Alcohol-related liver damage is notoriously difficult to catch early because the first stage, fatty liver, almost never causes symptoms. About 90% of heavy drinkers develop fatty liver, yet most have no idea anything is wrong until the damage has progressed. Detection relies on a combination of blood tests, imaging, and knowing which physical signs to watch for at each stage of disease.
Why Early Damage Is Easy to Miss
Fatty liver (steatosis) is the earliest form of alcohol-related liver disease, and it’s largely silent. Most people feel completely fine. When symptoms do appear, fatigue is typically the first one, and it’s vague enough that most people attribute it to stress, poor sleep, or aging. There’s no sharp pain, no yellowing skin, no obvious warning at this stage.
This is what makes proactive testing so important if you drink regularly. Waiting for symptoms means you’re likely waiting until the disease has moved past its most reversible stage.
Blood Tests That Flag Liver Stress
A standard liver panel measures enzymes that rise when liver cells are inflamed or damaged. The two most relevant are ALT and AST. In alcohol-related damage, AST tends to run higher than ALT, sometimes at a 2:1 ratio. This pattern helps distinguish alcohol-related injury from other causes of liver inflammation.
GGT is another enzyme your doctor can check. It’s inexpensive and sensitive to alcohol consumption, making it a useful screening tool. The catch is that GGT can also be elevated by obesity, diabetes, certain medications, and other liver conditions, so a high GGT alone doesn’t confirm alcohol as the cause. It’s most useful when combined with other markers and your drinking history.
Bilirubin, albumin, and clotting time (measured through prothrombin time) round out the picture. Rising bilirubin suggests your liver is struggling to process waste. Falling albumin means it’s losing the ability to make essential proteins. Prolonged clotting time signals that your liver can no longer produce enough clotting factors. These markers tend to shift later in the disease, so normal results don’t guarantee a healthy liver.
Imaging That Measures Fat and Scarring
A standard ultrasound can reveal fat buildup or an enlarged liver, but it’s not very precise for measuring how much damage exists. For more detailed staging, a FibroScan (liver elastography) provides two measurements in a single, painless session.
The first is a CAP score, which estimates how much fat has accumulated in the liver. A score below 238 dB/m is considered normal. Scores between 238 and 260 indicate mild fatty change (less than a third of liver cells affected), 260 to 290 reflects moderate fat (roughly a third to two-thirds), and anything above 290 means more than two-thirds of the liver contains fat.
The second measurement is liver stiffness, which correlates with scarring (fibrosis). Normal liver stiffness falls between 2 and 7 kPa. Results of 7 to 11 kPa indicate moderate scarring, 11 to 14 kPa means severe scarring, and 14 kPa or higher points to cirrhosis. This test has largely replaced liver biopsy as the first-line tool for staging fibrosis, since it’s noninvasive and takes only a few minutes.
Physical Signs You Can See
A doctor’s physical exam can pick up signs of advancing liver disease, though these tend to appear once damage is already significant. Spider angiomas are small blood vessel clusters shaped like tiny spiders, most common on the chest, back, and face. They fill with blood from the center outward when pressed. Palmar erythema, a persistent redness on the palms, is another visible marker.
Your doctor may also feel for liver enlargement or, paradoxically, a shrunken liver (a normal right lobe spans about 9 cm). An enlarged spleen and fluid shifting in the abdomen suggest the liver is struggling to manage blood flow and fluid balance. These findings are most reliable for detecting decompensated cirrhosis. Compensated cirrhosis, where the liver is scarred but still functioning, is much harder to catch on a physical exam alone.
Signs That Damage Has Become Severe
Advanced alcohol-related liver disease announces itself more clearly. Jaundice, a yellowish tint to the whites of the eyes and skin, signals that bilirubin is building up in the blood. Ascites, a swelling of the abdomen caused by fluid accumulation, is one of the most common complications of cirrhosis. Confusion, difficulty thinking, memory problems, personality changes, and disrupted sleep can all result from hepatic encephalopathy, where toxins the liver normally filters begin affecting the brain.
In acute alcoholic hepatitis, a sudden inflammatory flare triggered by heavy drinking, doctors use a scoring formula called the Maddrey Discriminant Function to assess severity. A score of 32 or higher identifies patients at high risk of short-term mortality. This isn’t something you calculate at home, but it’s worth understanding that alcoholic hepatitis can become life-threatening quickly, and it sometimes strikes people who didn’t realize their liver was already compromised.
How Much Drinking Creates Risk
There’s no perfectly safe threshold, but risk rises substantially with quantity and pattern. A large UK study of over a million women found that those consuming 15 or more drinks per week had roughly 3.4 times the cirrhosis risk compared to women having just one or two drinks weekly. Among women drinking seven or more per week, daily drinking carried about 60% higher cirrhosis risk than non-daily drinking of the same total amount. Spreading the same number of drinks across every day of the week appears to be harder on the liver than concentrating them into fewer occasions.
For context, a “standard drink” contains about 14 grams of alcohol: one 12-ounce beer, one 5-ounce glass of wine, or one 1.5-ounce shot of spirits. Many people underestimate how much they actually consume, especially with generous pours or higher-alcohol craft beverages.
The Liver Can Recover, but Timing Matters
Fatty liver is almost entirely reversible with abstinence. Research shows that liver function begins to improve in as little as two to three weeks after stopping alcohol. A 2021 review of multiple studies found that two to four weeks of abstinence by heavy drinkers was enough to reduce inflammation and bring down elevated liver enzymes. Partial healing of liver tissue can occur within that same window.
Fibrosis (scarring) is slower to reverse and depends on how advanced it is. Mild to moderate fibrosis can improve over months to years of abstinence, but cirrhosis represents permanent structural change. Even with cirrhosis, stopping alcohol prevents further damage and reduces the risk of complications like liver failure. The earlier you catch the damage, the more reversible it is, which is the strongest argument for getting tested before symptoms force the issue.
What to Ask Your Doctor
If you drink regularly and want to check your liver health, ask for a liver panel (including GGT), a complete blood count, and an abdominal ultrasound as a starting point. If anything comes back abnormal, a FibroScan can provide a more detailed picture of fat content and scarring. Be honest about how much you drink.
