Summer heat forces your cardiovascular system to work significantly harder, which can cause blood pressure to swing unpredictably, sometimes dropping dangerously low and other times spiking in response to dehydration or stress. If you have hypertension, managing it in summer requires adjustments to hydration, exercise timing, diet, and how you handle your medications. The risks climb sharply when temperatures exceed 70°F with humidity above 70%.
Why Heat Changes Your Blood Pressure
When temperatures rise, your body opens up blood vessels near the skin’s surface to release heat. This process, called vasodilation, is controlled by two competing systems in your nervous system: one that constricts blood vessels and one that actively dilates them. In mild heat, both systems run simultaneously, and your blood pressure depends on which one is winning at any moment. As it gets hotter, vasodilation dominates, and blood pressure can drop.
That drop sounds like it might be welcome if you have high blood pressure, but it’s not straightforward. Your heart compensates by beating faster and pumping up to twice as much blood per minute as it would on a moderate day. This extra cardiac workload is risky on its own, especially if you’re already on medications that lower blood pressure or heart rate. And if you become even mildly dehydrated from sweating, blood volume drops, which can cause pressure to rebound upward or lead to dizziness and fainting.
Humidity makes everything worse. In dry heat, sweat evaporates and cools you. In humid conditions, sweat sits on your skin and does very little. Your body keeps trying harder, your heart keeps pumping faster, and the strain accumulates without the payoff of actual cooling.
How Summer Affects Blood Pressure Medications
Several common blood pressure medications interact with heat in ways that increase your risk. This doesn’t mean you should stop taking them, but you need to understand what’s happening so you can compensate.
- Diuretics (water pills) push fluid out of your body, which is exactly the opposite of what you need when you’re sweating heavily. They can cause dehydration, electrolyte imbalances, and reduce your sensation of thirst, so you may not realize you need water until you’re already in trouble.
- Beta blockers reduce your heart rate and can decrease sweating, impairing your body’s natural cooling mechanism. They also limit the dilation of surface blood vessels, trapping heat inside.
- ACE inhibitors and ARBs lower blood pressure and can reduce thirst sensation. Combined with heat-induced vasodilation, they increase the risk of blood pressure dropping too low, leading to fainting or falls.
- Calcium channel blockers further lower blood pressure and can contribute to electrolyte imbalances, compounding the effects of sweating.
Certain drug combinations are especially concerning. Taking an ACE inhibitor or ARB together with a diuretic, a very common pairing for hypertension, significantly increases your risk of heat-related harm. If you’re on one of these combinations, you may need adjusted dosing during prolonged heat waves. Talk to your prescriber before summer’s worst stretches arrive, not during them.
Stay Hydrated Without Overdoing Sodium
The instinct to drink more water in summer is correct, but plain water alone can dilute your electrolytes if you’re sweating heavily. Potassium, magnesium, and calcium all leave your body through sweat, and losing too much of any of them can destabilize blood pressure and heart rhythm. The challenge for people with hypertension is replacing these electrolytes without loading up on sodium.
The DASH diet, originally designed to lower blood pressure, is particularly well suited to summer eating. It centers on vegetables, fruits, whole grains, low-fat dairy, fish, poultry, beans, and nuts, all of which are naturally rich in potassium, calcium, magnesium, and fiber. Watermelon, cantaloupe, berries, leafy greens, bananas, and sweet potatoes are all potassium-dense foods that are in season during summer and easy to eat cold or in salads. The DASH approach keeps sodium under 2,300 milligrams a day while ensuring you’re replacing the minerals you lose through sweat.
Avoid sports drinks marketed for athletes unless you’re doing genuinely prolonged outdoor work. Most contain far more sodium and sugar than someone with hypertension needs. Coconut water or diluted fruit-based options are gentler alternatives. Eating water-rich fruits like cucumber and watermelon counts toward both hydration and electrolyte replacement.
Exercise Safely in the Heat
Physical activity is one of the most effective long-term strategies for controlling blood pressure, so you don’t want to abandon it just because it’s hot. But you do need to adjust when and how you move. When the heat index reaches 90°F or above, exercising outdoors requires extreme caution, and for people with hypertension, the safer choice is to move indoors entirely at that point.
If you prefer outdoor activity, early morning is the best window, typically before 8 or 9 a.m. Late evening can work too, but pavement and buildings radiate stored heat well after sunset in urban areas, so the air may be hotter than you expect. Avoid midday entirely from June through August in most of the U.S.
On days when you do exercise in warmth, reduce the intensity by about 20 to 30 percent from your normal effort. Walk instead of jog. Swim if you have access to a pool, as water naturally dissipates heat. Pay attention to warning signs like dizziness, nausea, a sudden headache, or a feeling that your heart is pounding harder than the effort warrants. These can signal that your blood pressure has dropped too low or that heat is overwhelming your body’s cooling capacity.
Keep Your Bedroom Cool at Night
Summer doesn’t just affect you during the day. Research published in the American Heart Association’s journal Hypertension found that high ambient temperatures in summer reduce daytime blood pressure through vasodilation but do not reduce nighttime blood pressure to the same degree. This creates a pattern called “nondipping,” where your blood pressure stays elevated during sleep instead of dropping by the normal 10 to 20 percent. Nondipping is an independent risk factor for heart attack, stroke, and kidney damage.
The mechanism behind this is a decrease in the stress hormone norepinephrine during daytime heat, which relaxes blood vessels during waking hours. At night, that compensation doesn’t carry over fully, so your body doesn’t get the cardiovascular rest it normally would during sleep. Keeping your bedroom temperature around 65 to 68°F with air conditioning or fans helps restore normal nighttime dipping. If you don’t have air conditioning, a fan combined with damp skin or a cool washcloth on pulse points (wrists, neck, temples) provides meaningful relief.
Practical Cooling Strategies That Protect Your Heart
The American Heart Association identifies air conditioning as one of the most effective interventions for reducing heat-related cardiovascular events. If your home doesn’t have it, spending even a few hours a day in air-conditioned spaces like libraries, shopping centers, or designated cooling centers makes a measurable difference. In humid conditions, fans paired with skin wetting are effective. In dry heat, a water mist spray works better because evaporation is more efficient.
Beyond cooling technology, a few habits go a long way. Wear light, loose-fitting clothing that allows sweat to evaporate. Limit alcohol, which acts as a diuretic and compounds dehydration risk. If you spend time outdoors, take breaks in shade every 15 to 20 minutes rather than pushing through continuous exposure. Monitor your blood pressure more frequently during heat waves, ideally morning and evening, so you can catch changes early. A sustained drop below your normal range can be just as dangerous as a spike, especially if you’re on medication that’s now overcorrecting in the heat.
Controlling hypertension year-round, not just during summer, is itself a protective strategy against heat-related cardiovascular problems. People whose blood pressure is well managed going into summer are far less vulnerable to the swings that high temperatures cause. Consistent medication, a potassium-rich diet, regular movement, and good hydration habits form the foundation. Summer simply demands that you pay closer attention to all of them.
