Getting a dementia diagnosis typically involves several stages, starting with your primary care doctor and potentially progressing to specialist evaluations and brain imaging. The full process takes an average of 3.5 years from when symptoms are first noticed to a confirmed diagnosis, according to a UCL meta-analysis of over 30,000 participants. Understanding what each step involves can help you move through it faster and with less uncertainty.
Start With Your Primary Care Doctor
Your first step is a visit to your regular doctor. They’ll take a detailed history, review your medications, and run a short cognitive screening test. Commonly used screening tools include the Mini-Cog, the General Practitioner Assessment of Cognition, and an eight-question informant questionnaire designed to flag cognitive decline. These take only a few minutes and help your doctor decide whether further evaluation is warranted.
One important detail: your doctor will want to speak with someone who knows you well, such as a spouse, adult child, or close friend. People experiencing cognitive decline often have limited insight into how much their daily functioning has changed. A family member can provide the kind of specific, observable information that screening tests alone can miss.
What Family Members Should Prepare
If you’re accompanying a loved one, expect the doctor to ask about changes across several areas of daily life. These include memory (repeating questions, forgetting appointments, misplacing things), orientation (tracking the day of the week, navigating familiar places), and judgment (handling emergencies, managing finances). You’ll also be asked about changes in personality, language difficulties like trouble finding words, and whether the person can still manage household tasks, hobbies, and personal care independently.
Being specific matters more than being comprehensive. “She asked me the same question four times in an hour last Tuesday” is more useful than “her memory seems worse.” Write down concrete examples before the appointment, along with a rough timeline of when you first noticed changes and how quickly things have progressed.
Blood Tests to Rule Out Treatable Causes
Before assuming symptoms point to dementia, your doctor needs to rule out conditions that mimic it but are reversible. Standard blood work checks for vitamin B12 deficiency, thyroid problems, anemia, diabetes, and liver or kidney disease. All of these can cause memory problems and confusion that look like dementia but improve with treatment.
Depending on your situation, your doctor may also order a toxicology screen, a urinalysis, or tests for infections known to affect the brain, such as HIV or syphilis. Vascular dementia shows a different pattern: white matter damage in deeper brain regions, often with evidence of small strokes. In some cases, a more specialized scan called a PET scan may be ordered. PET imaging measures how actively different brain regions are using energy. Each dementia type produces a characteristic pattern of reduced activity. In Alzheimer’s, the back of the brain loses activity first. In Lewy body dementia, the visual processing areas are affected early, producing what radiologists call an “occipital tunnel” sign. These patterns help doctors distinguish between dementia types when MRI alone isn’t conclusive.
Neuropsychological Testing
If your brief screening scores are normal but cognitive problems are still suspected, or if your doctor needs a more detailed picture of which mental functions are affected, you may be referred for a full neuropsychological evaluation. This is a much more thorough assessment than the quick office screening. It tests memory, attention, processing speed, reasoning, problem-solving, language use, reading comprehension, and motor skills through a combination of writing, drawing, puzzle-solving, answering questions, and computer-based tasks.
The results create a detailed cognitive profile. This matters because different types of dementia affect different abilities first. Someone with Alzheimer’s will typically show early memory deficits, while someone with frontotemporal dementia might show personality and language changes with memory relatively intact. The pattern of strengths and weaknesses helps narrow the diagnosis and can also establish a baseline for tracking progression over time.
When You’ll Be Referred to a Specialist
Many dementia diagnoses can be made by a primary care doctor, but certain situations call for a specialist referral to a neurologist, geriatrician, or memory clinic. Rapid symptom onset, where noticeable decline happens over weeks or months rather than years, raises the possibility of rarer and sometimes treatable causes and should prompt an urgent referral. You should also expect a referral if you’re under 65 (early-onset dementia takes an average of 4.1 years to diagnose and often requires more specialized evaluation), if screening tests are normal but something still seems off, or if the type of dementia is unclear.
Biomarker and Genetic Testing
For cases where the diagnosis remains uncertain, cerebrospinal fluid (CSF) analysis can measure proteins associated with Alzheimer’s disease. A spinal tap collects a small sample of fluid, which is tested for ratios of specific proteins. Abnormal ratios of tau and amyloid proteins are a strong indicator of Alzheimer’s pathology, even before symptoms become severe. This test is not routine for every patient but can be valuable when distinguishing Alzheimer’s from other dementia types matters for treatment decisions.
Genetic testing, particularly for the APOE gene, is sometimes discussed but serves a different purpose. It identifies a risk factor, not a diagnosis.
