Fixing a vitamin D deficiency typically involves a combination of higher-dose supplementation for several weeks, followed by a lower daily maintenance dose to keep your levels steady. Most people see measurable improvement in blood levels within four to six weeks of starting treatment. How aggressively you need to supplement depends on how low your levels are, which a simple blood test can tell you.
Know Your Numbers First
A blood test measuring 25-hydroxyvitamin D is the standard way to assess your status. The results are reported in nanograms per milliliter (ng/mL), and the cutoffs are straightforward:
- Below 12 ng/mL: Deficient, associated with bone diseases like rickets in children and soft bones in adults
- 12 to 19 ng/mL: Inadequate for bone and overall health
- 20 ng/mL or above: Generally adequate
- Above 50 ng/mL: Potential for adverse effects, especially above 60 ng/mL
These thresholds come from the National Academies of Sciences, Engineering, and Medicine. The Endocrine Society sets a slightly higher target of 30 ng/mL, which is why you’ll sometimes see different numbers depending on the source. Either way, the blood test gives you a clear starting point and tells you whether you need a mild bump or a more aggressive correction.
The Supplementation Protocol
For true deficiency, the standard approach recommended by the Endocrine Society is a loading phase: 50,000 IU of vitamin D taken once a week for eight weeks. This rapid correction brings your blood levels up to a healthy range relatively quickly. After that, you shift to a maintenance dose, typically between 1,160 and 2,200 IU daily depending on how deep your deficiency was and how your body responds.
If your levels are merely inadequate rather than truly deficient, you may not need the loading phase at all. A daily dose in the 1,000 to 2,000 IU range is often enough to gradually bring you into a sufficient range over a few months.
One thing worth noting: people with obesity may need significantly more vitamin D to reach the same blood levels. Research suggests they can require up to 40% more than people at a healthy weight, because vitamin D is fat-soluble and gets sequestered in fatty tissue rather than circulating in the blood.
Choose D3 Over D2
Vitamin D supplements come in two forms: D3 (cholecalciferol, the form your skin makes from sunlight) and D2 (ergocalciferol, derived from plants and fungi). D3 is the better choice. In a head-to-head study published in the Journal of Clinical Endocrinology & Metabolism, D3 raised blood levels nearly twice as effectively as D2. Daily D3 increased levels by an average of 9.2 ng/mL over 12 months, compared to just 6.1 ng/mL for D2.
D2 has another drawback: it actually suppresses your body’s natural D3 levels. Participants taking D2 saw their circulating D3 drop by roughly 12 ng/mL, partially undercutting the benefit of the supplement. D3 doesn’t cause this tradeoff. Whether you take your supplement daily or monthly doesn’t appear to make a significant difference in the final blood levels achieved, so pick whichever schedule you’ll actually stick with.
Take It With Fat
Vitamin D is fat-soluble, meaning it dissolves in fat rather than water. Your body absorbs it much more efficiently when you take it alongside a meal that contains some dietary fat. This doesn’t need to be a greasy feast. A handful of nuts, avocado on toast, eggs, or a meal cooked with olive oil all provide enough. Taking your supplement on an empty stomach with just water means a meaningful portion passes through without being absorbed.
Nutrients That Help Vitamin D Work
Your body can’t fully activate vitamin D without magnesium. The enzymes that convert vitamin D into its usable form and the protein that carries it through your bloodstream both depend on magnesium as a cofactor. If you’re magnesium-deficient (and many people are), high-dose vitamin D supplementation may not work as well as you’d expect. Good magnesium sources include dark leafy greens, pumpkin seeds, almonds, black beans, and whole grains. A magnesium supplement in the 200 to 400 mg range is another option.
Vitamin K2 also plays a supporting role. When you raise your vitamin D levels, your body absorbs more calcium from food, which is generally the point. But that calcium needs to go to your bones, not your arteries. Vitamin K2 activates proteins that direct calcium into bone tissue and away from blood vessel walls. Evidence suggests that combining vitamin D with vitamin K produces better results for bone health than either vitamin alone. Fermented foods, egg yolks, and certain cheeses are natural sources of K2, and it’s also available as a supplement.
Sunlight as a Source
Your skin produces vitamin D3 when exposed to UVB radiation from the sun. According to the German Federal Office for Radiation Protection, exposing your face, hands, and arms to sunlight two to three times per week for about half the time it would take your skin to begin turning pink is enough to support adequate production. At a UV index of 7 (a sunny summer day), that works out to roughly 12 minutes for someone with light skin.
The catch is that this only works when the UV index is high enough, which rules out most of the day during winter months at higher latitudes. Sunscreen, darker skin tones, cloud cover, and aging all reduce production further. Sunlight is a great supplement to your supplement, but if you’re already deficient, it’s rarely enough on its own to correct the problem, especially outside of summer.
Food Sources Worth Adding
Very few foods contain meaningful amounts of vitamin D, which is a big reason deficiency is so common. The richest natural sources are fatty fish like salmon, mackerel, sardines, and trout, which can provide 400 to 600 IU per serving. Cod liver oil is exceptionally high, delivering over 1,000 IU per tablespoon. Egg yolks contain smaller amounts, around 40 IU each. Many milk, orange juice, and cereal products are fortified with vitamin D, typically providing 100 to 150 IU per serving.
Relying on food alone is difficult if you’re trying to hit 1,500 to 2,000 IU daily. Dietary sources are best thought of as a complement to supplementation rather than a replacement.
When to Retest Your Levels
After starting supplementation, plan to recheck your blood levels at 6 to 12 weeks. A response to treatment is usually visible on lab work within four to six weeks, but waiting a bit longer gives a more stable reading. If your levels still aren’t where they should be, your dose can be adjusted, and you should retest again 6 to 12 weeks after any dose change.
Once you reach a sufficient level, you still need to maintain it. Vitamin D doesn’t stay in your system indefinitely. Stopping supplementation entirely, especially during winter, will typically cause your levels to drift back down over a few months. Most people settle into a long-term maintenance dose of 1,000 to 2,000 IU daily to keep their levels stable year-round.
Avoiding Too Much
Vitamin D toxicity is rare but real, and it only happens from supplements, not from sunlight or food. Blood levels above 50 ng/mL are where potential problems begin, and levels above 60 ng/mL carry a clearer risk. The main danger of excess vitamin D is too much calcium in the blood, which can cause nausea, kidney stones, and in extreme cases, kidney damage or heart rhythm problems.
The tolerable upper intake level set by the National Academies is 4,000 IU per day for adults. The 50,000 IU weekly loading dose used for true deficiency is a short-term therapeutic protocol, not a long-term strategy. Once you’ve corrected your levels, staying within the 1,000 to 2,000 IU daily range keeps virtually everyone in a safe and effective zone.
