Your ears are largely self-cleaning, and most people never need a deep clean. The skin lining your ear canal slowly migrates outward at about 0.1 millimeters per day, carrying old wax, dead skin, and trapped debris toward the opening where it dries up and falls out. When that system works well, the best thing you can do is leave your ears alone. But sometimes wax builds up faster than it can clear itself, and that’s when a safe at-home approach or a professional visit makes sense.
Why Most Ears Don’t Need Deep Cleaning
Earwax exists for a reason. It traps dust, bacteria, and small particles before they reach your eardrum, and its slightly acidic composition discourages infections. The outward migration of ear canal skin acts like a slow conveyor belt, moving wax toward the outer ear continuously. For most people, this process handles everything without any intervention at all.
That said, some people genuinely produce more wax or have narrow, curved ear canals that slow the conveyor belt down. A single gene variant (in a gene called ABCC11) determines whether your earwax is wet and sticky or dry and flaky. Wet earwax is more common in people of European and African descent and is more prone to impaction because it can clump together and resist the natural outward drift. Dry earwax, more common in East Asian populations, tends to clear on its own more easily. Hearing aid and earbud use can also push wax deeper and block its exit route.
What Not to Do
Cotton swabs are the most common cause of traumatic eardrum perforations seen in emergency departments. In one survey of regular cotton swab users, nearly a third reported complications: 21% experienced ear pain or discomfort, about 11% developed worsened wax blockage, and 9% reported hearing loss or muffled hearing. The swab pushes wax deeper rather than removing it, packing it tighter against the eardrum. If the swab goes too far, it can scrape the canal lining, cause bleeding, or puncture the eardrum itself, which can lead to long-term hearing loss and recurrent middle ear infections.
Ear candles are another method to avoid entirely. The FDA has determined that ear candles are dangerous when used as directed, carrying a high risk of severe burns to the skin, hair, and ear. The agency also found that their labeling claims have no validated scientific evidence behind them. The residue you see inside a burned ear candle is from the candle itself, not from your ear. The FDA actively detains imported ear candles at the border.
Safe At-Home Wax Softening
If you feel fullness, mild hearing loss, or a plugged sensation and suspect wax buildup, the safest first step is softening the wax so your ear’s natural migration can clear it. You have several options:
- Over-the-counter drops: Products containing 6.5% carbamide peroxide are widely available. Tilt your head, place 5 to 10 drops into the affected ear, and keep your head tilted for several minutes to let the solution fizz and break down the wax. You can also use the included ear plug to hold the drops in place.
- Hydrogen peroxide: Standard 3% hydrogen peroxide from any pharmacy works similarly. Place a few drops in the ear, let it bubble for a few minutes, then tilt your head to drain. Stop using it if it causes pain or irritation.
- Mineral oil or olive oil: A few drops of either can soften hardened wax over the course of a few days, making it easier for your ear to push it out naturally. These are gentler options if peroxide-based drops feel too fizzy or uncomfortable.
Use softening drops for a few days before attempting any rinsing. Trying to flush out hard, dry wax that hasn’t been softened first is less effective and more likely to cause discomfort.
How to Rinse Safely at Home
After a few days of softening drops, you can gently rinse with a bulb syringe. Fill it with clean water warmed to body temperature, roughly 38 to 40 degrees Celsius (about 100 to 104 Fahrenheit). Temperature matters here. Water that’s too cool or too warm can trigger a caloric reflex, causing sudden dizziness or vertigo because the inner ear’s balance system reacts to temperature changes in the canal.
Tilt your head so the affected ear faces slightly downward over a sink or bowl. Gently squeeze the bulb to send a low-pressure stream of water into the ear canal, aiming along the canal wall rather than directly at the eardrum. Let the water drain out freely. You may need to repeat several times. If you feel any pain at all, stop immediately. Irrigation should never hurt.
Do not irrigate your ears if you have a history of ear surgery, a known or suspected hole in your eardrum, active ear pain, or any discharge coming from the ear. In those situations, water entering the middle ear through a perforation can cause serious infection.
What a Professional Deep Clean Looks Like
If home methods don’t resolve the blockage, or if you have symptoms like significant hearing loss, ear pain, ringing, or dizziness, a professional cleaning is the next step. These symptoms don’t always mean wax is the problem. There’s no way to confirm a wax blockage without someone actually looking into your ear canal, and other conditions can produce the same symptoms.
The most common professional technique is microsuction, where a clinician uses a small vacuum tip under magnification to gently pull wax out of the canal. It’s precise, doesn’t involve water, and works well even for ears with perforations or past surgery. A study of 159 patients found microsuction successfully cleared the wax in 91% of cases. The procedure takes a few minutes per ear and can be a bit noisy, but it’s generally quick and well tolerated.
Some clinics use professional-grade irrigation with pressure-controlled devices that start at the lowest setting and increase gradually only if needed. These are safer than home irrigation because the clinician can see what they’re doing and control the water pressure precisely.
Keeping Your Ears Clean Long-Term
For routine maintenance, all you really need to do is wipe the outer ear with a damp cloth after a shower. The outer folds and the very entrance of the canal collect visible wax and debris that you can clean without going deeper. Nothing smaller than your elbow, as the old saying goes, should go into the canal itself.
