Most blisters heal on their own within three to seven days and don’t need any special treatment. The fluid-filled pocket beneath your skin is actually protective, cushioning the damaged tissue while new skin grows underneath. Your main job is to keep that natural barrier intact, prevent infection, and reduce the friction that caused the blister in the first place.
Leave It Intact When You Can
Unbroken skin over a blister acts as a natural seal against bacteria. As long as the blister isn’t causing significant pain, the best approach is to leave it alone. Your body will slowly reabsorb the fluid over several days, and the top layer of skin will dry out and peel off on its own once the fresh skin beneath is ready.
Cover the blister with a bandage or moleskin to protect it from further rubbing. Moleskin is a thick, soft cotton fabric with adhesive backing that works especially well on feet because it cushions high-friction areas. Cut a piece larger than the blister, or cut a donut shape around it so the padding absorbs pressure without pressing directly on the raised skin.
How to Safely Drain a Painful Blister
If the blister is large, in a spot where it’s likely to burst on its own, or painful enough to limit your movement, draining it yourself is reasonable. The key rule: leave the overlying skin in place. That loose flap of skin still protects the raw layer underneath.
Here’s how to do it:
- Wash your hands and the blister thoroughly with soap and warm water.
- Swab the blister with iodine or another antiseptic.
- Sterilize a sharp needle by wiping it with rubbing alcohol.
- Puncture the blister at several spots near its edge, not the center.
- Gently press the fluid out, keeping the top layer of skin flat against the wound.
- Apply a thin layer of petroleum jelly or similar ointment over the area.
- Cover it with a nonstick gauze bandage.
Change the dressing daily, reapplying ointment each time. If you notice a rash developing around the ointment, stop using it, as some people react to certain topical products.
Burn Blisters Are Different
If your blister came from a burn rather than friction, do not drain it. The American Burn Association specifically advises against breaking burn blisters because they protect the damaged skin as it heals. Run cool (not ice-cold) water over the burn for initial relief, cover it loosely, and avoid applying creams or home remedies directly to the wound. Burns with large blisters or burns covering a wide area need professional medical care.
Signs of Infection to Watch For
A blister that’s healing normally may look red at the edges and feel mildly tender, but those symptoms should improve each day, not worsen. Watch for these warning signs:
- Redness that spreads outward from the blister rather than staying contained
- Increasing pain instead of gradual improvement
- Pus (cloudy or yellowish fluid, distinct from the clear fluid inside a normal blister)
- Skin that feels warm or hot to the touch around the area
In rare cases, bacteria from a blister can trigger an infection in the lymph vessels. This shows up as red streaks extending away from the wound, sometimes spreading noticeably within just a few hours. Fever, chills, and general achiness can accompany this. Red streaks moving up your arm or leg from a blister need prompt medical attention.
Why Diabetes Changes the Approach
If you have diabetes, even a small blister on your foot deserves professional care. Diabetes can damage the nerves in your feet, which means you may not feel a blister forming or notice when one becomes infected. It also reduces blood flow, making wounds slower to heal and more vulnerable to infection. A blister that would be minor for most people can progress to a foot ulcer in someone with diabetes, and severe infections that don’t respond to treatment can lead to amputation. The CDC recommends that people with diabetes visit their doctor for any blister, sore, or wound on the feet rather than treating it at home.
Preventing Blisters in the First Place
Blisters form when repeated friction separates the upper layer of skin from the layer beneath it, and fluid rushes in to cushion the damage. Anything that reduces friction or moisture on your skin lowers your risk considerably.
Socks matter more than most people realize. Moisture-wicking synthetic or merino wool socks pull sweat away from your skin, while cotton socks trap it and increase friction. Some runners and hikers wear two thin sock layers so the friction happens between the socks rather than between sock and skin.
Lubricants create a slick barrier that prevents skin from catching against fabric. Petroleum-based or silicone-based balms work well for long runs or hikes. Anti-chafing sticks designed for athletes combine lubrication with moisture control. On the other end of the spectrum, talcum powder or foot-specific powders absorb sweat and keep skin dry. Antiperspirant spray on the soles of your feet can also reduce moisture buildup during intense activity.
Moleskin isn’t only for treating existing blisters. If you know a particular shoe rubs your heel or that long walks give you blisters on specific toes, apply moleskin to those spots before you head out. You can also stick it directly to the inside of a shoe where a seam or edge digs into your foot. Breaking in new shoes gradually, rather than wearing them for a full day right away, prevents most shoe-related blisters entirely.
