How to Control High Blood Pressure in Old Age

Controlling high blood pressure after age 65 is both more important and more nuanced than at younger ages. The general target is below 130/80 mmHg for most adults, including older ones, though the approach to getting there requires careful attention to how aging changes your body and your response to treatment. The good news: a combination of dietary changes, regular movement, and, when needed, medication can meaningfully reduce the risk of heart attack, stroke, and early death.

Why Blood Pressure Behaves Differently as You Age

Aging changes the plumbing. Over decades, your arteries gradually stiffen and lose elasticity, partly from the slow buildup of plaque along artery walls. Stiffer arteries can’t expand and contract the way they once did, so each heartbeat pushes against more resistance. The result is a pattern doctors see constantly in older adults: the top number (systolic) climbs while the bottom number (diastolic) stays the same or even drops. This is called isolated systolic hypertension, and it’s the most common form of high blood pressure in people over 60.

This matters because treatments and lifestyle changes that work well for a 45-year-old with uniformly elevated blood pressure may need to be adjusted for someone whose systolic pressure is high but whose diastolic pressure is already low. Pushing both numbers down aggressively could cause dizziness or fainting, especially when standing up quickly.

The Blood Pressure Target for Older Adults

The 2025 guidelines from the American Heart Association and American College of Cardiology set an overarching goal of below 130/80 mmHg for all adults. For people 80 and older, the same threshold applies, but with an important caveat: treatment should begin when clinical judgment suggests the benefits outweigh the risks, and when it aligns with the person’s own goals for their care. That language exists because frailty, limited life expectancy, or the burden of taking multiple medications can shift the calculus.

Large clinical trials support aiming for these lower numbers even in older age. The SPRINT trial found that intensive blood pressure management (targeting a systolic pressure below 120) reduced major cardiovascular events by 29%. A follow-up analysis showed that even people with frailty benefited similarly from intensive control without a higher rate of serious side effects. So age alone is not a reason to accept higher readings.

Dietary Changes That Lower Blood Pressure

The single most effective dietary intervention is reducing sodium. Keeping intake below 2,300 milligrams per day is the standard recommendation, but dropping to 1,500 mg daily lowers blood pressure even further. For context, a single teaspoon of table salt contains about 2,300 mg of sodium, and most of the sodium in a typical diet comes from processed and restaurant foods rather than the salt shaker.

The DASH eating plan, developed specifically for blood pressure control, emphasizes foods rich in potassium, calcium, magnesium, and fiber. In practice, that means building meals around vegetables, fruits, whole grains, fish, poultry, beans, and nuts while choosing fat-free or low-fat dairy. Potassium is particularly important because it helps your kidneys flush out excess sodium. Bananas, sweet potatoes, spinach, and white beans are all high in potassium, though people with kidney problems should check with their doctor before loading up, since impaired kidneys can struggle to clear excess potassium.

Alcohol also plays a role. The American Heart Association recommends no more than two drinks per day for men and one for women. Even moderate drinking above these levels can raise blood pressure and interfere with medications.

Exercise That’s Safe and Effective

Regular physical activity lowers blood pressure independently of weight loss, though losing excess weight helps too. The goal is at least 150 minutes of moderate aerobic activity per week, or 75 minutes of vigorous activity. Walking, swimming, cycling, dancing, and even active gardening all count. If 30 minutes at once feels like too much, three 10-minute sessions throughout the day provide the same benefit.

Combining aerobic activity with some form of resistance training (light weights, resistance bands, or bodyweight exercises) appears to offer the most heart-healthy benefit. The key is consistency. Exercising most days of the week matters more than occasional intense workouts. If you haven’t been active, start slowly and build up over weeks rather than jumping to the full 150 minutes.

How Medications Work in Older Adults

When lifestyle changes aren’t enough, medication becomes necessary. Two classes of drugs are most commonly chosen as first-line options for older adults: calcium channel blockers and low-dose thiazide diuretics. Calcium channel blockers relax the walls of your blood vessels, making it easier for blood to flow. In older adults, they tend to cause fewer side effects like flushing and rapid heartbeat than they do in younger people. Thiazide diuretics help your kidneys remove extra sodium and water, reducing the volume of blood your heart has to pump.

Doctors typically start with low doses and increase gradually. This “start low, go slow” approach is especially important in older adults because their bodies process medications differently: kidneys clear drugs more slowly, and the cardiovascular system is more sensitive to sudden pressure drops.

Watching for Falls and Dizziness

One of the biggest risks of blood pressure treatment in older adults is orthostatic hypotension, a sudden drop in pressure when you stand up. It causes lightheadedness, blurred vision, or outright fainting, and it’s a risk factor for falls and fractures. This is worth taking seriously: a hip fracture in someone over 75 can be life-altering.

Research from the large ALLHAT trial found that over the long term, the choice of blood pressure medication didn’t significantly affect fall risk. However, one calcium channel blocker (amlodipine) was associated with a higher risk of falls during the first year of use compared to a thiazide diuretic or an ACE inhibitor. The short-term fall risk was roughly double. This doesn’t mean calcium channel blockers should be avoided, but it does mean the first few months after starting or changing medication require extra caution. Stand up slowly, hold onto something stable, and pay attention to any new dizziness.

Practical steps to reduce fall risk include staying well hydrated, avoiding standing for long periods in hot weather, and getting up in stages (sit on the edge of the bed for a moment before standing). If you notice consistent dizziness after starting a new medication, that’s a signal your dose or drug may need adjusting.

Monitoring Blood Pressure at Home

Home monitoring gives you and your doctor a much clearer picture than occasional office visits. A joint policy statement from the American Heart Association and American Medical Association recommends taking two readings at least one minute apart, both in the morning and in the evening, for a total of four readings per day. Do this for at least seven days (28 readings total), with a minimum of three days if seven isn’t feasible. Some guidelines suggest discarding the first day’s readings, since they tend to be less reliable.

Once your blood pressure is stable and well controlled for several months, you can scale back to monitoring one to three days per week. Use an upper-arm cuff rather than a wrist device, sit quietly for five minutes before measuring, and keep your feet flat on the floor with your arm supported at heart level. Small details like crossing your legs or talking during the reading can inflate results by several points.

Putting It All Together

Managing blood pressure in older age isn’t about any single intervention. The most effective approach layers several changes: cutting sodium below 2,300 mg (ideally 1,500 mg), eating more potassium-rich fruits and vegetables, walking or swimming most days, limiting alcohol, and taking medication if needed. Each of these individually lowers blood pressure by a modest amount, but combined, they can bring readings down substantially.

The pace of change matters too. Abrupt dietary overhauls or aggressive medication increases are harder to sustain and more likely to cause problems. Gradual, steady adjustments, tracked with regular home monitoring, give you the control to catch what’s working and flag what isn’t before it becomes a bigger issue.
