How to Clear Up Cystic Acne: Home Remedies to Prescriptions

Cystic acne is one of the most stubborn forms of acne, and clearing it almost always requires more than what you can buy off a shelf. Unlike regular pimples that form near the skin’s surface, cystic acne develops deep in the dermis, the middle layer of your skin, where bacteria, oil, and dead skin cells trigger an intense inflammatory response. That depth is exactly why it hurts, why it lingers for weeks, and why it often needs professional treatment to fully resolve.

Why Cystic Acne Is Different

All acne starts the same way: oil and dead skin cells clog a pore. With cystic acne, bacteria get trapped inside that clogged pore and the resulting infection spreads deeper than a typical whitehead or papule. Your immune system reacts by sending inflammatory cells to the area, creating a swollen, painful lump beneath the skin. These cysts don’t come to a head the way surface-level pimples do, which is why squeezing them doesn’t work and usually makes things worse by pushing the infection deeper or rupturing the cyst wall under the skin.

The inflammation involved is significant enough that scarring is almost a given without treatment. Some degree of acne scarring may affect up to 95 percent of people with acne, and cystic lesions carry the highest risk because of how deep the damage goes. The most common types of scars left behind are icepick scars (narrow, deep pits), rolling scars (broad depressions with sloped edges), and boxcar scars (wider depressions with sharper edges). Treating cystic acne early and effectively is the single best way to prevent permanent scarring.

What You Can Do at Home

Over-the-counter products won’t cure cystic acne on their own, but they can support whatever prescription treatment you’re using and prevent new breakouts from forming. A simple, consistent routine matters more than layering on a dozen products.

In the morning, use a gentle cleanser to remove excess oil, follow with a lightweight non-comedogenic moisturizer, and finish with sunscreen rated SPF 30 or higher. This last step isn’t optional if you’re using any retinoid or acid-based product, since these ingredients increase your skin’s sensitivity to UV light. At night, cleanse again and apply your treatment products.

For active ingredients, benzoyl peroxide kills the bacteria that drive cystic inflammation, while salicylic acid helps unclog pores. You can use both, but start slowly to avoid drying out your skin. Despite what it might feel like, acne-prone skin still needs hydration. The American Academy of Dermatology recommends moisturizing daily even when you have active breakouts. Look for formulas with ingredients like glycerin, hyaluronic acid, niacinamide, or dimethicone, all of which hydrate without clogging pores.

One thing to avoid: picking, popping, or applying DIY treatments like toothpaste or lemon juice. Cystic lesions sit too deep for any of these to help, and the irritation can trigger more inflammation and increase scarring risk.

How Diet Plays a Role

There’s now strong enough evidence to say that what you eat can influence cystic acne. Two dietary factors stand out: high-glycemic foods and dairy.

High-glycemic foods, things like white bread, sugary drinks, chips, and pastries, cause rapid spikes in blood sugar. Those spikes trigger a cascade of hormonal signals that ramp up oil production and inflammation in the skin. Multiple studies have confirmed this link. One case-control study found that glycemic load and frequency of milk and ice cream consumption were both positively associated with acne severity. Another study of Italian subjects found that people who followed a Mediterranean-style diet, which is naturally low-glycemic and low in dairy, had significantly less acne than those eating a typical Western diet.

You don’t need to overhaul your entire diet overnight. Swapping refined carbohydrates for whole grains, eating more vegetables and lean protein, and cutting back on sugary processed foods can meaningfully reduce the hormonal triggers that feed cystic breakouts. Reducing dairy, particularly skim milk, is also worth trying for a few months to see if your skin responds.

Prescription Topical Treatments

When over-the-counter products aren’t enough, prescription-strength topicals are typically the next step. Adapalene, a topical retinoid, is one of the most effective options. It works by speeding up skin cell turnover so pores are less likely to clog, and it also reduces inflammation. A higher-strength prescription version (0.3%) showed a 61 percent reduction in inflammatory lesions over 12 weeks in clinical trials, along with a 51 percent reduction in non-inflammatory lesions.

Your dermatologist may also prescribe a topical antibiotic, usually combined with benzoyl peroxide to prevent antibiotic resistance. Azelaic acid is another option that targets both bacteria and inflammation while being gentler on sensitive skin. Expect any topical prescription to take 8 to 12 weeks of consistent use before you see meaningful results, and your skin may temporarily look worse in the first few weeks as deeper clogged pores come to the surface.

Hormonal Treatments for Women

Cystic acne in women often has a hormonal component, especially when breakouts cluster along the jawline, chin, and lower cheeks, or flare around your menstrual cycle. Two main hormonal approaches can help.

Spironolactone is a prescription medication that blocks the effects of androgens, the hormones that drive excess oil production. Clinical trials have shown it’s effective at doses of 50 to 100 mg daily, with some evidence that higher doses provide additional benefit. It’s only prescribed to women because of its hormonal effects. Results build gradually over several months, so patience is important.

Certain birth control pills can also help by regulating the hormonal fluctuations that trigger breakouts. Three specific oral contraceptives are FDA-approved for acne treatment: norgestimate/ethinyl estradiol (Ortho Tri-Cyclen), norethindrone/ethinyl estradiol (Estrostep), and drospirenone/ethinyl estradiol (Yaz). Not every birth control pill helps acne, and some can actually make it worse, so the specific formulation matters.

Isotretinoin for Severe Cases

For cystic acne that doesn’t respond to other treatments, isotretinoin (formerly known by the brand name Accutane) is the most powerful option available. It’s the only acne treatment that addresses all four causes of acne at once: excess oil, clogged pores, bacteria, and inflammation. It works by dramatically shrinking the oil glands, and its effects often last long after you stop taking it.

A typical course lasts five to seven months. The standard cumulative dose ranges from 120 to 150 mg per kilogram of body weight, though some patients who don’t respond fully may need doses of 220 mg per kilogram or higher to achieve lasting results. Higher doses have also been shown to significantly decrease the risk of relapse.

Isotretinoin requires regular blood monitoring. Current guidelines recommend testing liver function and fasting lipid levels at baseline, one month after starting, and every three months throughout treatment. The medication causes severe birth defects, so women of childbearing age must use two forms of contraception and take monthly pregnancy tests. Side effects are common and include very dry skin, dry lips, joint aches, and increased sun sensitivity. Most of these resolve after treatment ends.

Cortisone Injections for Individual Cysts

When you have a painful, swollen cyst that needs to go away fast, a dermatologist can inject a small amount of diluted corticosteroid directly into it. This is one of the quickest treatments available for cystic acne. Most people get noticeable pain relief within 24 hours, and the cyst typically flattens within two to three days.

These injections treat individual cysts, not the underlying condition, so they’re best used as a spot treatment alongside a longer-term plan. If you have an important event coming up or a cyst that’s been lingering for weeks, this is the fastest path to relief.

Building a Treatment Plan That Works

Cystic acne rarely responds to a single product or approach. Most people need a combination: a consistent daily skincare routine, one or two prescription treatments targeting different mechanisms, and dietary adjustments where relevant. The specific combination depends on the severity of your breakouts, your hormonal profile if you’re a woman, and how your skin responds over time.

The hardest part is the timeline. Nearly every effective treatment for cystic acne takes weeks to months to show results, and some options temporarily worsen breakouts before improving them. Sticking with a plan for at least three months before switching gives it a fair chance to work. If topicals and hormonal options don’t produce enough improvement after that window, isotretinoin is often the next conversation to have with your dermatologist, and for many people with persistent cystic acne, it’s the treatment that finally provides lasting clearance.
