, which can be a feature at bedtime but leaves some people groggy in the morning. Newer options like cetirizine or loratadine are less sedating and work well taken in the evening.
When Congestion Signals Something Else
Most nighttime stuffiness is caused by colds, allergies, or dry air and resolves with the strategies above. Certain patterns, however, point to something that needs professional evaluation. Congestion that is consistently worse on one side only is not typical of a standard cold or allergy and warrants a visit to an ear, nose, and throat specialist. The same applies to congestion accompanied by nosebleeds, vision changes, persistent severe headache, swelling or redness around the eyes, or congestion that simply doesn’t improve after weeks of appropriate treatment. These signs don’t necessarily mean something serious, but they do need a closer look to rule out structural issues or other conditions.
